HandHygiene The CDC estimates nearly 2 million patients in
the United States contract a hospital-related
infection each year. About 90,000 of these
patients die as a result of their infections.* Since
the introduction of the CDC Guidelines for Hand
Hygiene in Healthcare Settings, healthcare
workers now have specific indications for pre-
venting healthcare-related infections that may
ultimately save many lives. To sanitize hands
that are not visibly soiled, the CDC recommends
the use of alcohol-based handrubs. The CDC
also recommends products that contain
emollients to reduce skin irritation and drying.

Hand Hygiene Using VioNexus CDC guidelines recommend the use of alcohol sanitizers for routine antisepsis. Using an alcohol handrub takesone fourth of the time of standard handwashing and provides better antimicrobial effect than washing with anantimicrobial soap.
1. Apply generous amount of soap.2. Rub hands together vigorously for
at least 15 seconds.3. Cover all hand surfaces.4. Rinse. Avoid hot water.5. Dry thoroughly with a disposable towel.
Multiple-use cloth towels are not recommended.

InstrumentReprocessingNo sterilization system can reliably penetrate the
bioburden on soiled surgical instruments; thus
the precleaning step is critical. The Metrex
instrument care line will help the healthcare
facility to adhere to the AAMI guidelines, “Safe
handling and biological decontamination of
reusable medical devices in healthcare facilities
and in non-clinical settings (Section 7,
Decontamination Processes).” Metrex offers a
number of reliable precleaners – EmPower®
Foam, EmPower ® and MetriZyme® – all dual-
enzymatic detergents. These products offer fast,
thorough removal of proteinaceous debris.
MetriSponge,® a specially contoured sponge
saturated with MetriZyme, fits snugly around
endoscopes and cylindrical instruments to
easily remove gross contaminants prior to the
enzymatic soak. MetriLube® coats metal
instruments with an oil film to prevent spotting,
staining and rusting, while allowing instrument
hinges to move freely with precision.

instruments to be cleaned.3. Allow instruments to remain covered with
enzymatic foam to dissolve bioburden.4. Reapply as needed to keep instruments moist.5. Rinse instruments thoroughly.
Preclean...using EmPower Enzymatic Detergent.1. Place items in clearly marked decontamination area.2. Dilute EmPower Enzymatic Detergent 1 ounce
(1 pump) per gallon of water.3. Immerse instruments until soil is dissolved and removed.
If matter is dried on, soak longer until visibly clean.4. Manually clean with brush or irrigate lumens under
the solution level.5. Rinse instruments thoroughly.6. Air-dry or dry with clean towel. 7. Inspect instruments. If instruments are not clean,
repeat steps.
Lubricate…using MetriLube Instrument Lubricant Spray or Concentrate.1. Spray MetriLube directly onto clean instruments. 2. Wipe dry with clean towel.

High-LevelDisinfectionMost healthcare facilities rely on glutaraldehyde-
based products for their high-level disinfecting
and sterilizing needs. The MetriCide® family of
glutaraldehydes has a 20+ year history of excel-
lence. It is a tried and true formula. Available
in three concentrations of glutaraldehyde,
MetriCide is noncorrosive and has excellent
instrument compatibility for the high-level disin-
fection of endoscopes as well as sterilization of
heat-sensitive surgical instruments and other
medical devices. Metrex recommends using
MetriTest™ Strips for testing the minimum effec-
tive concentration (MEC) of glutaraldehyde prior
to each use.

Preclean1. Immediately wipe the insertion tube with a MetriSponge. 2. Suction the EmPower or MetriZyme detergent through
the scope and the biopsy/suction channels until the solution is clear. Alternate detergent/air.
3. Flush air/water channel per endoscope manufacturer’s instructions.
4. Attach water-resistant cap to videoscope. Transport scope separately from accessories to reprocessing room.
5. Repeat steps 2 and 3 several times before finishing with air. Attach soaking cap to videoscope.
Perform Leak Test6. Remove adapters and valves.7. Use a leak tester to pressurize the scope before
submerging it in water, following the endoscope manufacturer’s instructions.
Manually Clean8. Immerse scope in EmPower or MetriZyme detergent.9. Wash debris from exterior of scope.
10. Clean inside all valve openings and elevators.11. Brush entire suction/accessory channels, using
appropriate adapters to access all channels.12. Repeat steps 9 -12 until endoscope is completely free
of debris.
Rinse13. Rinse the scope and removable parts repeatedly with
clean water, flushing all channels.14. Purge water by flushing all channels with air. 15. Wipe exterior of scope dry and remove parts using soft,
clean, lint-free cloth or sponge. 16. Expel any remaining water by flushing with air.
Disinfect with High-Level Disinfectant17. Test the effectiveness of MetriCide using the MetriTest
Strips before soaking.18. Completely immerse scope in MetriCide. 19. Inject MetriCide into all channels until it exits in the
opposite ends.20. Soak according to MetriCide label, covering disinfectant
soaking tray with tight-fitting lid.21. After soaking, use channel adapters to flush disinfectant
repeatedly, leaving adapters attached for rinse cycle.
Rinse Again (Final)22. Rinse exterior of scope and its removable parts
thoroughly, referring to MetriCide label.23. Flush all channels thoroughly with large amounts
of water. 24. Purge all channels with air using the channel
cleaning adapters.

Testing MetriCide Using MetriTest™ Strips1. Remove one MetriTest Strip from container and replace
cap immediately. (For MetriCide, use MetriTest 1.5%Strips. For MetriCide 28 or MetriCide Plus 30, use MetriTest 1.8% Strips.)
2. Dip indicator end of the strip into the MetriCide solution being tested for a full 2 seconds and remove immediately.
3. Lean strip against bottle (indicator end down) and wait 60 seconds. Use a paper towel underneath to absorb excess reagent dye.
4. Read color change at 60 seconds after strip is removed from solution. Disregard the top 2mm of indicator pad, which will be yellow. Pad will be completely purple to indicate the concentration of glutaraldehyde is above its minimum effective concentration (MEC). If ANY yellow appears, apart from the top line, the solution is below MEC and should be discarded. See visual standard below for color change interpretation.

Following SurgeryTo help prevent the blood from clotting inside the suctioncanister, immediately dispense PremiCide following the endof surgery or after the suction canister has been completelyfilled.
• Wear hand, eye and face protection when handlinginfectious waste and using PremiCide.
• Shake and squeeze bottle to loosen any settled material.• Following surgery, open the appropriate port and place
the PremiCide cap onto the open port and push downto secure proper fitting.
• Following surgery, turn off suction and remove thepatient tubing. Close all ports and activate suction.Firmly grasp the base of the cap and pull upward on thebottle. Suction will pull the PremiCide powder into thecanister.
• Once the PremiCide bottle is empty, tap the side of thebottle to remove any remaining powder. Push down onthe bottle to close the system.
