am trying to determine the best analogy with which to
start this article. My first thoughts are of the old lesson
about a frog in boiling water. Keep in mind that I have
never tried this myself, nor do I plan to. Apparently, when a
frog is placed in a pot of boiling water it will quickly do any-
thing and everything to escape. If, however, you place the frog
in room temperature water, and slowly bring it to a boil, it
will not struggle at all. You will wind up with frog soup. This
may be an all too accurate description of what is occurring in
regards to our profession at the present time. We continue
to work hard in our everyday lives to maintain our practices
and care for our patients while the waters are being warmed
around us.
The possibility of mid-level providers working in our state
is creeping from an idea toward a reality. Several states across
the country have been forced to accept this encroachment
upon our profession. The water got too warm before they had
the chance to jump out. Establishing a mid-level provider to
solve the access to care issue will create a two-tiered system of
dental care in our state where those in underserved communi-
ties will receive a lower level of care. It may also allow for the
entrepreneurial minded to find venues to expand the role of
mid-level providers into all circles of practice, lowering the
overall quality of dental care statewide and threatening our
livelihoods. If history has taught us anything, we can be sure
that the best intentions behind a law do not guarantee that
everyone interprets it that way.
There are actions we can take as a profession before the
water gets too hot. First, we have to challenge our association,
at both the state and national levels, to create and implement
strategies to assure the profession we cherish is protected.
Those strategies should include hard work on both the leg-
islative side as well as the public relations side of the issue.
We have to make sure our State of Michigan Representatives
and Senators understand the implications of the proposed
changes that are bound to be introduced in the coming years.
We have to direct our legislative efforts both locally and state-
wide toward establishing relationships with leaders before it
becomes too late. Our MDA legislative staff has done their
part, now we as dentists must do ours to ensure that we are
speaking with a united voice. We must also be speaking regu-
larly to those involved in the decision-making process. Once
the water is boiling it will be too late.
It may be time to push for a change in our public relations
message that we are spreading across the state. We should
begin the process of establishing a message that will make
it clear to the general public why a mid-level provider will
not be good for the overall oral health within our state. We
must get out ahead of the issue by establishing a message
that people will understand in regards to quality of care that
people will receive.
We also have to challenge ourselves as individuals regard-
ing the public trust that has been placed in us. The access to
care issue is not going away. Several state level work groups
and committees have been working tirelessly on solutions, but
the process is slow. The answer is going to demand creativity,
hard work, and sacrifice. We all do plenty every day within
our practices to help those in need of care without resources.
We need to find ways to organize and increase our philan-
thropy, increase the efficiency of our efforts and get the public
to recognize what we do on a regular basis. You may have
done that entire denture case for Mrs. Smith at no cost, but
no one knows about it. They are, however, acutely aware of
your crown fee and what kind of car you drive. A very big por-
tion of defining our profession’s future will be determined in
the arena of public relations. We have to be actively defining
ourselves rather than waiting to defend ourselves. It is time to
call on our MDA to put a public face on our profession that
we deserve, and it is time for us to make sure we deserve it.
I apologize if this message is heavier or more negative than
past submissions. It is just starting to feel uncomfortably hot
in this water.

ne day back in December started out as my typical
calm Saturday morning, enjoying hot coffee,
watching the snow fall steadily, procrastinating
my snow shoveling duties, Playstation blaring in the back-
ground, kids fighting over who could be “player one,” the
steady dose of “Mom”-“Mom”-“Mom” every few minutes
(seemed like seconds) coming from one or both of my chil-
dren, which evolved into “Daddy”-“Daddy”-“Daddy” once
Mommy began to boycott the persistent chants. Nothing
new, just my typical weekend morning. I decided to turn on
my favorite national news station and get caught up on the
latest events. The “hot” topic at the time was the media barrage
about airport scanners, how they work, their reliability, their
ability to potentially increase safety, and the privacy issues
that come along with it. Now, the debate of giving up some
form of privacy or possibly risking your plane getting blown
out of the sky is a completely different topic which I don’t
intend on getting into. As the discussion evolved in this
segment, they began to touch on whether or not the scanners
posed a health hazard. The ’ol risk-benefit debate. Toward the
end of this discussion, the topic took an eerie turn. The medi-
cal “expert” began to talk about the current trend and over-
use of cone beam CT scans in dentistry! This dangerous and
somewhat misleading statement that came out of nowhere
gave me the sense that our profession has just been subjected
to an involuntary “pat down” (palms out of course). After
I lifted my jaw off my chest, I began to think, well, did we
deserve such a jab below the belt? Two thoughts: no and yes.
No: As a dental profession, we understand and are
obligated to the ALARA principle (As Low As Reasonably
Achievable). This is the radiation safety principle for mini-

