One (1) long brown envelope ADDITIONAL REQUIREMENTS FOR TRANSFER STUDENTS Original & photocopy of Transcript of Records Incoming Sophomore No grade below 80% Original Honorable Dismissal or Transfer Credential Subject description (for crediting of subject) The applicant is previously enrolled in only one school in her entire freshmen year ADMISSION PROCEDURE 1. Present the requirements to the Academics Office and, if qualified, secure the Permit to take the Entrance Examination. 2. Pay the application and testing fee to the Finance Office. 3. Present the official receipt of payment at the Guidance Office; secure a testing form and an information form for your Personal Data File. 4. Complete the testing form to secure the examination permit and testing schedule. 5. Bring and submit on the examination day the completed filled-out Personal Data File and recommendation form secured from the Guidance Office. 6. Take the entrance examination at the Guidance Testing Center. 7. After taking the entrance examination, get the schedule of your medical and dental check-up and interview from the test administrator. 8. On the scheduled date of your interview, submit yourself for medical and dental examination at the school clinic. 9. Proceed to the Program Chairperson’s Office for the interview (with parent or guardian). 10. Secure permit to enroll from the interviewing officer/Program Chairperson if you pass the interview. 11. Present permit to enroll at the Registrar’s Office for Initial Enrolment. 12. Pay the Initial Enrolment Fee at the Finance Office. Initial Enrolment Fee is non-refundable but is deductible from the total fees. 13. Return the Permit to Enroll form at the Registrar’s Office to confirm your enrolment.
