I. CALL TO ORDER Chair Hirose called the meeting to order at 10:00 am.

II. MINUTES OF PREVIOUS MEETING The minutes of the July 22, 2015 meeting were reviewed and approved.

III. CHAIR REPORT A. Advisory Board Membership Update

Chair Hirose provided an update on Center Advisory Board membership. Margaret Crane is retiring from her position as CEO at Barlow Respiratory Hospital.

Amit Mohan, Ms. Crane’s successor at Barlow, will complete the last year of Ms. Crane’s Advisory Board term. Past-Chair David Brown presented Ms. Crane with an award commemorating her service on the Advisory Board and expressed his gratitude for her faithful service.

The nominating committee presented five recommendations for the 2016 CPAC

Advisory Board: Monique Bedi, Lodi Memorial Hospital; Laura Biscaro, Santa Barbara Cottage Hospital; Patricia Bishop, Ojai Valley Community Hospital; Trudy Johnson, Santa Clara Valley Medical Center; and, L. Edwards, Loma Linda University Medical Center.

All five nominees were unanimously approved by the Advisory Board.

B. CHA Board of Trustees

Chair Hirose provided an update on the most recent meeting of the CHA Board of Trustees.

IV. VICE PRESIDENT REPORT

A. CHA Case Management Committee

Staff Blaisdell provided an update on the development status of the CHA Case Management Committee. The first Case Management Committee meeting was held on September 9, 2015. A mission statement was drafted and operating guidelines were established. The next Case Management Committee meeting will be held in the first quarter of 2016. Todd Cook agreed to serve as a liaison between the Case Management Committee

and the Post-Acute Care Advisory Board.

B. Annual Meeting Staff Blaisdell provided an update on the planning activities for the 2016 Annual Meeting, which will be held at the Crowne Plaza in Redondo Beach on February 11-12, 2016. Staff Blaisdell will distribute a call for new and innovative programs to present at

the Annual Meeting.

C. Advisory Board Member Survey

Staff Blaisdell reported on the 2015 member survey of the effectiveness of the CHA Center for Post-Acute Care.

Staff Blaisdell will look into restructuring the Advisory Board meeting at the Annual Meeting to make it more retreat-like, using the top three issues identified in the survey as a basis for topics of discussion.

Staff Blaisdell will work with Board leadership to identify mentors for the five new Advisory Board members.

Staff Blaisdell will conduct a survey of forum members regarding the effectiveness of the provider forums.

Staff Blaisdell will provide forum chairs with a list of members on the forum list serves.

D. Discussion Board

Staff Blaisdell reviewed current development of the CHA Center for Post-Acute Care website and discussion board. Staff Blaisdell will include a brief overview of this new feature at the Annual

Meeting.

E. Presentation Information Staff Blaisdell provided an overview of the IMPACT Act presentation. Members are encouraged to utilize this presentation within their hospitals as they see fit. As schedule allows, Staff Blaisdell will conduct individual hospital, or regional roundtable, presentations, as well. Staff Blaisdell will distribute the original PowerPoint to Advisory Board

members.

F. CPAC Congressional Education Day Update Staff Blaisdell provided an update on planned activities for the 2015 CPAC Congressional Education Day. Staff Blaisdell will arrange a preparatory conference call between Alyssa Keefe

and those participating in this event.

V. State Issues

A. Workplace Violence Staff Blanchard-Saiger provided an update on the Healthcare Workplace Violence Prevention regulations. CHA is preparing both a guidebook and a webinar on the new regulations. Members are encouraged to monitor the CHA website for draft regulations and updates on this topic. Staff Blanchard-Saiger will also be hosting regional roundtables, in conjunction with the regional associations, once the regulations are finalized.

B. Coordinated Care Initiative Staff Blaisdell provided an update on implementation of the Coordinated Care Initiative, including CHA’s comment letter to the Department of Health Care Services on the Continuity of Care Dual Plan Letter.

