IV. TESTING PROCEDURE
A. Cause-Tes. If a! e#ploees de#ea!o$r ca$sed the Co#pa! to s$spect that he is $si!",
possessi!", soliciti!" or selli!" alcohol i! the workplace, the latter will re0$est thefor#er to s$%#it hi#self to a ca$se-test. If the e#ploee o%3ects to s$%3ect hi#self toca$se-test, he #$st state his o%3ectio! a!d the reaso! thereof i! writi!". TheCo#pa! shall i##ediatel decide whether the "ro$!d for o%3ectio! is +alid or !ot.

/. If the ca$se-test showed a positi+e res$lt, the e#ploee will %e referred to theassess#e!t tea# for f$rther e&a#i!atio!. =owe+er, if the e#ploee still disa"reeswith the res$lt of the assess#e!t tea#, he will %e s$%3ected to a!other alcoholtesti!" at the !earest #edical facilit.
V. CONFIDENTIALITY
The JBW FLOORCENTER, INC. shall o%ser+e at all ti#es co!fide!tialit of the res$ltsrelati+e to alcohol tests do!e to e#ploees.
VI. TREATMENT, REHABILITATION AND REFERREAL
The ssess#e!t Tea# shall deter#i!e whether or !ot a! e#ploee fo$!d addicted toalcohol wo$ld !eed referral for treat#e!t a!d*or reha%ilitatio! i! a ;epart#e!t of =ealth-
ccredited Ce!ter. This %e!efit is o!l "i+e! to e#ploees who seek help fro# the assess#e!ttea#.
VII. MONITORING AND EVALUATION
The i#ple#e!tatio! of the alcohol-free workplace polic a!d pro"ra# shall %e #o!itoreda!d e+al$ated periodicall % the ssess#e!t Tea# to e!s$re that the "oal of a! alcohol-freeworkplace is #et.
VIII. DISCIPLINARY ACTION
. Fail$re to s$%#it to ca$se-test, post accide!t-test, ra!do#-test, or alcohol test % a!earest #edical facilit shall %e a "ro$!d for a discipli!ar actio! with a pe!alt ofo!e >? #o!th s$spe!sio! witho$t pa)
