A Refresher on Medicaid Transformation and ACHs

Medicaid Transformation Project

The Medicaid Transformation Project is a five-year agreement between Washington State and the Centers for

Medicare and Medicaid Services (CMS) that provides up to $1.5 billion of investments in local health systems, to benefit Apple Health (Medicaid) clients.

This work is led by nine Accountable Communities of Health (ACH) regions, each governed by a backbone organization responsible for convening local leaders from multiple sectors that impact health to collaborate on projects that are testing new and innovative approaches to transform how we deliver health care.

ACHs: About Us

Each ACH, and their community

partners are taking on a locally

established portfolio of projects,

(options listed on the back side)

ACHs draw down Medicaid Waiver

incentive funds as they achieve

milestones and show improvement

in key health outcomes related.

These projects require collaboration

across multiple sectors in order to

be successful.

Existing Healthier Washington Strategies

Because health is more than health care, we

know we need increased collaboration across

communities and partners

• Convening and connecting to identify shared

goals, alignment, roles, and resources.

• Engaging community members (beneficiaries) and

creating a feedback loop for continuous

improvement.

• Coordinating regional data analytics and

strategic planning across communities and

organizations.

• Creating braided funding models that support

collaborative efforts to improve population health

across communities and organizations.
