ambulatory surgery qualifications
DESCRIPTION
Once restricted to basic preventative medicine, our clients now perform 70-90% of procedures in outpatient settings through advanced laparoscopic and minimally invasive surgery. Successful ambulatory care facilities are designed to maximize flow. At Array, we develop separate and distinct circulation areas for both patients and staff, introduce intuitive wayfinding to enhance and streamline the patient care process, and then test our designs through simulation modeling.TRANSCRIPT

to view our thought leadership on Medical Office Buildings
As primary care is positioned at the forefront
of the evolution of healthcare delivery and
represents the catalyst to feed a sprawling
network of diagnostics, specialty care
and tertiary care, we must reconsider the
environment for better integrating. Medical
office buildings should be the heart of the
healthcare experience, empowering patients
with education and wellness, integrating the
healthcare community with telemedicine,
and providing well-integrated, highly efficient
care. Array planners and designers are setting
new trends in the ambulatory setting, and
see medical office buildings as a tremendous
opportunity for health institutions to enhance
their position in a changing market.

As a companion building to a new state-of-the-art 237-bed
Replacement Hospital for Capital Health, Array in association
with HKS, designed a new 328,000 SF Cancer Center and
Medical Office Building. Joined to the new Hospital by a
two-level pedestrian mall, the building consists of five floors
with suites ranging from 1,000 SF to 10,000 SF. The building
houses a combination of hospital-based outpatient services
and physician practice suites.
Additional MOB Services include a comprehensive Cancer
Center with state-of-the-art linear accelerators and cyber
knife therapies to treat outpatients; outpatient Imaging with
radiography, nuclear medicine, 3.0T MRI, CT and PET/CT
equipment; and, Spa services tailored to Oncology patients
which include wig fittings and lymphatic massages. Floors
three through five are dedicated physician office space.
Corridors overlooking the five story atrium connect the MOB/
Cancer Center with the Medical Center and allow for easy
access for physicians who have rounds to make.
Infusion programs are vertically connected by the lobby atrium
and pedestrian mall. The infusion bays have access to and
views of a roof garden developed over the Cancer Center’s
first floor with a coffee bar adjacent to the roof garden. An
additional second floor function accessed from the mall is a
four-OR ambulatory surgery suite.

Patient and family engagement was very important to the client’s overall vision so the
design team was asked to not only design hospital functions but also gift shops, a spa,
public resource library, business center, coffee shop and bistro. The Oasis Spa was
designed to offer full spa services to the public, visitors and patients, with a particular
focus on cancer support.

The Laser Spine Institute provides chronic back pain relief
through an intense five-day session that includes initial
screening, surgery, post-surgical physical therapy and body
reconditioning on a fee-for-service basis.
With patient volumes increasing, LSI tasked Array with their
facility needs across the United States. LSI’s clientele
expects a level of design most often seen in five-star
resorts. In addition to designing clinical area and operating
suites for patient care, Array included amenities such as
personalized work areas with internet access, conference
facilities, fine dining, media & relaxation rooms as well as
concierge services.
Two locations in Ohio were completed in early 2015.
Mayfield Heights, outside Cleveland; and Cincinnati.
With the success of their current facilities throughout the
US, LSI anticipates future expansion and continues to call
on Array’s expertise in site selection, planning and design.

Wellness
Jonathan M. Bykowski, AIA, Lean Six Sigma Black BeltPrincipal and Practice Leader, Continuous Improvement
As a father, I am even more focused on the value of wellness and prevention and setting a good example for my son. As an architect, I want to empower others to do the same.
In conjunction with Anchor Health Properties and Indian
River Medical Center, Array is designing a 150,000 SF
integrated medical home model health and wellness center
built on approximately eight acres adjacent to the Medical
Center. The project will feature cross-discipline care
coordination and a unified patient experience while being a
market differentiator for Indian River Medical Center.
The Wellness Center is designed to promote prevention,
education and healthy living in addition to providing
chronic disease management and interventions.
Customer service will be highlighted throughout the
facility from registration and greeting through to
departure which could include appointments at multiple
services in the same visit. Centralized concierge-style
check in and standardized physician clinics allow for
program flexibility without the need for renovation. EHR
integration allows for decentralized clinic care model
featuring team centers for physicians and clinicians.

Beginning with a clear target outcome, we used Lean tools to help the client streamline their arrival and check-in
process. We are presently designing a multi-specialty integrated medical home outpatient facility on Indian River
Medical Center’s Vero Beach, FL. campus. The new Wellness Center will be home to six physician practices, an imaging
center, two therapy sites, fitness center, and a resource and education program.
The team desired only one stop upon arrival for patients to check-in despite the programs occupying four floors.
Our team brought together the departments and providers including Registration and Information Technology to
understand the opportunities and limitations of the available systems and develop a seamless integration of the
different programs. To minimize wait times, check-in is decentralized by floor, however, all patients will have to ‘check-
in’ at only one location to begin their visit.
Lean process improvement is a powerful tool to enhance the patient experience as well as improve staff efficiency.

