Nurses Notes
End of the School Year
Very soon school will be over and summer will be here. Students who have medications in the clinic will be receiving forms for next year by May 10, 2016.
By the last day of school all medications must be sent home either with the student or picked up by the parent or the parent’s representative. I am required to discard/destroy any medi-cation still in the clinic after the last day of school.
Also included are medication forms for your child’s physician to complete over the summer so that you have the forms and the medication ready to put in the clinic at the beginning of the next school year. These forms would need to be completed after June of 2016. If you are unsure if you will put medication in the clinic, I would recommend that you at least have the physician complete the form over the summer and send the form to the clinic. You can de-cide at a later date if you want to actually keep the medication in the clinic. No medication, prescription or nonprescription (even cough drops) can be taken to school and used without a least a parent note (for one time use less than 15 days) or this medication form completed by the physician and on file in the clinic.
For students who have special medical conditions that require an emergency plan, I will also send home a blank emergency plan for you to review and have signed with the physician and parent signature.
Summer Sun: It is very strong in Texas.
Remember to apply sunscreen to your child before
they leave for school so they are protected at recess.
A good website to learn about health tips for parents
and children is http://kidshealth.org. It has articles for
children and for parents.
Illnesses:
Remember that you need to report your absence to the attendance clerk. If your doctor diagnoses your child with a contagious illness, you will need to let the school nurse know.
Thank you.
Please feel free to call the clinic if you have any ques-tions or concerns. The clinic hours are 8:00am to 4:00pm daily.
We are excited to announce an online enrollment system for new student registration. A new student to Katy ISD is a student who has never attended a district school or was not enrolled in Katy ISD at the end of the previous school year.
How do I get started? Visit the KISD online registration page and click the New Student Registration link. Select “Create Account” on the left. You will need to create a unique login and password for your account This allows you to securely save your work and come back at a later time if necessary. You can use your email address or cell phone number. Please remember this login and password as Katy ISD utilizes InfoSnap each year to update information on students re-turning to our schools. Do I have to answer all the questions? Questions marked with a red asterisk (*) are required. What if I make a mistake? If you would like to make a change, prior to submitting the form, you can either navigate back to the page using the “< Prev” and “Next >” buttons. Or if you are on the Review page, click on the underlined field. If you have already submitted the form, then you will need to contact your student’s school, so they can make the changes for you. I’ve completed the form, now what? Once you have finished entering your information, click “Submit” and you will receive additional instructions. YOU STILL MUST GO TO THE CAMPUS YOUR CHILD WILL ATTEND TO COMPLETE YOUR REGISRATION. All of the information you’ve entered will electron-ically be sent to the school. If you cannot click on the Submit button, you will need to make sure that you have answered all RE-QUIRED questions.
What if I have more than one student in the district? Do I need to do this for each child?
Yes, because you’ll need to provide information that is specific for each child. We recommend that you complete and submit one
form and then start another – this will allow you to “snap” (or share) selected family information, which saves you time.
I’m not sure how to answer a question. I don’t know what the question is asking.
You can contact your child’s campus.
Help! I’m having technical difficulties.
If you experience technical issues completing the online form in Infosnap, you may contact the Infosnap support line at 866-986-
1601 x7095 or email support@infosnap.com. If you forget the user credentials, you may have them e-mailed to you and/or use
security questions set up previously to retrieve that information. You may also contact technical support.
