Returning Student Registration

Returning Student Registration If your children are enrolled in the district, the Returning Student Registration process allows you to review existing information and update that information as needed.

1. Log into your Parent Portal account.

2. Select the Online Registration option from the Index

3. Review the Registration Information that displays. 4. Select the 19-20 Registration Year from the dropdown list and click Begin Registration.

Once you have logged in and chosen your language, a welcome screen will appear, click “Begin Registration”. Navigate through the screens to review/enter student data.

Student(s) Primary Household Household information will be pre-filled from the current information in Infinite Campus. Review the information in each tab and click Next to move to the next tab. Primary Phone:

Home Address: Review and verify the address. If your address needs to be updated; type in the house number and the first few letters of the street name and a listing of addresses to select will appear below. Once your address appears, click on the address and the remaining fields will be filled. If your address does not appear, you will not be able to continue with registration. Contact the ASC as indicated on this screen. You will be required to provide proof of residency to your child’s school or upload using the link in the screen, before address can be updated for your student.

Once you click on the address in the box and the remaining fields are completed, review the address. In this screen you will have the ability to upload your proof of residency. Click “Upload Proof of Residencel”. Click Next.

Mailing Address: Check the box if the mailing address is the same as the home address, uncheck if the it’s different and enter the mailing address information as you entered your home address. Click Next.

Income Survey: Check the box if you do not wish to provide your income information for the Fee Waiver. If you would like to complete the survey, begin with entering the total household members, select Monthly Income and Payment Option at the end of the survey. Click Save/Continue.

Parent/Guardian Information Enter information in each tab and click Next to move to the next tab. Demographics:

Contact Information: Enter additional phone number and email address. Click Save/Continue.

Click on “Add new Parent/Legal Guardian” to add a second Parent/Legal Guardian, otherwise if you have no additional members you may continue by clicking Save/Continue.

Emergency Contacts Enter emergency contact, these contacts will not be pre-populated from previous years. Click the “Add New Emergency Contact” button. Although emergency contacts are not required, it is recommended to have an additional contact person in addition to the Parent/Legal Guardian in the event you can’t be reached. If you wish to continue without the addition of an emergency contact, Click Save/Continue.

Adding New Emergency Contact: First Name, Last name & Gender are required. Click Next.

Contact Information: At least one phone number is required. Click Next.

Verification: Enter the address for this emergency contact. This information will only be used to verify the contact doesn't already appear in our system. Click Save/Continue.

To enter additional emergency contacts, click the Add New Emergency Contact button. Repeat the instructions for entering emergency contacts. If no further additions are needed, click Save/Continue.

Non- Enrolled Children Enter all children living in the same household but not attending school. Doing this creates a record in the district's SIS system for future use. To add a new household member, click Add New Household Member. If there are no non-enrolled children to list, click Save/Continue. Adding a child not currently enrolled.

Demographics: Enter First, Middle & Last Name, Birth Date & Gender. Click the Save/Continue button.

Continue adding by clicking Add New Household Member, if you are finished click Save/Continue.

Student Click on Add New Student to enter student information.

In the next student tabs, you will be asked to review or enter information regarding the student you are enrolling. You must complete all required information, before continuing on to the next screen.

Demographics Enter/select the following about the student:

Race Ethnicity Required by Federal reporting, mark the following checkboxes related to the student's race/ethnicity.

Housing Mark either “Yes” or “No” for the student's homeless status. If “Yes” is marked, several options are made available to best describe the student's current housing situation.

Military Connected Families Mark either “Yes” or “No” to help identify Illinois military families.

Relationships- Parent/Legal Guardians Select the relationship, contact preferences and contact sequence for each person. If this person should not have a relationship with the student, mark the “Remove” checkbox.

Relationships - Emergency Contacts

If emergency contact(s) is/are listed this screen will be available, select the relationship and contact sequence for each person. If this person should not have a relationship with the student, mark the “remove” checkbox.

Relationships- Other Household

Select the relationship to student type from the dropdown list. If this person should not have a relationship with the student, mark the “No Relationship” checkbox.

Health Services - Emergency Information

Click the checkbox if you agree with the statement and enter the student's primary care provider name and phone number (clinic name or doctor's name). If you chose not to check the box, click next.

Health Services - Medical or Mental Health Conditions

If the student does not have medical or mental health conditions, mark the “No medical or mental health conditions” checkbox and click next. Doing this will remove the entry fields from panel.

Click the “Add Condition” button. Additional fields will appear to select the Health Condition for the student, enter any Comments related to the health condition. If additional conditions exist, click the “Add Condition”.

Medications Medications can be listed for Daily Medications, Emergency Medication or Medication as Needed. If the student does not have any medications to list, mark the” No Medications” checkbox and click Next.

Click on the “Add Medication” button. Enter the Medication. Select where the medication is taken, select Medication Type and enter any Comments and Instructions related to health condition. If additional medication exists, click the “Add Medication”.

If Medication is to be given at school, click the link to print the “Medication Authorization Form”. When finished, click Next.

Health and Wellness Consent Form Select “Yes” or “No” to the following Health and Wellness consent form. If you select “No”, enter name and date. Click Next. If you agree select “Yes” additional information will be required to complete.

Release Agreement The following release agreement items are required to be reviewed. Click on the link to review the required registration information. A new window will launch that contains District Authorization Forms.

After reviewing, close the window and you will be returned to the registration screen. Click the box to accept and sign your name in the signature box. Review and make payments for fees by clicking on link “Please click link to review and pay fees.”

A new window will launch to Revtrak. Login or create an account. Once the student fees have been reviewed and paid for, close out of Revtrak to return to the registration screen.

Once you have reviewed your fees, you can close out of Revtrak to return to the registration screen.

Click Save/Continue.

Additional Student Forms

The following grade levels will have additional student forms to complete.

Kindergarten: Preschool Survey regarding students preschool experience.

Grades 9-12: Consent for TB Screening and Testing

Completed Registration Once you have completed the registration steps and you are ready to submit your application click “Submit”. You can view and print a copy of the information you have entered by clicking on “Application Summary PDF” link.
