MISSION STATEMENT

The mission of the Aging and Disability Services Division (ADSD) is to provide leadership and advocacy in the planning, development and delivery of a high quality, comprehensive support service system across the lifespan to allow all of Nevada’s elders, children and adults with disabilities or special health care needs, to live independent, meaningful and dignified lives to the greatest extent possible.

AGING AND DISABILITY RESOURCE CENTER (ADRC)

PURPOSE

The Aging and Disability Resource Center (ADRC) program provides information, assistance and access to a wide variety of programs and services to support Nevada’s elders, individuals with disabilities and caregivers in meeting their long-term care needs. The mission of the ADRC program is to maintain or enhance the quality of life of Nevada consumers and communities in a respectful, efficient, and fiscally responsible way.

TARGET POPULATION

Persons 60 years and older

Individuals with a disability at any age

Caregivers

Individuals planning for long-term services and supports

SERVICES

The ADRC program provides an array of information and assistance to consumers in the community. Services are broken down into three levels of service. All three include a follow up component:

Information & Referral: ranges from basic information about a direct request to detailed information about a community service. Referrals are made when the Information and Referral Specialist has given the consumer information about choices, and the consumer requests contact information for a specific service.

Assistance & Advocacy: known as “Options Counseling,” which is a holistic service that attempts to find an array of services that meets the total needs of consumers. This level of service involves assessment and plan development. It is person centered and consumer directed.

Eligibility & Access: known as “Benefits Counseling,” which supports consumers in determining their possible financial and functional eligibility for an array of public programs. Consumers are supported in accessing programs through application assistance, consultation and appeals assistance, if needed. Benefits Counseling is the implementation of the plan of services developed during Options Counseling.

In addition to the physical sites located throughout the state, the Nevada ADRC web portal has been developed to provide information and assistance to consumers across the state, 24 hours a day and seven days a week. The portal is an entryway into the long-term services and supports system, utilizing a comprehensive resource directory, interactive forms, referrals and networking through an online community (i.e. Calendar of Events). It also helps to enhance service delivery in Nevada through online training and sharing of information and data.

ASSISTIVE TECHNOLOGY (AT) FOR INDEPENDENT LIVING (IL) PROGRAM

PURPOSE

The AT/IL program provides systems and resources at the community level which promote equal opportunity and life choices for people with disabilities, which enables them to live independently and exercise choice and control in their lives. Various kinds of assistive technology equipment such as home and vehicle modifications are provided when necessary for a person to live more independently.

TARGET POPULATION

Anyone whose disability causes a substantial functional limitation, and who lacks other resources to meet needs, can receive services from the AT/IL program.

SERVICES

ADSD IS THE STATE AGENCY OVERSEEING THE PROGRAM. THROUGH GRANTS, DAY-TO-DAY SERVICES ARE

PROVIDED BY COMMUNITY PARTNERS, C*A*R*E* CHEST OF SIERRA NEVADA AND REBUILDING ALL GOALS

EFFICIENTLY (RAGE), INC.

There are 21 services which may be provided with Part B Independent Living Services federal funding in addition to the "Core" services which must be provided. Those core services are: information and referral, independent living skills training, peer counseling (including cross-disability peer counseling), and individual and systems advocacy. The Statewide Independent Living Council (SILC) has selected nine of the available additional service categories, which may be provided to eligible applicants who do not meet eligibility criteria for other State/Federal Programs. They include:

Services related to securing housing or shelter;

Rehabilitation technology;

Mobility training;

Transportation, including referral and assistance for such transportation;

Consumer information programs on rehabilitation and IL services, especially for minorities and other individuals with disabilities who have traditionally been unserved or underserved ;

Therapeutic treatment;

Provision of needed prostheses and other appliances and devices;

Services for children with significant disabilities; and

o Community awareness programs to enhance the understanding and integration into society of individuals with disabilities.

The IL Program will not provide surgical or medical procedures.

