Tamyra Garcia serves as the Program and Policy lead for the End-Stage Renal Disease (ESRD) Quality Incentive Program (QIP). The ESRD QIP seeks to promote high-quality services in outpatient dialysis facilities treating patients with ESRD. Ms. Garcia worked as an Epidemiologist at the Centers for Disease Control and Prevention for over 6 years. She is a results-oriented professional with over 14 consecutive years of experience working in the public health sector.

Facility POC

A single user designated for each facility who is the primary POC for ESRD QIP

Note: A single user can be the Facility POC for multiple facilities.

• Draft and submit formal inquiries, clarification questions, and systemic clarification questions (SCQs)

• Communicate with CMS regarding formal inquiry responses and requests for additional information

• Run Performance Score Reports (PSRs), Performance Score Certificates (PSCs), and Patient List Reports

Facility Viewer

All non-POC users at each facility

Note: A facility may have an unlimited number of Facility Viewers, but each facility must have at least one individual with this role.

• Run PSRs and PSCs• View CMS additional