mizing radiation doses and releases of radioactive materials
by employing all reasonable methods. This is a sound safety
principle that we use in our everyday practice. For example,
the benefit of recommending cone beam CT scans in select
cases far exceeds the risk.
Yes: Promoting new technology to the public that involves
a sensitive topic such as “radiation” without a strong public
education/relations front is dangerous. It opens up misunder-
standings and unnecessary concerns to the general public. I
have read some dental blogs in which dentists like to scan every
patient and/or every dental implant case. Clearly an example
of overuse, and surely (I hope) is an example that represents
the minority of our profession. Standard radiographs are fully
capable of providing sufficient information in the majority of
our patients. Nonetheless, the promotion of new technology
with CT scans without a strong public education front spurs
headlines in the media and internet such as “Radiation from
Dental CT Scans Raising Concerns,” “Overuse of Cone Beam
CT Scans in Dentistry?” and “Radiation Worries for Children
in Dentist’s Chairs.”
The ADA has posted responses to some of these articles
by reiterating our long-standing principle applying ALARA
to reduce radiation exposure to our patients. Stating “dentists
should weigh the benefits of dental radiographs against the
consequences of increasing a patient’s exposure to radiation,
the effects of which accumulate from multiple sources over
time.” I have also started to see some local publications
educating the public of the benefits of CT scans in den-
tistry, which is great! But I think there needs to be more.
Highlighting the need for more education is a quote in an
article featured on the front page of The Grand Rapids Press
in January of 2011. “Because the machines can cost between
$150,000 and $250,000, there is a concern dentists could be
focused on recouping their investment”… To avoid a prolif-
eration of the machines, “We want the fewer you can get to do
more scans of the kind that are necessary, so that dentists who
have them aren’t tempted to overuse them for the things that
aren’t necessary.”—Lody Zwarensteyn, President of Alliance
for Health, a nonprofit organization based in Grand Rapids.
We as a profession know the benefit/risk ratio utilizing CT
scans in selected cases, but does the general public? The two
are equally important to our patients and our profession.

Foundation grant recipients are required to provide out-
come reports regarding the results of the program, and Kid’s
Food Basket surveys GRPS teachers regarding their success.
The feedback was positive regarding the program with many
teachers noting that these children do not receive new tooth-
brushes regularly.
Kid’s Food Basket’s outcome report also noted that with
the Foundation’s support, as well as the support of other like-
minded businesses and individuals, brushes and toothpaste
will be included in the Spring Break Bags as well.
Kid’s Food Basket would welcome your donation of tooth-
brushes or small tubes of toothpaste. If you have toothbrushes
or toothpaste that you would like to donate, please contact

id’s Food Basket is the only organization in our com-
munity dedicated to attacking childhood hunger.
They serve more than 3,200 children sack suppers
(take-home dinners) five days a week in the Grand Rapids
Public Schools. Students in schools where 80% or more live
at or below the poverty level are the recipients of this sup-
port. Twice a year, at the start of school breaks, the students
are also provided with break bags that include healthy food
and snacks. In 2010, Kid’s Food Basket was awarded a West
Michigan Dental Foundation Community Grant to support
the cost of providing a toothbrush and small tube of tooth-
paste in the break bags. The bags were delivered the week of
December 13, 2010 to each child in the program’s service
area. A total of 3,183 children received a Holiday Break Bag.