AUTISM TREATMENT ASSISTANCE PROGRAM (ATAP)

PURPOSE

Autism Treatment Assistance Program (ATAP) was created to assist parents and caregivers with the expensive cost of providing Autism-specific treatments to their child with Autism Spectrum Disorder (ASD). ATAP provides a monthly allotment to pay for on-going treatment development, supervision and a limited amount of weekly intervention hours based upon a child’s individual treatment plan, age, and income. ATAP only funds treatments which have been proven by research to be evidence-based including Applied Behavioral Analysis (ABA), Verbal Behavioral (VB), and Pivotal Response (PRT) programs. Covered services include program training; development and supervision; daily intervention hours; and essential tools, supplies or equipment. ATAP may also fund Speech, Occupational and Physical Therapy when other resources do not provide coverage.

TARGET POPULATION

Individuals who reside in the State of Nevada who are under age 19 and are diagnosed as a person with a Autism Spectrum Disorder by a physician, psychologist, child/adolescent psychiatrist, pediatric neurologist or other qualified professional. A diagnosis from a multidisciplinary team is acceptable when in accompanied by an appropriate assessment report.

SERVICES

ATAP uses Applied Behavior Analysis (ABA) techniques for increasing useful behaviors and reducing those that may be harmful or that interfere with learning, in order to address socially important problems, and to bring about meaningful behavior change. The ATAP program uses a Supervision Tier Structure designed to protect the child’s budget by limiting what providers can charge and ensuring a basic level of treatment supervision is received and expected on a monthly basis. ATAP contains a variety of plan types. The plan type is selected during a collaborative assessment between the ATAP care manager, provider, and parent. The program types contain a variety of required hours ranging from 25 hours a week in the comprehensive plan to 10 hours a week in the basic plan and are designed to address each child’s individual needs.

COMMUNICATION ACCESS SERVICES (CAC)

FOR NEVADANS WHO ARE DEAF, HARD OF HEARING, AND/OR SPEECH DISABLED

PURPOSE

The purpose of the Communication Access Service program is to provide access to the Relay system for persons who are Deaf, hard of hearing, and persons with speech disabilities; thus providing access to Nevada’s telephonic system. The program also provides communication access advocacy for persons utilizing captioning services called CART (Communication Access Realtime Translation) and sign language interpreter services in order to access programs, healthcare, the legal system, and other needed services and systems. The program includes a statewide Interpreter/CART registry to allow agencies and individuals to find, contact and hire communication access service providers.

TARGET POPULATION

The target population consists of any Nevadans who are Deaf, hard of hearing or speech impaired. The programs also serve late-deafened older citizens.

SERVICES

The purpose of the program is to provide communication access by either distributing devices allowing persons with communication disabilities to access and use the telephone system available to all Nevadans, and by providing advocacy for persons with communication disabilities to access services and information. Relay Nevada is a system that allows standard telephone users and persons with communication disabilities access to telecommunications.

COMMUNITY SERVICE OPTIONS PROGRAM FOR THE ELDERLY (COPE)

PURPOSE

The COPE program provides supportive essential services to eligible older persons to help them maintain independence in their own homes as an alternative to nursing home placement.

TARGET POPULATION

Persons 65 years and older; and

Have a nursing facility level of care and are at risk of nursing home placement without supports and services to keep them in their home and community; and

Income limit $3094 per month; and

Assets limit $10,000 single/ $30,000 couple.

SERVICES

Case Management: Identification of resources and assisting recipients in locating and gaining access to waiver services, as well as needed medical, social, educational and other services regardless of the funding source;

Homemaker: Assistance with light housekeeping, meal preparation, laundry and essential shopping.

Social Adult Day Care: Provision of supervision and social activities in an Adult Day Care setting on a regularly scheduled basis for four (4) or more hours per day and one (1) or more days per week.

Adult Companion: Provides non-medical care, supervision and socialization to a functionally impaired recipient in his or her home or place of residence, which may provide temporary relief for the primary caregiver.

