How we got here Background of Austin/Travis County CHA

and CHIP MAPP Model

Community Health Improvement Planning Process

August 2011- December 2012 National Association of City and County Health Officials (NACCHO) awards

grant to build community support for a CHA/CHIP and required a robust CHA be developed.

January – June 2012Community Health Assessment (CHA) is conducted to identify the health related needs and strengths of Austin/Travis County.

July – December 2012Community Health Improvement Plan (CHIP) is drafted. CHIP is a long-term, systematic effort to address public health problems and is based on CHA results.

January – June 2013Developed Implementation for CHIP (I-CHIP) Year 1 Action Plan, a clear sequence of steps to achieve strategies.

July 2013 – PresentImplementation of the CHIP and public forums or presentations.

At the CIC Conference in 2012, on behalf of A/TC Community Health Improvement Planning, Lawrence Lyman with Travis County Health and Human Services and Veterans services, presented the CHA/CHIP work.
