DESCRIPTION

Our 12-month Recovery Assistance Program (RAP) provides a structured safety net to keep people on the right path and offers a safety net for relapse, saving time, money and effort to get the individual back on track. The program builds pride in one’s self, opens communication between the person and the family, provides a chance to voice concerns or celebrate victories, and truly changes the lives of everyone involved in this difficult process.

RAP RECOVERY ASSISTANCE PROGRAM

LONG-TERM CARE FOR LONG-TERM RECOVERYFor the Individual - For the Family

AiR

At AiR, we believe treatment builds a foundation for understanding addiction, alcoholism or mental health illnesses, but is only the first step towards a full life in recovery for the addicted and their family.

Behavioral health issues can affect an entire family and all those affected deserve equal attention, compassion, and understanding. AiR’s 12-month Recovery Assistance Program (RAP) is designed to support the individual as well as manage the questions, fears, and concerns of their family, friends and employer after leaving treatment.

We believe that of the approximately 30,000 days we each have on this planet, we should spend fewer days suffering from a treatable illness and more days living a fulfilled life. We are committed to empowering the participants of RAP and their families with the necessary tools to live better days, one day at a time!

Life after treatment... Living a life in recovery and establishing trust

Recovery Assistance Program A support service for a life of recovery

While treatment builds a foundation for understanding the illness, it is only the first step towards a full life in recovery. Once one leaves treatment and enters back into the “real world,” life can seem scary. At AiR, we believe it doesn’t have to be. Our Recovery Assistance Program (RAP) is designed to support the participant and their family as they re-enter life after treatment as a person in recovery.

FACTS• Nearly 50% of those who successfully complete treatment will relapse within the first year after

leaving. The leading cause for relapse is failure to follow the prescribed continuing care plan set up for them during treatment.

• RAP participants experience an 80% success rate within their first year of recovery, 30% more than those without a supportive program.

• For those who do struggle in the first year, RAP participants are back in the appropriate level of care far faster than those who are on their own.

Recovery Assistance Provides• Support and assistance in transitioning home after treatment• Relapse prevention• Respectful, continuous monitoring so family and employers don’t need to• Professional case management• Random toxicology screenings• Setting the ground work for moving forward in recovery• Assessing and overcoming fears• Need identification and solutions• Consulting services• 24 hour, 7 days a week resourcing

It is important for everyone involved in the recovery process to remember that recovery is a lifelong process with the first year being the most challenging. Through RAP, our professional team is here to guide and support you to help make the first year the best it can be.

For the ParticipantRelapse happens long before a person starts using - RAP is proven to identify, address, and resolve negative behavior and patterns that can lead to relapse and impede the recovery process. Our program is individualized to meet the participant’s needs through on-going meetings, assists in repairing relationships in sober life and uses toxicology screenings to further substantiate the recovery process. The RAP case manager is the participant’s advocate and provides positive reinforcement of healthy decision making skills which lead to self-reliance and the formation of a stable sense of self. The individual case manager will also facilitate regular updates and communication with the participant’s collateral contacts (support system). This removes the family from the role of “monitor,” helps all parties to begin to trust again, and helps them manage their expectations of each other.

I have a foundation that gives me the ability to deal with things I couldn’t before. Now I can look in the mirror and like what I see and know that whatever it is, I’ll be able to deal with it - thanks to RAP.

For the FamilyWe understand that lasting change takes time and AiR offers our resources to the family throughout the 12-month RAP program. AiR has learned that by offering support to the family members, as well as the participant, greatly improves the quality of communication, further enhancing the quality of recovery. The RAP case manager will keep regular communication with collateral contacts (support system) that have been identified in the participant’s continuing care plan. This communication provides the most accurate picture of how the participant is progressing in their recovery without relying strictly on “self reporting.”