Personal Emergency Response Systems (PERS): An electronic device connected to the phone that enables persons at risk to secure help in an emergency for recipients who live alone in a private residence, or who are alone for significant parts of the day in that residence, have no regular caregiver for extended periods of time, and who would otherwise require extensive routine supervision.

Chore Service: Chore services are intermittent in nature and may be authorized as a need arises for the completion of a specific task which otherwise left undone poses a home safety issue.

Respite: Respite care is provided in the recipient’s home on a short-term basis because of the absence or need for relief of the primary caregiver.

Attendant Care: Assistance with personal care including bathing, dressing, grooming, shampooing, toileting, transferring, and eating.

ELDER PROTECTIVE SERVICES (EPS)

PURPOSE

The EPS program investigates allegations of abuse, neglect, self-neglect, financial exploitation and isolation of persons, age 60 and older. Interventions through preventative measures are taken to prevent abuse to elders. Self-neglect is the single largest source of repeat cases among elder abuse victims. The availability of necessary services and resources is essential for elders at risk. The EPS program develops outreach strategies to educate seniors and their caregivers about elder abuse. Good working relationships are maintained with law enforcement and other agencies, in order to increase EPS visibility and advocate effectively for EPS clients.

TARGET POPULATION

Persons age 60 and older, who are at risk of abuse, neglect, self-neglect exploitation and/or isolation.

SERVICES

EPS implements any necessary service that will remove older victims from risk of abuse, neglect, exploitation or isolation, including services that require client agreement, a court order, or other actions under the cover of law. Services may be state funded or community resources. Some of the services needed to alleviate or remedy an abusive situation include: Homemaker services which provide short term, intermittent housekeeping and shopping; Mental Capacity Evaluations to determine mental capacity and the need for guardianship; and Temporary Assistance for Displaced Seniors (TADS) when clients require temporary emergency placement until alternate housing arrangements can be established.

FAMILY SUPPORT

PURPOSE

The Family Support Services Program was developed to assist families to care for their relatives in the family home as a means to prevent out-of-home placement. It provides financial assistance for eligible families to obtain services for their family member. Individual/Family must meet the 300% of Federal Poverty Guidelines.

TARGET POPULATION

Individuals of all ages who meet the 300% poverty guidelines and reside at home with their family/primary caretaker are eligible for Family Support services.

SERVICES

Respite provides the primary caregiver an occasional break or temporary relief from the day-to-day responsibilities of caring for their family member with a disability. Regular respite care for the caregiver helps sustain the health and well-being of the caregiver, helps avoid or delay costly out-of-home placements, and reduces the likelihood of abuse and neglect.

Respite services can be provided in the family’s home, in the respite provider’s home, in a day care or recreational setting. Eligible families will receive a quarterly voucher in which they can use to purchase ongoing or emergency respite care.

FAMILY PRESERVATION PROGRAM

PURPOSE

To provide financial assistance to family members caring for their relatives with severe or profound developmental disabilities so that the family member can remain at home avoiding costly out of home placements.

TARGET POPULATION

Family members with profound or severe mental retardation ;

Children under six years of age who have developmental delays that require supports equivalent to that required by a person with profound or severe mental retardation;

Families with income at or under the 300% of current Federal poverty levels.

JOBS AND DAY TRAINING

PURPOSE

Jobs and Day Training (JDT) assists individuals in obtaining meaningful employment and living skills to help them achieve community inclusion, independence, and productivity.

TARGET POPULATION

The JDT program serves individuals of 18 years old or older who have been an open case with Developmental Services and indicate a desire of service.

SERVICES

Day Habilitation: Day Habilitation services provide day and individualized activities that support the participant's definition of a meaningful day. Day Habilitation services enable the participant to increase or maintain their capacity for independent functioning and decision making. Services consist of a daily program of functional activities that assist with acquisition, retention, or improvement in self-help, socialization, and adaptive skills that take place in a variety of day habilitation settings, including facilities or settings separate from the participant’s private residence or other residential living arrangement.

