Introduction
Medication safety:
Medication safety may be defined as safe and quality usage of
medicine. Adverse drug events (ADEs), including medication errors and adverse
drug reactions (ADRs) have a significant impact in the individuals who take
medications. Pharmacists have great role to prevent these impacts and ensure
the patient safety.
Pharmacist:
A professional who prepare and dispense drug and medicines.
Pharmacists are very familiar with medication ingredients, interactions and
caution. A pharmacist is a health care professionals who cooperates with,
consults with and sometimes advises the patient about concerning drug.
Medication Safety
For reducing error and harm from medicines through safe
and quality use of medicines.
It’s also helping us to achieve our objective of leading
and coordinating national safety and quality for better
health care.
Why medication safety needed?
Medication Errors
Medication errors are unintended mistakes in the
prescribing, dispensing and administration of a medicine that
could cause harm to a patient.
They are the most common preventable cause of undesired
adverse events in medication practice.
Medication errors causes a major public health burden.
Medication errors cause at least one death every day and injure approximately 1.3 million people annually in the United States.

Individual pharmacist play individual role in medication
safety for improvement of patient health.
The role of pharmacists in medication safety can be
categorized in three major areas which are given below-
1. Hospital
2. Clinical and
3. Community.
Roles of the hospital pharmacist in medication
safety
There are three responsibilities of hospital pharmacist for both
In and Out patient, those are given below-
Dispensing Area’s Responsibilities:
Ensure that established policies and procedures are
follows
Check for accuracy in the work of supportive personnel
Maintains professional competence
Insures the proper techniques are used in
extemporaneous compounding.
Patient Care Area’s Responsibilities: Inspect periodically the medication areas on the nursing unit to
ensure and adequate supply of stock drugs and their proper
storage.
Identifies drug brought into clinic.
Assist in drug product and entity section.
General Responsibilities: Provide drug information as necessary to pharmacy, medical and
nursing stuffs.
Coordinates overall pharmaceutical needs of the ambulatory
services.
Provide adequate control.

Role of the clinical pharmacist in medication
safety
Prescription monitoring.
Medication history-taking and medicines reconciliation.
Communicates patient information to the physician.
Assist physician to select drug product, dosage form and schedules.
Patient education and counseling.
Monitoring the patients total therapeutic drug level, effectiveness,
side effects, toxicities, allergic drug reactions, drug interactions and
appropriate patient outcome.
Provide drug information to the patient, medical & nursing stuffs.
Clinical pharmacists plays a vital role in the hospital. Their role are as follows-

Dispensing prescriptions medicines to the public, Check
dosage, ensure the medicine are correct and safe and level it.
Liaising with doctors about prescriptions.
Supervising the preparation of any medicines.
Counseling and advising the public on the treatment of minor
aliments and any adverse side-effects of medicines or
potential interactions with other medicines/treatments.
Role of the community pharmacists in medication safety are given below-
Overseeing the ordering and safe storage of medical
products and in some cases, arranging the delivery of
prescription medicines to patients.
Keeping up-to-date with current pharmacy practice, new
drugs and their uses.
Managing, supervising and training pharmacy support stuff.

Hospital simply couldn’t function without highly trained
and highly skilled Pharmacists.
Pharmacists produce personalized medicines allowing for specific and individual treatment for every patient for medication safety.

Conclusion
Pharmacists plays a vital role in medication safety. The need of pharmacists is rising day by day for better health care of the patient. Only pharmacists contact with the patients, doctors ,nurses and other medical stuff for better medication.