When this national children’s hospital decided to move
into a new market, they needed move quickly. As with all
ambulatory projects in today’s competitive environment,
being first to market with the right product at the right cost
is critical.
Patient and Family focused design in the context of
maximum efficiency is the hallmark of the final Ambulatory
project and was achieved through a rigorous process to
define target outcomes and then test solutions.
Current and Future State process mapping, Data
Collection, Visual Programming, Experience Mapping and
Continuous Stakeholder Engagement are cornerstone
tools used to ensure the right space was built at the right
size at the right time. With the rapid evolution of care and
reimbursement models and the uncertainty of the future,
a flexible and adaptable platform was created to be viable
today and tomorrow.

Shifting the paradigm to create a new, integrated multi-specialty outpatient center. Array’s Process-led design approach rapidly defined and designed a new, fully integrated multi-specialty outpatient
center that features flexible provider clinics, physical therapy, diagnostic imaging, wellness, and ambulatory surgery.
Our approach, using Lean as a foundation, allowed Nemours to work through complex questions about integration,
resource deployment, and business/revenue implications while studying physical configurations and cost structures
to support them. All of this was achieved without sacrificing commitment to capital budget objectives as well as
schedule.
Patient and Family focused design in the context of maximum efficiency is the hallmark of the final project and
was achieved through a rigorous process to define target outcomes and then test solutions. Current and Future
State process mapping, Data Collection, Visual Programming, Experience Mapping, and Continuous Stakeholder
Engagement are cornerstone tools used to ensure Nemours built the right space at the right size at the right time.
Staffing efficiencies, provider load, throughput, and technology were integrated into the architecture process. With the
rapid evolution of care and reimbursement models and the uncertainty of the future, a flexible and adaptable platform
was created to be viable today and tomorrow.

Environments have the power to set our expectations, lift our spirits and inspire hope. This is what motivates me to create spaces which mitigate the challenges faced by all who enter a healthcare facility.
Healing Environments
Patricia D. Malick, CHID, EDAC, Lean Green BeltPrincipal and Practice Leader, Interior Design
Montefiore Medical Center selected Array to consolidate ambulatory
services from three campuses and outpatient clinics into a new state-
of-the-art Ambulatory Tower that would become the benchmark for
their expanding Ambulatory Care services. Array provided planning and
design services for a new 12-story Ambulatory Surgical Center on a site
that received prior zoning approval for a hotel. To avoid obtaining a vari-
ance, which would result in significant cost and delays, the footprint and
setbacks could not be altered.
Array developed a unique design solution incorporating dedicated ele-
vators for staff and materials within the operating suites and support
areas. Although the 12 ORs and four procedure rooms (sized to become
additional ORs if needed) are split between three floors, simulation mod-
eling showed that the dedicated elevators facilitated patient flow and
reduced room turnaround time.

Mount Sinai Health System Center for Health and HealingNew York, New York
The Mount Sinai Health System Center for Health &
Healing’s primary focus is on wellness and offers an
integrated, preventive approach to medicine. Their practice
has been at the forefront in the U.S., integrating sustainable
materials and practices as well as feng shui principles.
Demand for The Center’s services drove the need to expand
their practice. Array designed an expansion plan which
doubled their practice space while allowing the original
practice to operate during design and construction.

Placement, materials and colors became tools for incorporating
Feng Shui elements into the design. Special attention was
given to utilizing reclaimed materials to reinforce our client’s
focus on sustainable design. The facility was designed using
natural materials, soothing colors, and natural daylight to have
an uplifting effect on patients and staff. Throughout the Center,
Array incorporated bamboo and rubber/cork flooring. Materials
requiring no-wax or those that are maintained with “green”
cleaning solutions were sourced within the project to ensure
optimal air quality.
The project accomplished Mount Sinai Health System
Center for Health & Healing’s goal to transform the quality
of healthcare and its delivery toward a more holistic model
conscious of the role played by the environment in the healing
process. Demand for the Center’s services grows and is
expected to increase as healthcare in the nation continues
to shift from one focused on illness and reaction to one of
wellness and prevention.
Using principles of Feng Shui, the space organization was refined to promote harmony and balance.

Patient Experience
Beth Israel Medical Center, part of Mount Sinai Health
System, offers a broad-based fully integrated health ser-
vices network throughout the New York City metropolitan
region. After their lower Manhattan family practice patient
base increased beyond the capacity of their offices, Mount
Sinai committed to quickly expand capacity. Mount Sinai
selected a building at the corner of 8th Avenue and 23rd
Street for the expanded practice. The project included the
renovation of a three-story building adjacent to a popular
restaurant.
Mount Sinai’s new facility provides a multitude of re-
sources for their patients. The new walk-in, primary care
medical practice is located on the 2nd floor, while the 3rd
floor serves the specialty practices such as dermatology,
radiology, ophthalmology, orthopedics, GI, cardiology and
pain management. The design features a new curtain
wall and façade that incorporates the client’s brand with
LED-illuminated panels and signage. The exterior now
draws patients to the space and defines the entrance with
a matte silver façade that is vibrant blue in the evening.