Facility Based Work/Prevocational Services: Prevocational services prepare a participant for paid or unpaid employment. Services include teaching skills such as self care, social skills, attendance, mobility training, task completion, self direction, problem solving and safety. Services are not job or task oriented, but instead, aimed at a generalized result. Services are reflected in the participant’s Individual Service Plan (ISP) and are directed to habilitation rather than explicit employment objectives.

Supported Employment: Supported employment is a combination of intensive ongoing supports and services that enable participants for whom competitive employment at or above the minimum wage is unlikely or who may be able to work in a competitive work environment but who, because of their disabilities, need supports to perform in a work setting. Supported employment is conducted in a variety of settings including enclaves at community businesses and work sites where persons without disabilities are employed and are designed to increase or maintain the individual's skill and independence.

LONG TERM CARE OMBUDSMAN PROGRAM (LTCOP)

PURPOSE

To serve as advocates for residents living in long term care facilities, which includes residential facilities for

groups. Responsibilities include investigating and resolving a broad spectrum of issues such as:

Resident rights

Quality of care

Nutrition

Admission, transfer, and discharge

Medical care and treatment

Environmental concerns

The Long Term Care Ombudsmen provide educational presentations to the general public as well as for long

term care professionals with emphasis on resident rights and prevention of elder abuse.

SERVICE COORDINATION

(DEVELOPMENTAL SERVICES TARGETED CASE MANAGEMENT)

PURPOSE

To provide service coordination to individuals and families in developing a community-based, person- centered life plan focused on individual needs and preferences. The Service Coordinator is responsible for the overall coordination of services and resources for the person and family, providing health and welfare assurances, and meeting federal and state regulations and requirements for Title XIX (Medicaid) and other sources of funding, including the Home and Community Based Services Waiver.

TARGET POPULATION

Individuals of all ages who meet the eligibility criteria for intellectual disability and/or related conditions as established by NRS 433.174 and NRS 433.

SERVICES

Developmental Services uses a person-centered approach in the development of the Individual Service Plan (ISP) which is the tool that drives the service delivery to the individual served. The ISP reflects the services and supports needed to support the person/family in reaching desired outcomes and to become more independent, integrated into the community and capable of functioning fully as a citizen. Items to be included in the plan include specific goals to focus on for the coming year, methods to be used to achieve the goals, the services to be provided to the person/family, time frame for delivery of services, costs, and the responsibilities of each of the team members. Services shall maximize each individual’s independence, capabilities, and satisfaction with services and involve a collaboration of team members. The intent of Service Coordination is to assist recipients in gaining access to needed medical, social, educational, housing, vocational, transportation needs and other support services including natural supports. Targeted Case Management is specific to those eligible for DS services under the Medicaid State plan and includes assessment, care planning, referral/linkage and monitoring/follow-up.

SUPPORTED LIVING ARRANGEMENT (SLA)

PURPOSE

The Supported Living Arrangement Program offers residential supports to individuals who require assistance to live in the least restrictive community setting possible. SLA offers habilitative, and skill building supports to individuals designed to maximize independence in the community. Possible living arrangement settings include an individual’s family home, apartments, host homes, foster homes and shared residences. This service is provided through agencies that are contracted with the regional centers.

TARGET POPULATION

The SLA program serves individuals of all ages who have an open case with Developmental Services and:

Are Medicaid eligible;

Have the desire and ability to participate in SLA services and adhere to guidelines;

Can be safely supported in a community residential setting with specialized supports.

SERVICES

Intermittent SLA: The service recipients in this program require intermittent support while living in a home or

apartment by themselves, with a roommate, or with family. The service recipient receives a minimum (from

several hours per week) to moderate (daily) support from paid SLA staff according to the identified needs and

desires. Support hours are determined by the team’s assessment of the service recipient’s service/support

needs.

