Community Care Nutrition is an ACT Health community based service for adults who want to improve their nutrition and health. Our service has Accredited Practising Dietitians and Nutrition Assistants who see clients individually or as part of a group education session. There is no fee for our services.

What do I need to bring to my appointment?

If applicable, please bring any recent blood test results and a letter from your GP with your medical history and a 2–3 day food diary.

Group Education

Our service offers group education programs for people needing help to manage their weight.

Adult Healthy Weight Introduction Session • This is a one-off 2 hour session.• Provides the latest healthy eating tips for a

healthy weight• Explains other suitable nutrition, physical

activity and counselling services.

Who can come to our service?

Our service is available for ACT residents who are 25 years or older (except for enteral fed clients who must be 18 and over) and who are not receiving dietetic advice elsewhere.

If for medical reasons you are housebound and unable to come to a health centre, a limited home visiting service is available.

How can we help you?

The Community Care Nutrition Service offers dietary assessments, advice or counselling for nutrition related needs that are associated with medical conditions such as:

• Minor gastro intestinal conditions such as constipation, diarrhoea and Irritable Bowel Syndrome

• Malnutrition where people lose weight without trying or are eating poorly.

• Chronic diseases like Kidney Disease (CKD), Arthritis, Parkinson’s Disease

• Home enteral nutrition (tube feeding) service for people who need oral nutrition support or are tubefed at home

• Wound healing• Overweight or obesity (Adult Healthy

Weight Service)• Cardiovascular disease including hypertension

and high cholesterol levels.