This 16,500 SF renovation is now a benchmark
facility for Mount Sinai and is considered a ‘Center
of Excellence’ for ambulatory care. The architecture
promotes Beth Israel’s brand, announces the
presence in the neighborhood and helps attract
new patients. The interior design promotes better
workflow and clinical efficiencies for the staff. The
modern design and warm wood tones contribute to a
strong first impression and inspire confidence in the
patients and their families alike.

In conjunction with Anchor Health Properties, University of
Pennsylvania Health System and Chester County Hospital ,
Array has designed a 70,000 SF ambulatory care facility 15
miles from the main Chester County Hospital campus. The
design features a “Main Street” service line access corridor
for a patient friendly circulation system. First floor services
include Imaging (MRI, CT, Mammography, Ultrasound and
Radiology), Physical Therapy, Blood Lab, primary and specialty
care clinics. Second floor services include an Eye Clinic and
leasable space.
The site promotes health and wellness through an integrated
plan that includes healing gardens around the perimeter
of the center, a wellness path and rest areas. The wellness
path loops through open spaces to the east of the building
and around the wooded wetland areas and storm water
management basin and incorporates rest areas with park-
bench seating. Anchor Health Properties, Chester County
Hospital and Array held an extensive visioning session and
visited local gardens and farms to gain inspiration for the
landscape surrounding the ambulatory care center.
To encourage patients to use the facility to its fullest
potential, a full-service bistro is included at the main
entrance. This allows a restful place for a break, with inside
and outdoor seating, making multiple visits in one trip easier
for the whole family. Several different waiting areas allow
families comfortable spaces to congregate.

Following a merger/acquisition of two hospitals located in
the Cincinnati metropolitan area, St. Elizabeth Healthcare
recognized a need for a new Ambulatory Care Center to
serve as a ‘Gateway for Health’ to increase its profile in the
Cincinnati metropolitan area’s growing market.
Array sited this new, three-story, 120,000 SF Ambulatory
Care Center facing a major interstate to increase public
awareness. A “Main Street” circulation design provides
convenient access to all services including central
registration and reception, community conference zones,
bistro, gift shop and patient education center. This amenity-
laden spine follows the length of the building with access
to Imaging and Lab Services and the 10,000 SF Urgent
Care Center. The Urgent Care Center promotes an optimal
patient experience with a focus on convenience.
Patients are warmly greeted, then invited to use the
latest technology for registration. Retail amenities ensure
patients and visitors have an engaging, positive experience.
Exterior landscaped views and an intimate reflection
garden, beautiful artwork and rich colors and textures serve
as orienting devices for both patients and staff. Second and
third floors house physician offices, Dialysis and Diabetes
Center for primary and specialty care.

The Ambulatory and Urgent Care Center provides services to
this growing area of northern Kentucky. The facility promotes
an outpatient experience where the patient can be scheduled
more conveniently and treated quickly in a more patient-centric
clinical environment. The building was strategically located on
a major interstate that fronts the site and has direct access to
the facility. Ambulatory Care services are located on the first floor

Lancaster General Hospital Medical Office Building Lancaster, Pennsylvania
Array collaborated on the full fit-out of a new Medical Office Building
to provide additional outpatient and ambulatory care services
adjacent to Lancaster’s acute care hospital. The design concept
for this project capitalizes on the tenets of evidence-based design
including patient empowerment, enhanced staff provisions,
education, intuitive wayfinding and family-centered care.
The project features outpatient care and education / community
support and amenity spaces throughout the four-level facility.
Ground Floor Imaging and Lab Testing units streamline the
outpatient testing and diagnosis process. In addition to First Floor
patient functions for Cardiothoracics and Outpatient Physical
Therapy, a bridge connecting the new facility to the main hospital
provides employee amenities that include a bistro / convenience
store and Health and Fitness Center. The facility’s Second
Floor includes an additional connector bridge as well as Sleep
and Pulmonary Labs. The Third Floor Family Health Center was
subdivided into three distinct treatment pods to create a less
daunting and overwhelming space for patients. A supplemental
community learning center provides on-site group education
programs for patients and staff. Here again, access to a familiar
location has facilitated an expanded number of group discussion
participants and improved patient compliance and clinical follow-up.

Doylestown Hospital Medical Office Building & Cancer CenterDoylestown, Pennsylvania
Doylestown Hospital is among the last independent community
hospitals in the greater Philadelphia area. It is largely known
for its quality of care and innovative approach to medical
advancement. When Doylestown was interested in expanding
its oncology program, it partnered with world-renowned
University of Pennsylvania Health System to develop an
addition to their state-of-the-art outpatient care pavilion on the
Hospital’s campus.
The addition accommodates the radiation oncology program,
as well as physician practices and infusion services. The
radiotherapy suite includes exam and consultation rooms, a
linear accelerator vault, simulation room.
Array provided a series of studies dealing with impacted portions
of the campus as a result of the new addition. Inpatient access
to the radiotherapy suite was examined, as the preferred interior
route was not code compliant. Additional studies on the building
footprint’s impact on parking, University of Pennsylvania Health
System / Doylestown signage, and new entrance logistics were
also included in the design study.