C. State Legislation Staff Glaser provided an overview of state bills of interest to post-acute care providers.

Five new members have been appointed to the advisory board of the CHA to begin their services in 2016. The new appointees are Monique Bedi, Acute Physical Rehabilitation Coordinator, Lodi Memorial Hospital, Laura Biscaro, Director of Care Management, Santa Barbara Cottage Hospital, Patricia Bishop, Director of Nursing, Ojai Valley Community Hospital, Lyndon Edwards, Vice President and Administrator, Loma Linda University Medical Center. Additionally, Amit Mohan, CEO, Barlow Respiratory Hospital, will assume the board position previously held by Margaret Crane. Pamela Chevreaux, Vice President for Ambulatory Services, Long Beach Memorial Medical Center and current chair-elect of the advisory board, will assume to role of Chair for 2016. Adam Darvish, Senior Vice President, Hospital Division, Kindred Healthcare has been appointed to serve as 2016 chair-elect.

B. Congressional Education Day

The Center for Post-Acute Care held a Congressional Education Day on October 27, 2015 in Washington, D.C. The purpose of the visit was to provide information to Members of the CA Congressional delegation and their staff regarding the role and contribution of post-acute care services in health care delivery and health care reform, and to advocate for effective policy regarding the delivery of post-acute care services. Several members of the Center’s Advisory Board participated in the event, which also included meetings with staff at the Centers for Medicare & Medicaid Services (CMS) to discuss key patient care issues, such as CHA member experiences with Cal MediConnect/Coordinated Care Initiative, acquisition of durable medical equipment (DME) under the Medicare Competitive Bidding Program, and implementation of the Improving Medicare Post-Acute Care Transformation (IMPACT) Act of 2014.

C. DP/SNF Medi-Cal Rate Reductions

CHA continues to advocate for the elimination of the “clawback,” or retroactive recoupment of reimbursement for services provided by distinct part skilled nursing facilities for the time period from June 1, 2011 to September 30, 2013 (August 31, 2013 for rural facilities). CHA has proposed a potential settlement to DHCS and continues to engage in direct discussions with bot HHS and the Department.

DHCS has indicated it plans to proceed with implementation of recoupment in April 2016. CHA continues to seek information from DHCS regarding recoupment in special circumstances, and review of individual facility requests for exceptions based on impact on beneficiary access.

D. Post-Acute PPS

HHA PPS The final rule for the home health prospective payment system (HH PPS) was published in the November 5 Federal Register. The updates are estimated to reduce home health payments by a net $260 million, or negative 1.4 percent, in CY 2016. In addition, the rule implements a significant new proposal to test a home health value-based purchasing model in nine states, not including California, as well as updates to the home health quality reporting program.

E. Post-Acute Quality Measures

The Centers for Medicare & Medicaid Services (CMS) released its list of quality measures under consideration for future rulemaking on November 27, including several measures for post-acute care services, such as potentially preventable readmissions during and after post-acute care stays, discharge to community, and others. The Measure Applications Partnership (MAP) will review the measures during a post-acute workshop to be held December 14-15. CHA has provided input to the measures throughout the development process and will attend the in-person meetings.

F. CHA Center for Post-Acute Care Annual Meeting

The annual meeting of the CHA Center for Post-Acute Care, “Break the Mold, The New Shape of Post-Acute Care,” will be held on February 11 & 12, 2016 in Redondo Beach, CA. Topics include the role of post-acute care services in population health management and bundled payment, care transitions, regulatory updates and more.

G. Center Forums

The Center for Post-Acute Care has established several member forums for member provider groups, including 1) inpatient rehabilitation facilities and therapy services, 2) skilled nursing facilities and subacute care units, and 3) home health agencies and hospices. The goals of these forums, which meet regularly by phone and web, are to provide an opportunity for members from specific post-acute provider groups to receive timely information regarding issues of interest, and to provide input to Center activities. Participation in the forums is open to employees of CHA institutional members.
