AFFIDAVIT OF DISCREPANCY

I, ROSEMARIE R. AGNILA, Filipino, of legal age, widow, and aresident of New Panay, Aleosan, Cotabato, after having been sworn to inaccordance with law, do hereby depose and state, THAT ---

1. I am the wife of the Late SSg. Roger G. Agnila who died last August21, 2011;

2. I am applying for a Death Claim at GHQ, AFP, Camp EmilioAguinaldo, Quezon City;

3. As one of the requirements, I have a copy of our NSO duly-authenticated Certificate of Marriage with Registry No. 97-445registered at the Local Civil Registry of Midsayap, Cotabato;

4. However, in our Certificate of Marriage, the Middle Initial of myfather in-law was erroneously written as letter R instead of letter Pfor Penaflor, as evidenced in my late husbands Certificate of Birth

which is also authenticated by NSO;

5. Also, in my NSO duly-authenticated Certificate of Live Birth withLocal Civil Registry No.181 registered at the Local Civil Registry ofLibungan, Cotabato, the last name of my mother was erroneouslywritten as APOLINAR instead of POLINAR;

6. The entries sought to be corrected is clerical error and innocuous,and not substantial which would otherwise affect civil status,citizenship, or nationality;

I am executing this AFFIDAVIT to attest to the truthfulness of theforegoing facts, in support for my application for Death Claim at GHQ,AFP, Camp Emilio Aguinaldo, Quezon City, and for whatever legalpurpose it may serve.

1. I am applying for a Passport at the Department of Foreign Affairs;

2. I have an NSO duly-authenticated Certificate of Live Birth with

Serial No. 97-00187 registered at the Local Civil Registry ofBadiangan, Iloilo;

3. However, in my Certificate of Live Birth, under Box No. 11: Ageof my mother, it was erroneously written as 36, instead of 26;

4. The entries sought to be corrected is clerical error and innocuous,and not substantial which would otherwise affect civil status,citizenship, or nationality;

I am executing this AFFIDAVIT to attest to the truthfulness of theforegoing facts, in support of the CORRECTION OF THE ENTRIES in myCertificate of Live Birth, and for whatever legal purpose it may serve.