24 hour SLA: 24 hour Supported Living Arrangements support service recipients in need of maximum support services. Staff are present whenever there are service recipients at home; there is awake or asleep staff at night depending on the needs of the service recipients. People typically live in a home with roommates in a community neighborhood and share the support services from the provider. Host Homes: Service recipients who desire or need a family living situation receive services from a Host Home provider who includes the service recipient in their family life and activities. Direct services/supports are to assist in the acquisition, retention or improvement of skills for the service recipient to successfully reside in the community.

NEVADA EARLY INTERVENTION SERVICES (NEIS)

PURPOSE

To provide specialized supports and services for infants/toddlers with disabilities and their families to meet their individualized needs.

TARGET POPULATION

Infants/toddlers who are birth through two years of age, through medical review, developmental assessments and/or informed clinical opinion meet the following:

50% delay of a child’s chronological age in any one developmental area or 25% delay of child’s chronological age in any two developmental areas, which includes:

HOME AND COMMUNITY BASED WAIVER (HCBW) FOR ASSISTED LIVING (AL)

PURPOSE

The intent of this program waives Medicaid eligibility rules to allow institutional income guidelines and maximize the independence of Nevada’s frail elderly by providing assisted living services to eligible individuals in a residential facility that includes individual living units, a kitchenette, sleeping area or bedroom, contain private toilet facilities, and offers 24-hour supervised care as an alternative to nursing home placement.

TARGET POPULATION

Persons 65 years and older living in Clark County, Nevada; and

Have a nursing facility level of care and are at risk of nursing home placement without supports and waiver services to keep them in their home and community; and

Income below 300% of SSI ($2,094/month).

SERVICES

Case Management: Identification of resources and assisting recipients in locating and gaining access to waiver services, as well as needed medical, social, educational and other services regardless of the funding source. Augmented Personal Care: Augmented personal care provided in a licensed residential facility for groups is a 24-hour in home service that provides supervision and assistance for functionally impaired elderly recipients with basic self care and activities of daily living. This care is over and above the mandatory service provision required by regulation for residential facility for groups.

HOME AND COMMUNITY BASED WAIVER (HCBW) FOR THE FRAIL ELDERLY

PURPOSE

The intent of this program waives Medicaid eligibility rules to allow institutional income guidelines to provide supportive essential services to eligible older persons to help them maintain independence in their own homes as an alternative to nursing home placement.

TARGET POPULATION

Persons 65 years and older; and

Have a nursing facility level of care and are at risk of nursing home placement without supports and waiver services to keep them in their home and community; and

Income below 300% of SSI ($2,094/month).

SERVICES

Case Management: Identification of resources and assisting recipients in locating and gaining access to waiver services, as well as needed medical, social, educational and other services regardless of the funding source;

Homemaker: Assistance with light housekeeping, meal preparation, laundry and essential shopping.

Social Adult Day Care: Provision of supervision and social activities in an Adult Day Care setting on a regularly scheduled basis for four (4) or more hours per day and one (1) or more days per week.

Adult Companion: Provides non-medical care, supervision and socialization to a functionally impaired recipient in his or her home or place of residence, which may provide temporary relief for the primary caregiver.

Personal Emergency Response Systems (PERS): An electronic device connected to the phone that enables persons at risk to secure help in an emergency for recipients who live alone in a private residence, or who are alone for significant parts of the day in that residence, have no regular caregiver for extended periods of time, and who would otherwise require extensive routine supervision.

Chore Service: Chore services are intermittent in nature and may be authorized as a need arises for the completion of a specific task which otherwise left undone poses a home safety issue.

Respite: Respite care is provided in the recipient’s home on a short-term basis because of the absence or need for relief of the primary caregiver.

Augmented Personal Care: Augmented personal care provided in a licensed residential facility for groups is a 24-hour in home service that provides supervision and assistance for functionally impaired elderly recipients with basic self care and activities of daily living. This care is over and above the mandatory service provision required by regulation for residential facility for groups.

