01 00 00 GENERAL REQUIREMENTS

2. PROJECT COMMENCEMENT:A. OBTAIN WRITTEN APPROVALS FROM AUTHORITIES HAVING JURISDICTION PRIOR TO COMMENCING FIELD
OPERATIONS ON ANY PORTION OF THE PROJECT.
3. INFECTION CONTROL AND RISK ASSESSMENT (ICRA):A. PROJECTS WITHIN A LICENSED HEALTHCARE FACILITY SHALL COMPLY WITH APPLICABLE ICRA GUIDELINES - REFER
TO ICRA INFORMATION WITHIN THE CONTRACT DRAWINGS.B. SCHEDULED ICRA MEASURES SHALL BE IN PLACE PRIOR TO COMMENCING FIELD OPERATIONS ON ANY PORTION OF
THE PROJECT.C. SCHEDULED ICRA MEASURES SHALL BE STRICTLY ENFORCED FOR THE DURATION OF THE PROJECT.
4. STANDARD PROJECT PROCEDURES:A. MAINTAIN TEMPORARY ENCLOSURES AROUND CONSTRUCTION OPERATION AREAS AS DETERMINED BY THE
OWNER'S REPRESENTATIVE TO LIMIT ACCESS TO NON-CONSTRUCTION PERSONNEL AND TO PROVIDE CONTINUED SAFE OPERATION OF SURROUNDING CONTINUOUSLY OCCUPIED AREAS.
B. PROVIDE EQUIPMENT AND CONDUCT CONSTRUCTION OPERATIONS AS NECESSARY TO CONFORM WITH THE SAFETYPROGRAM DETERMINED BY THE OWNER'S REPRESENTATIVE.
C. CONDUCT REGULARLY SCHEDULED PROJECT MEETINGS FOR THE DURATION OF CONSTRUCTION AS DETERMINED BYTHE OWNER'S REPRESENTATIVE.
D. SCHEDULE DELIVERY OF MATERIALS WITH THE OWNER'S REPRESENTATIVE TO AVOID CONFLICT WITH ADJACENT ONGOING SYSTEM OPERATIONS
E. DRAWINGS ARE DIAGRAMMATIC INDICATIONS OF WORK TO BE PERFORMED; FIELD VERIFY DIMENSIONS OF ACTUALCONDITIONS PRIOR TO COMMENCING WORK OR ORDERING MATERIALS.
F. PROVIDE SHOP DRAWINGS/CATALOG CUTS OF SCHEDULED EQUIPMENT AND MATERIALS FOR ALL INSTALLATIONCONDITIONS PRIOR TO COMMENCING WORK OR ORDERING MATERIALS.
G. CONTRACTOR/VENDOR PAYMENTS WILL BE ISSUED MONTHLY FOLLOWING APPROVAL BY THE OWNER'S REPRESENTATIVE OF INVOICES OR PAYMENT REQUISITIONS SUBMITTED IN ACCORDANCE WITH THE SYSTEM'S PROCEDURES FOR PAYMENTS.
H. USE FM GLOBAL APPROVED MATERIALS WHERE APPLICABLE AND AS REQUIRED.
5. SPECIAL PROJECT PROCEDURES:A. FIELD OPERATIONS AND INSTALLATION OF MATERIALS INTENDED TO CONTAIN OR RESIST THE PASSAGE OF
RADIATION OR ELECTRO-MAGNETIC ENERGY SHALL COMPLY WITH APPLICABLE PROCEDURES AS DETERMINED BYTHE SYSTEM'S DEPARTMENT OF MEDICAL HEALTH PHYSICS.
B. MEASURES SCHEDULED BY MEDICAL HEALTH PHYSICS SHALL BE IN PLACE PRIOR TO COMMENCING FIELD OPERATIONS ON ANY PORTION OF THE PROJECT.
C. MEASURES SCHEDULED BY MEDICAL HEALTH PHYSICS SHALL BE STRICTLY ENFORCED FOR THE PROJECT DURATION.
D. MATERIALS AND INSTALLATION PROCEDURES SCHEDULED BY MEDICAL HEALTH PHYSICS SHALL BE PROVIDED INSTRICT ACCORDANCE WITH CONTRACT DOCUMENTS.
