•Generate evidence that digital health tools can positively impact this group

• Digital health is flourishing, but not focused on low income, high‐need individuals
•Many challenges faced by this population lend themselves to digital health solutions

All eligible entries may be considered for optional, invitation‐only, discussions with Kaiser Permanente, the Association for Community Affiliated Plans and/or the Alliance of Community Health Plans regarding potential piloting opportunities.

• Functioning solutions that address one or more of the challenges faced by super‐utilizers
• Built out enough that could be piloted with consumers• Apps for Android, iOS, Blackberry, Windows phone, web,
and mobile web; SMS‐based (text) solutions, and wearables
• Only US‐based companies• Demo video (hosted on YouTube or Vimeo) that walks
through the main functionality of the submission via screencast or video
• At least one image/screenshot of submission
