Who We Are
Established in 1980, the Detroit Area Agency on Aging (DAAA) is a nonprofit agency that serves
older adults, adults with disabilities and caregivers residing in Detroit, Hamtramck, Harper Woods,
Highland Park and the five Grosse Pointes. We are one of 16 Area Agencies on Aging in the
state of Michigan and 670 in the nation.
Our Mission
To educate, advocate and promote healthy aging to enable people to make choices about home
and community-based services and long-term care that will improve their quality of life.
What We Do
We assist older adults, adults with disabilities and caregivers by removing social, economic,
physical and psychological barriers and creating aging friendly neighborhoods through nutrition,
health and wellness and senior independence. Our programs and services include:

* Healthy Aging Services * Clinical Services
DAAA’s service provider network provide long term care supports and services to assist seniors
to remain in their homes independently. These services include adult day care, caregiver support,
education and training specialized care for visually and hearing impaired, legal assistance and
other types of support. Five community support coordination agencies make services available
through three service tiers: 1) community navigator, 2) community living support (chore,
homemaker, medication management, personal care, respite care and transportation services)
and 3) community wellness centers in specific zip codes within five service areas in Region 1-A.
Currently, there are 28 congregate meal sites and three Nutrition Service Incentive Program sites
that make meals available to seniors.
About the Eldercare Directory
This directory provides service descriptions for each service, contact information and the service
area and/or targeted zip codes. The 60 Direct Purchase of Service (DPOS) providers DAAA
contracts with who exclusively support MI CHOICE and Project CHOICE are not included in this
directory.

Kinship Support Services
Supportive services such as respite care or supplemental education, support and training for older

DAAA Direct Services
Long-Term Support Services
Care Management for the Elderly:
A program designed to find, mobilize and coordinate a variety of home care and other services
needed by frail: homebound elderly persons age 60 years and older at “high-risk” of entering a
nursing home. The mission of this program is to help the grail, older person remain independent by
helping him/her arrange for formal and informal services in order to prevent premature admission to
a nursing home or other institution.
Care Transition:
Hospital to home program and the goal is to minimize hospital, readmission and emergency visits.
Michigan Medicare-Medicaid Assistance Program (MMAP):
MMAP provides free health insurance counseling for people with Medicare and their families or
caregivers. MMAP counselors are staff and volunteers trained to provide education, assistance and
advocacy related to Medicare and Medicaid.
MI Choice/Medicaid Home and Community Based Waiver for the Elderly:
MI Choice is a home and community-based care option for individuals 65 years of age or older and
disabled adults aged 18 to 64 who are Medicaid Waiver eligible for their long-term care. It is a less
costly and preferred alternative to nursing home care that allows people to get the support they
need in their own homes. Must meet financial and medical eligibility.
MI Health Link:
MI Health Link offers a broad range of medical and behavioral health services, pharmacy, home and
community-based services and nursing home care. MI Health Link provides a care coordinator at
the Integrated Care Organization level to coordinate benefits of all offered services.
Eligibility Criteria for MI Health Link includes:
Live in one of the four regions
Age 21 or over (Adults age 21 or over who are enrolled in the Children’s Special Health Care
Services program are not eligible for MI Health Link)
Are eligible for full benefits under both Medicare and Medicaid
Are not enrolled in hospice
People in a nursing home are eligible and must continue to pay their patient pay amount to the
nursing home
People with Medigap (Medicare supplemental insurance) can enroll in MI Health Link if they
meet all criteria

Nursing Facility Transition Program:
The Nursing Facility Transition Program provides nursing facility residents the opportunity to
transition back to the community. The participant may choose to return to their home, move in with
family or friends, find an apartment or choose another living option in the community which could
include a licensed adult foster care or homes for the aged. Transition planning with our licensed
social workers and/or registered nurses helps identify appropriate support and services needed to
make the transition.

Information & Assistance
A free service for older adults, adults with disabilities and caregivers to give them and their families
a place to turn when they have questions or need help finding resources. Certified
information and referral specialists and options counselors are trained to provide information and
links to community resources and services to help families navigate and understand the programs
and services available to them—whether those programs are offered directly by the DAAA or other
local organizations.

Long-Term Care Ombudsman
Assistance to older residents of long-term care/nursing home facilities to help them resolve
complaints through problem identification, education regarding their rights, providing information
about appropriate rules and making referrals to community resources.
