I, JOSEFINA ZABAT GUBAN, of legal ages, Filipino, and resident of 42 Sauyo Road, Novaliches, Quezon City, after having been duly sworn to in accordance with law, do hereby depose and state the following:

1. That I am the older sister of LUIS REYES SABAT, of legal age, Filipino, and a resident of Sydney, Australia;2. That as such I knew Luis Reyes Sabat since childhood who was born on February 11, 1968 at DR. J. R. Reyes Memorial Hospital, Manila;3. That I personally know for a fact that Luis Reyes Sabat was baptized at National Shrine of our Lady of Lourdes located at Kanlaon Cor. Retiro Street, 1114 Quezon City Philippines in the same year he was born in 1968;4. That I personally attended the said Baptismal of my brother;5. That I am executing this affidavit to attest the truthfulness of the fact of baptismal of LUIS REYES SABAT for the reconstruction of his baptismal record in full and to be entered in the Book of Baptism and for whatever legal purposes that this statement may serve.