orn and raised in West Michigan, Dr. Bouws has
forged a life that satisfies his devotion to family, his
love of travel, his skills as a clinician, his caring for
those in need and his ability to see a need and solve it. His
faith in God has been a guiding principle for his life.
Dr. Bouws, the son of Clarence and Betty Bouws, was born
in Grand Rapids. His father was an accountant and his mother
a registered nurse. Both he and his sister, Barbara Helmus,
have lived and worked in the West
Michigan area much of their life.
Many growing up memories include
visiting Lake Macatawa and trips out
West and to Canada.
His high school years at Grand
Rapids Christian High School were
filled with sports, music, and student
government. John ran track and was a
saxophone and piano player. He was
class president for his first three years
of high school and, as a senior, was
student council president. He contin-
ued his love of the outdoors—fishing,
canoeing, and camping. He was a 4-H
member.
Dr. Bouws enhanced his leadership
skills in student government during
his undergraduate days at Calvin College. As a biology major,
he had the opportunity to be mentored by several professors
in the science department, including Dr. DeVries, Dr. Dirkse
and Dr. Bratt. These fine instructors were very committed
to helping their students who were aspiring to professional
schools.
After his first year of dental school at University of
Michigan, he hitchhiked through Europe for thirteen weeks.
He describes this as a life changing experience that exposed
him to the pleasure and possibilities of travel, which included
access to differing cultures.
The travel experience was additionally beneficial since it
served as the “pick up” lines that he used when he met his
future wife, Beth Beelen, at a Campus Chapel mixer in Ann
Arbor. At the time, Beth was an undergraduate in nursing at
the University of Michigan.
John and Beth married in 1971 after his graduation from
dental school, and moved to Boston. Beth worked as a public
health nurse in Roxbury, Massachusetts and John served a
rotating internship with the US Public Health Service. During
1972-73, he was on active duty in the US Public Health
Service to the Coast Guard.
Public health service dentistry exposed him to the various
dental specialties and hospital dentistry. This was a very
enriching experience, getting much
clinical experience, under the tutelage
of senior officers, who supported him
with a “You can do it!” mentoring.
Upon completing his Coast Guard
obligation, he started private prac-
tice in Grand Rapids. He was espe-
cially appreciative of many in the
West Michigan dental community
for their help during the early prac-
tice years. Drs. Lentz, Sommerdyke
and Bonofiglo were wonderful men-
tors. Dr. Stepanovich was a strong
influence in encouraging him to be
involved with St. Mary’s Hospital to
provide dentistry under anesthesia.
Dr. Bouws provided that service for
more than twenty years.
Dr. Bouws has been active in organized dentistry. He
has had leadership roles at the local, component, state and
national level. Professional memberships include American
Dental Association, the Academy of General Dentistry, the
American College of Dentists, and Luke Society.
His love of travel, dentistry and desire to give to others
have resulted in numerous dental mission trips. Through
the Luke Society, he has participated in five mission trips to
Honduras. His work through the Healing the Children orga-
nization has resulted in three trips to Bolivia and two addi-
tional Honduras trips. There was another mission trip to the
Dominican Republic through the Medical Group Missions.
Dr. Bouws has also volunteered at Clinica Santa Maria and
Mel Trotter Ministries in Grand Rapids.
Dr. Bouws has served as an elder, deacon, and church
school teacher at Fellowship Christian Reformed Church.

His leadership roles at Calvin Christian School included the
school board with four years as the President, the school’s
Foundation President, and Building Committee Chairman.
Though Dr. Bouws has been a generous worker within his
profession and his community, he has always declared that
family comes first. He and Beth have been blessed with four
children – Douglas (Sheri Meyer), Robert (Amy Downing),
Brent (Kristyn TerHaar), and Kara (Jeff Plantinga) and seven
grandchildren – Ellie, Sophia, Katherine, Holly, Benjamin,
Autumn and Savannah.
As the family was growing, time was always made for
family activities and vacations, which included lots of boating
and camping trips. This family’s folklore includes the time
when they were on vacation in Massachusetts in the RV. The
story goes that Beth got out of the RV to help John get out of
a tight parking spot, only to be left behind. The family will tell
of the panic when they realized about an hour later that Mom
wasn’t in the RV. Fortunately, when they circled back, they
found her, patiently waiting for their return.
Dr. Bouws speaks lovingly of Beth’s patience with him, her
love of travel and adventure, and her organizational skills. He
also speaks with great pride about his children and the direc-
tions they have taken in their lives.
For John Bouws, dentistry was an ideal profession. It has
afforded him many experiences and opportunities. He has
used his many skills to give back to society.
Congratulations to Dr. John Bouws, this year’s Silent Bell
recipient.

