Accounts

***IMPORTANT REMINDER*** You should NEVER link a rendering provider’s NPI to the same Business profile as the FQHC. Each rendering provider should have a separate email address, User Profile, and Business Profile.

Now let’s add a rendering provider to your FQHC.

18

Use the Add Affiliation button.

Rendering Provider Affiliation

HINT: To add a rendering provider, start in the Accounts screen.

19

Fill out the application fields in each section. You may complete fields in both the group and rendering provider sections… up until it’s time to sign.

Complete the Application

Keep an eye on the completion prompts to guide you.

Electronic Signature

20

• All applications must be electronically signed. Who can sign? Depends on the application!

• You can sign for your existing FQHC accounts that were

already enrolled.

• For new accounts, the person signing must add themself as an owner or managing employee in the Disclosure section.

• New rendering providers need to sign their own applications

Electronic Signature

21

• When you have completed the group signature, send the application to the rendering provider.

• The rendering provider must sign their part of the application.

• On a Rendering-S application only: You may sign on behalf of a rendering provider if you are an Administrator on the provider’s Business Profile.

Electronic Signature

22

New Group Application (Uncollapsing)

23

• FQHCs currently bill for services under a single NPI for all of their locations – but that is changing.

• In the future, FQHCs will need to “uncollapse” their locations - enroll each location under a separate NPI.

• New Group applications require some additional steps.

24

Use this option to create a New Group application for a new (or previously enrolled FQHC location (Uncollapse). Using this option will save time by auto-populating some of the application fields.

New Group Application (Uncollapsing)

25

New Group Application (Uncollapsing)

Select this option to enroll a new service location for your FQHC.

26

New Group Application (Uncollapsing)

Enter the NPI of the new service location. The NPI should be different than your location that is already enrolled.

Then, select the existing account that most closely resembles the new location, (i.e. same provider type).

27

New Group Application (Uncollapsing)

Operational info about your practice location, (hours, age of patients, managed care participation, etc.)
