PRIMARY RESPONSIBILITIES: Attend orientation and training sessions. Participate in general program meetings Attend special team meetings as needed. Learn and be familiar with program’s resources Contribute to the development of individual life plans. Monitor, follow up, compliance with plan. Person to person contact with participants. Phone contacts with participants. Identify available resources and inform clients about them. Keep some minimal record for program files.