t the WMDDS Holiday Dinner and Awards Night
on December 16, 2010, five WMDDS members were
recognized for achieving MDA and ADA life mem-
bership. To be an MDA life member, one must be 65 years old
with 35 total years of membership, or if retired and haven’t
reached 35 years total, have been a member since graduation.
To be an ADA life member, one must be 65 years old with 30
continuous years of membership. The dentists receiving life
membership in 2010 were: Dr. John Bouws, Dr. John Fellows,
Dr. Earl K. (Tom) Phares, Dr. Robert Rosebery, and Dr. Clare
Van Wieren. Of these, Drs. Bouws, Fellows and Phares were
present to be recognized.
Dr John Bouws was raised in West Michigan and attended
Grand Rapids Christian High School, Calvin College, and
received his DDS degree from the University of Michigan in
1971. He served in the United States Coast Guard prior to
starting his private practice in 1973. John has participated in
numerous dental mission trips to Honduras, Bolivia, and the
Dominican Republic. He also volunteered at Clinica Santa
Maria and Mel Trotter. John and his wife, Beth, love to travel.
They have four children and seven grandchildren. Dr. Bouws
was also awarded the WMDDS Silent Bell at this event and you
can read more about him in the Silent Bell article in this issue.
Dr. John Fellows is from Grand Rapids. He graduated from
Ottawa Hills High School and attended Grand Rapids Junior
College. He received his DDS degree from the University
of Michigan in 1971. John held officer positions and was
president of Kent County Dental Society from 1985-1988.
He served on the MDA Scientific Committee from 1984-1995
and hosted many speakers at MDA annual session. John wore
jeans as much as possible. He is the recipient of the “Elaine
Fleming Always Late to Register or Reply” award. He is look-
ing for a new hobby to replace the absence of University of
Michigan football on Saturday afternoons.
Dr. Earl K. (Tom) Phares graduated from Indiana University
with an AB degree in 1967. He received his DDS degree from
Indiana University School of Dentistry in 1973. Tom served in
the US Navy Dental Corps from 1971-1973 on Marine Corps
Air Station in Jacksonville, NC. He practiced in the Holland
area from 1973-2010. Dr. Phares was president of the Holland/
Zeeland Dental Society, president of the Michigan Society of
Dentistry for Children, and was Ottawa County representa-
tive to the WMDDS board. He also served on the WMDDS
Peer Review Committee, and the Peer Review Committee
for the Michigan Dental Association. He is a Master in the
Academy of General Dentistry and a member of the American
College of Dentists. He and his wife, Susan, have two sons.
Matt is married to Dr. Heather Phares, who practices in New
Era. They have two children. His son Thomas has assumed
the practice and is married to Laura, and they have two chil-
dren. For hobbies, he enjoys golf and old Corvettes.
Drs. Rosebery and Van Wieren were unable to attend the
awards night. Dr. Rosebery is a general dentist and practices
in Sparta, MI. Dr. Van Wieren is an oral surgeon who prac-
tices in Holland, MI.

The Grand Rapids Dental Hygienists’ Society
would like to announce that it has started
a scholarship fund for dental hygiene
students. The scholarships were awarded
this year to two second-year dental hygiene
students – Hannah Sterk from Ferris State
University, and Staci Carroll from Grand
Rapids Community College. We wish them
continued success and support them as they
continue their education and become future
leaders in dental hygiene.

on January 26, 2011 that our AGD-approved
program sponsor status was approved and our
courses once again can be submitted for AGD credit.
They also informed us that all formally structured

back to 4/11/2009 would be approved by the AGD

Report of the December BOT Meeting and Farewell as Your MDA TrusteeBy Dr. Norm Palm, MDA Trustee
he Michigan Dental Association Board of Trustees met
on December 10, 2010, at the Okemos headquarters
building. The following is a brief synopsis of the meeting.
The Board reviews its strategic plan at the start of each
meeting. Executive director Drew Eason and staff give the
Board a brief update on the progress in achieving the strategic
plan. We then discuss one area of the plan as part of a more
complete update. At this meeting we discussed the goal of cre-
ating retention and recruitment committees in each compo-
nent district. At the MDA level this group is a subcommittee
of the committee on membership. Related to this project, Josh
Lord, our MDA staff person with membership and recruit-
ment, along with Alexa Vitek, membership committee chair,
discussed plans to merge two related committees. Presently
the MDA has a special committee on the new dentist as well
as a membership committee. The plan is to combine the two
committees, keeping the number of members the same, and
include a subcommittee on retention and recruitment. The
plan includes voting privileges for the four student members.
Staff member Tom Kochheiser presented a report on the
Michigan Oral Health Coalition and asked that individual
Board members and member dentists consider joining this
group. The MDA was an original sponsor of the MOHC along
with Delta, the Michigan Department of Community Health,
and the Michigan Primary Care Association back in 2003. The
coalition has various work groups on increasing fluoridation,
prevention programs, improving access to care, and support-
ing educational programs related to these issues. The MOHC,
along the MDCH, has received funding thru CDC grants in
the past for sealant programs. More recently, the MOHC has
had fewer dentists as active members. This becomes a concern
in this environment when progressive causes in workforce

