CREATING AN ATS ACCOUNT INSTRUCTIONS

MAKE SURE YOU READ AND FOLLOW EVERY STEP. THERE ARE 7 STEPS.

Step 1: Locate the ATS Athlete Portal Link by going to the Clarke Athletics website, clarkepride.com. Hover the

cursor over INSIDE ATHLETICS, select STUDENT-ATHLETE FORMS from the drop down menu

Step 2: Select MEDICAL FORMS, a new window will pull up

Step 3: Welcome to the ATS Athlete Portal!

Returners will use the username and password they created. If you do not remember your username or password, please contact your Clarke Athletic Trainer

Click LOGIN

Select ATHELTE INFORMATON

Step 4: Once you get logged in, UPDATE all three tabs: GENERAL, INSURANCE, and CONTACT if needed. Yellow

boxes are required to be filled out before moving on. If you do not need to update any of these tabs, move on

to Step 5.

INSURANCE – If you have a new insurance, update it here. Select the insurance policy you wish to

update and click EDIT. Be sure that when you are uploading a picture of the front and back of the

insurance card:

1. The image is clear

2. The print is readable

3. The image of the card takes up the entire frame of the camera

EMERGENCY CONTACT – Select the contact you wish to edit and click EDIT

Step 5: This step is where you will complete the MEDICAL HISTORY FORM and INSURANCE AUTHORIZATION.

THIS IS NOT AN OPTION

Click the FORMS tab.

From the pull down menu, select ATHLETE MEDICAL HISTORY FORM.

Click NEW

Step 6: Please read and answer each question, there are 4 pages.

1. At the end of the MEDICAL HISTORY FORM, you will be asked to sign your name using the touchpad on

your keyboard or your mouse

2. Type your name in the SIGNED BY box. Click SIGN.

3. Make sure the text box says DIGITALLY SIGNED.

4. Click SAVE located at the bottom, left side

Step 7: From the same pull down menu that you found the ATHLETE MEDICAL HOSTORY FORM, select the

INSURANCE AUTHORIZATION FORM. Read and sign the form.

ONCE THE FORMS HAVE BEEN YOU WILL RECEIVE AN EMAIL CONFIRMATION. PLEASE

FORWARD YOUR EMAIL CONFIRMATIONS TO pride.athletics@clarke.edu

They will also appear in the SUBMITTED FORMS box with the date the form was submitted. If they do not

appear in the box, than they did not save correctly and we have no record of you completing them.
