How to use

Google forms is used to

make survey forms or

polls. It can be used to

plan events. It can be

used to give students

quizzes, and many

more.

Type on your browser

Click on X

Click on New, hover mouse to

more and click on Google

Forms

You will be led to this

dashboard

Select the Colour Palette to put

color on your background

To select a theme, click on where

the arrow is pointed

Select a Theme and click

Select

Put a title for your survey

You can assign a file name

different from the title survey form

Include a brief description

Type your first question

List down your choices of

answers

Include Other for other

answers

Pull dropdown for choices on

how to answer the question

Here are your choices

Use the icon below to duplicate

your question

Delete a Question

Mark Required Questions

Use Shuffle option order to reshuffle

questions

Preview Survey Form

On Preview Mode

Click the plus sign to add

a question

Select paragraph to

provide explanation to an

item

Click on T to add a title and

description to another

survey

You can select checkboxes

Use dropdown to enumerate

answers

Use linear scale to rate

your answer to a question

Use Multiple-choice grid

This is the preview mode

Click on the icon to insert

an image

Select your image

Insert Video

Enter the title of video

Select your choice of

video

Click on Select

Give a title

Include a Caption

Center video

Change image

Another way of inserting

video is to paste the URL of

the video

Go to Youtube, type the title of your

topic

Select your topic and

copy the URL as shown

Click Preview

Click Submit

Include Sections to better

categorize survey forms

Click the dots to duplicate

and delete sections

Make

changes on

the Settings

based on

your survey

requirement

and click

Save

Click the dots to find other

menus

Click Make a copy to

duplicate an entire document

Click OK to Copy a

Document

Duplicated documents are

indicated as “Copy” on the

Filename

Selected section can be

deleted

Select Print to print

survey forms

Click Collaborators to share

your file with others

Click

Change to

make

adjustments

to recipients

of the

document

Secure your file by sharing

only to those who will receive

a link

Click Save

Take note

that the

copy will

only be

received by

anyone who

will receive

a link

Enter the

email

addresses

of

recipients

Send link

via gmail,

google+,

facebook

or twitter

Click

Done

Click on Google+

Include a description

Click Share

Click gmail to share

Supply email address and

subject

Click Send

To share on Facebook

Select your Choice

Click Post to Facebook

On Private message, supply

fb address

Click Send Message

Link sent on Facebook

Link sent on private message

Link sent thru the twitter

Log in to twitter and send

Click Send

Supply an email address

for the recipient

Include a Subject for the

email

Include a message

Finally click Send

Click Responses to see

the result of entries made

in the survey

Responses refers to the

number of respondents

Click on the green button

to create view responses

on sheets

This is the result on Excel

Form

Click the dots for more

choices of menus

Click email notifications

Click Download

responses on Excel form

Click Delete to remove

responses
