A quarter of the population in Scotland is taking four or more drugs and most of these patients are over 50 years old. In Glasgow this figure is higher.
Adverse reactions to drugs are a factor in 5-17% of hospital admissions of older people

Transformation Programme Building on the work established under the Change Fund programme 2011-2014
This year 32 diverse local and city-wide Third Sector organisations will focus on: •Improving the health of older people and/or their families and carers•Reducing social isolation of older people•Increasing capacity and resilience of older people and/or their families and carers•Building the capacity of individuals and communities to be more connected
Some funded services will work mainly or partly with older people and carers with whom they had already engaged through their previously funded transformation activities. Others will work predominantly or wholly with new beneficiaries this year. GCVS continue in their role as the lead partner for this award with Voluntary Action Fund (VAF) contracted to manage the Fund on their behalf.
Over 600 voluntary-managed organisations in Glasgow are GCVS members
VAF provides grants to, and works with voluntary organisations and
community groups across Scotland.
