hospital along poor roads

• Recruitment crisis and temporary

closure of community hospital …

Over 2000 people take to the streets in protest

Achievements 12 months on from the march:

A new town newspaper full of health promotion messages

Achievements 12 months on from the march:

• A community led GP recruitment campaign - video spread by the community on social media and twitter with 7000 views

• 4 doctors recruited

• No further adverts placed

Promoting self triage and self help

Spotting cancer early

Further achievements 12 months on from the march:

• Care homes support scheme

• New access system in general practice + many other innovations

• Nurse linked to community group mobilising the population for health and wellbeing

• Pharmacy minor illness scheme promoted and use now

highest in Cumbria

• Community hospital reopened and significant reduction in length of stay

• Dual trained nurse practitioner MH and physical health

• Successful infrastructure bid to NHS England

• First advanced community paramedic in the North West

• Care Navigator recruited

• Near patient testing introduced and new X-ray facility

• Successful dragons den pitch for developing a System Leadership Programme with NW Leadership Academy

• Telehealth – live link to A&E. Also planned links to outpatients, prison and local hospice

• Action group has surveyed the town and our model is designed around their priorities

• First donor community in UK

• Park run every weekend

• School plays on using the health services properly and

healthy eating initiatives , GP involved

700 children taught how to use the NHS

to Home Programme Simplifying the language - talking to people,

communities and stakeholders

Why? • We had clinical models, specifications, PPI data, diagrams

etc

• We needed to have a meaningful conversation

• So that we could build support and enable our stakeholders to respond

• But it was really difficult for us to tell our story in a way that we truly accessible

• So, we used animated stories; to show the model as

whole and focus on individuals
