Early Notice

• Early notice language added to forms.

• Compare revision date to forms available on website.

Use Current Version of Form

Strike! – Do not delete.

• Strike language that is not applicable, do not delete it.

Affidavit

• Affiant cannot notarize their own affidavit.

• Notary’s Commission cannot be expired.

• Law says petition must be “verified”.