6. PROJECT CLOSEOUT:A. PROVIDE A MINIMUM OF FIVE (5) COPIES OF OPERATION AND MAINTENANCE MANUALS FOR EQUIPMENT INSTALLED
ON THE PROJECT FOR ACCEPTANCE BY THE SYSTEM.B. PROVIDE A MINIMUM OF THREE (3) COLOR COPIES OF AS-BUILT DRAWINGS MAINTAINED BY THE PROJECT
FORMAN/SUPERINTENDENT.C. PROVIDE A MINIMUM OF THREE (3) COPIES OF WRITTEN DOCUMENTATION OF FINAL INSPECTION/OCCUPANCY
CERTIFICATION FROM EACH AUTHORITY HAVING JURISDICTION PRIOR TO ALLOWING SYSTEM OCCUPANCY OF RECENTLY CONSTRUCTED AREAS.
02 00 00 SITEWORK
1. SITE IMPROVEMENTS:A. PROVIDE APPLICABLE SIGNAGE AS DETERMINED BY THE OWNER'S REPRESENTATIVE. SIGNAGE SPECIFICATIONS
ARE AVAILABLE SYSTEM FACILITIES ADMINISTRATION, (570) 271-6938.
2. HAZARDOUS MATERIALS:A. HAZARDOUS MATERIAL ABATEMENT SHALL BE VIA SEPARATE CONTRACT BETWEEN THE SYSTEM AND QUALIFIED
ABATEMENT CONTRACTORS.B. UNCOVERED POTENTIALLY HAZARDOUS MATERIALS SHALL BE BROUGHT TO THE ATTENTION OF THE OWNER'S
REPRESENTATIVE. CEASE CONSTRUCTION OPERATIONS IN AREAS CONTAINING HAZARDOUS (OR POTENTIALLY HAZARDOUS) MATERIALS UNTIL ABATEMENT IS COMPLETE.
C. HAZARDOUS MATERIAL ABATEMENT SHALL BE COMPLETED PRIOR TO COMMENCING ANY FIELD OPERATIONS.
3. LANDSCAPING:A. LAWN AREAS DISTURBED BY CONSTRUCTION OPERATIONS SHALL BE SEEDED WITH A SEEDING MIXTURE
ACCEPTABLE TO THE OWNER'S REPRESENTATIVES.B. LANDSCAPING MAY BE ACCEPTABLE AT THE DISCRETION OF THE OWNER'S REPRESENTATIVE.
03 00 00 CONCRETE
1. MATERIALS:A. INSTALLED CONCRETE SHALL CONFORM TO SCHEDULED COMPRESSIVE STRENGTH AND SLUMP RATIOS. A MINIMUM
OF FOUR (4) TEST CYLINDERS SHALL BE TAKEN FOR EACH SIX (6) CUBIC YARDS OF CONCRETE INSTALLED AND COMPRESSION TESTS SHALL BE PERFORMED AT 7-DAY AND 28-DAY INTERVALS. DELETERIOUS MATERIAL SHALLBE REPLACED AT NO ADDITIONAL COST TO THE SYSTEM.
B. CONCRETE CONSTRUCTION SHALL CONTAIN SCHEDULED REINFORCEMENT; REINFORCEMENT SHALL BE INSTALLED INA WORKMANLIKE MANNER AND SHALL BE FREE OF RUST, SCALE OR OTHER DELETERIOUS MATERIAL.
C. ACCEPTABLE ADDITIVES SHALL BE RESTRICTED TO ACCELERANT, PLASTICIZERS, AND MOISTURE PROOFING AGENTS, AND SHALL BE SUBJECT TO WRITTEN APPROVAL BY THE OWNER'S REPRESENTATIVE. VERIFY ACCEPTABILITY OF ADDITIVES WITH SUBSEQUENT CONSTRUCTION MATERIALS AND SCHEDULED FINISHES.

REPLACED AT NO ADDITIONAL COST TO THE SYSTEM.B. SCHEDULED W.W.F. SHALL BE INSTALLED AT MID SLAB.C. SCHEDULED REINFORCING BARS, DOWELS, ETC. SHALL BE INSTALLED IN CONFORMANCE WITH DESIGN INFORMATION
PROVIDED BY THE PROFESSIONAL. SUPPORT CHAIRS OR ALTERNATIVE MEANS APPROVED BY THE OWNER'S REPRESENTATIVE SHALL BE UTILIZED TO ASSURE CORRECT PLACEMENT OF REINFORCEMENT.
D. SLAB CONTROL JOINTS SHALL BE SAWCUT WITHIN 24 HOURS OF SLAB INSTALLATION TO A MAXIMUM DEPTH OF 1/2OF THE SLAB THICKNESS; SPACING SHALL BE AS SCHEDULED. CONTROL JOINTS AROUND STRUCTURAL MEMBERSSHALL BE DIAGONAL TO THE DIRECTION OF FIELD JOINTS AND SHALL BE CUT IN A DIAMOND PATTERN.