COMMUNITY BASED CARE HOMEMAKER PROGRAM

PURPOSE

The intent of the Homemaker Program is to provide in-home supportive services for individuals requiring assistance with activities such as housekeeping, essential shopping, meal preparation and laundry to prevent or delay placement in a long-term care facility.

TARGET POPULATION

Individuals with functional deficits, lack of support system, and who are at risk of institutionalization; and

Income at or below 110% of Federal Poverty; and

Asset Limits $10,000 individual/$30,000 couple

persons with severe physical disabilities to remain in their own homes and avoid placement in a long-term care facility. The provision of home and community-based services is based upon the identified needs of the recipient and available funding. ADSD assists recipients with accessing other available services, as needed.

TARGET POPULATION

Individuals throughout Nevada, age 18 or older, in need of supportive services to live independently, who have been diagnosed with a physical disability that substantially limits their ability to complete activities of daily living.

SERVICES

Case Management: Identifies resources and assists recipients in locating and gaining access to waiver services, as well as needed medical, social, educational and other services regardless of the funding source.

Attendant Care: Assists with personal care, including bathing, dressing, grooming, shampooing, toileting, transferring, and eating.

Homemaker: Assists with light housekeeping, meal preparation, laundry and essential shopping.

SENIOR AND DISABILITY RX PROGRAM

PURPOSE:

The Senior and Disability RX Program provide seniors and persons with a disability relief from high cost of prescription medicine. The program provides assistance with Medicare and Part D expenses for members who are eligible for Part D, and provides a cost-sharing benefit for members who are not eligible for Part D.

TARGET POPULATION

DISABILITY RX PROGRAM

Age 18-61 with a qualifying disability;

Nevada Resident continuously for 12 months;

Annual Income no more than $26,836 for singles and $35,773 for couples.

SENIOR RX PROGRAM

Age 62 at the date of application;

Nevada Resident continuously for 12 months;

Annual income no more than $26,836 for singles and $35,773 for couples.

SERVICES

The Senior and Disability RX Program helps Nevada seniors and persons with a disability who are not Medicare eligible with their Monthly premium, their deductible, co-payments of $10 or $25. The program has a coverage limit of $5,100. For seniors and persons with disability who are Medicare eligible, the program assists them with their monthly premiums for the Medicare Part D Prescription Drug Plan, and helps with prescription costs if they are subject to the Part D coverage gap. The program has also begun offering access to a dental insurance program for a limited number of members.

TAXI ASSISTANCE PROGRAM (TAP)

PURPOSE

The Taxi Assistance Program (TAP) is a subsidized program that provides taxi rides at a reduced cost.

TARGET POPULATION

Clark County residents that are age 60 or older or persons of any age that are permanently disabled;

Income below 400% of the Federal Poverty Level.

SERVICES

Clients may purchase $20 taxicab coupons for a $10 fee. Coupons are valid for any Clark County taxicab company 24 hours a day, seven days a week. Each client is entitled to purchase a varying amount of books per calendar month based upon income.

TRAUMATIC BRAIN INJURY PROGRAM (TBI)

PURPOSE

The Traumatic Brain Injury (TBI) Program was created to provide assistance to individuals and their families who are confronted with inadequate or unavailable TBI services and supports.

TARGET POPULATION

Persons diagnosed with a traumatic brain injury as defined in NRS 427A.850.

SERVICES

ADSD provides a grant to Accessible Space Inc. to fund the Nevada Community Enrichment program (NCEP). NCEP provides the following services:

Treatment during the day on an outpatient basis;

Care provided in a facility operated and maintained to furnish food, shelter, assistance and limited supervision;

Care provided in the home;

Instruction in the skills required for independent living;

Placement for jobs;

Counseling and treatment for the abuse of drugs and alcohol;

Support services for families of persons with TBI.
