Beacon Community Update
• Featured successes include:– Early results featuring improvements in quality, cost and population health (all
17 communities have at least 2 measures trending positively)
– Launch of new exchange capabilities in communities like New Orleans and San Diego
– Innovative methods to engage patient through remote patient monitoring and mobile health
• Collaboration between 8 Beacon Communities and 6 EHR vendors to advance interoperability and exchange– T h ce ollaboration has now enabled 51 primary care practice locations
representing 432 providers and 447,000 patients to exchange a consistent patient summary care document to better manage transitions of care and to populate community data repositories or registries