may choose to encourage the MOHC to become an advocate
for a change in the dental workforce that we could not support.
The Board authorized a workgroup more than eighteen
months ago to look at redesigning our current radiography
training program. The new program would include digital
radiography training, and would be available online. The
development of the new course has progressed to the point
that the Board was able to commit $150,000 to finish devel-
oping and then to implement the new course. More detailed
information on the availability of the new course will be
forthcoming by this fall.
The Dental PAC Board of Governors was appointed and
Steve Dater is our district IV member. Steve is also our local
WMDDS legislative affairs chairperson as well. Thanks, Steve,
for getting the job done with our local legislative contact list.
You need to know your new representative and senator in the
Michigan legislature. There are over 30 new senators, and 60
new house members in the legislature. Contact your legisla-
tors, get on his mailing list, and tell their staff you would like
to attend any future fundraising events. Then go to these
events and introduce yourself as a member of the WMDDS
and the MDA. Let your representative or senator know who
you are. You do not need, nor should you feel it is necessary
to discuss any agenda with your representative or senator at
these events. At these events small talk will do. Over time a
relationship can be developed that can be valuable at a future
date when an issue arises that we do need access to our repre-
sentatives and senators to discuss. Building these relationships
now is especially critical, as there are many new faces in the
Michigan house and senate.
The committee on annual session reported a small deficit
for the 2010 meeting held in Novi. Increasingly, keeping
expenses for annual session in line with income has been a
challenge as the number of vendors is difficult to maintain.
The committee reported it is considering a small registration
fee or fewer free courses and some increases in fees for other
course offerings. The Board made comment on the matter.
The annual session committee has since opted to not institute
a registration fee, but rather adjust the fees for course offer-
ings marginally higher.
MDAIFG staff chairperson Craig Start reported that they
have been receiving numerous inquires from members about

the increased premiums with Blue Cross medical coverage.
The IFG staff will assist members in looking at their coverage
and options that might be available to soften the rate increase.
The increases reflect the mandating of certain preventive and
diagnostic procedures as well as coverage for those up to age
26 on their parents’ plans. These changes in coverage were
part of the health care reform. These mandates have resulted
in an average 16% rate increase for the MDA plan. Across the
country similar increases in rates for medical insurance are
occurring.
On a happier note, The Michigan Department of Natural
Resources and Energy, in cooperation with the MDA, is a
recipient of a grant from the EPA for the installation of amal-
gam separators. The $270,000 grant is for new installations
with a $300 rebate available per dentist. The grant monies will
be available shortly and will be available on a first-come, first-
serve basis. Call the MDAIFG for more details.
This report is my last to you as I finish my six-year stint as
one of your West Michigan trustees. I hope to continue to be
active in the MDA as vice president next year. I will look for-
ward to serving in that position at the discretion of this year’s
House of Delegate.
Colette Smiley will be your new trustee and join Brian Cilla
on the Board. Colette has a warm and reassuring manner that
is only matched by her poise and ability. More than capable
and always artfully articulate, you will be well represented by
her. She will become an influential Board member. Thanks,
Colette, for your willingness to serve. I will be one of your
biggest supporters.
My special thanks to Brian Cilla for his work at the MDA
as well as many others around West Michigan who have been
encouraging and ready to offer advice when it was needed
as I served as your trustee. Our profession needs your active
support now more than ever. If you are active, stay active. If
you are not active, become active. Please contact the WMDDS
Board and let us know where you think you can serve. There
will never be a better time than now to serve!
