What is Lifeline?

Since 1985, the Lifeline program has provided a discount on phone service for qualifying low-income consumers to ensure that all Americans have the opportunities and security that phone service brings, including being able to connect to jobs, family and emergency services. In 2005, Lifeline discounts were made available to qualifying low-income consumers on pre-paid wireless service plans in addition to traditional landline service. Lifeline is part of the Universal Service Fund.

The Lifeline program is available to eligible low-income consumers in every state, territory, commonwealth, and on Tribal lands. Consumers with proper proof of eligibility may be qualified to enroll. To participate in the program, consumers must have an income that is at or below 135% of the federal Poverty Guidelines or participate in a qualifying state, federal or Tribal assistance program.

Customer Ordering StepsStep One – Customer completes Lifeline form (Pages 2 & 5)Step Two – Customer shows proof of qualification and completes Proof form (Snap Card, Medicaid Card, etc.)

Document One – Page 2 Lifeline Enrollment Form

Customer must complete top section, initial all items, circle proof and sign and date.

You can help with the top section if you must, but customer must initial and sign. You can add the date if you must. And this should only happen if the customer cannot write.

Document Two – Page 5 Optional Lifeline Household Worksheet

This is important because NLAD may show this person’s address different in their database and for people who live in a location with more than one person who qualifies.

B. ________ I understand that violation of the one-per-household requirement is against the Federal Communication Commission’s rules and may result in me losing my Lifeline benefits, and potentially, prosecution by the United States government.

Document Three – Proof Organization Form

A Tempo Employee must see the proof and ID card. This document is important because it will help us if NLAD has incorrect information on the customer. We will destroy this document after we review it for privacy reasons.

Dealer Order Steps

Step One – Enter the information in portal

Enter portalplus.lightyear.net into your browser

Dealer/Sub Dealer – Enter

Login – ayournumberPassword – ayournumber

After logging in, click on the Menu and click on Wireless Order.

Enter Zip Code to see if the customer qualifies.

Click on the Customer Information Tab.

Click on Phone Information.

Select the Product. (LG Rumor Touch, etc.)

Select the Plan and Get a New Number or Port an Existing Number.

Select Cash Payment because its free and click Submit.

Important Training Items:

1. Customer clearly initials all the proper sections of the Lifeline Certification Form.

2. Customer signs the Lifeline Certification Forms.

3. You circle the proof items that you are turning in on the Lifeline Certification Form. For example, if they have a SNAP CARD, circle that.

4. Customer completes the entire Lifeline Certification Form.

5. On your proof document make sure you have:• Picture ID• Proof• Write the portal account number on the proof page• Write the customer’s current address clearly on the proof page• Write the customer’s Social Security Number and Date of Birch clearly on the proof page

6. Do not enroll anyone that has enrolled for Lifeline with anyone else in the last 60 days. If someone already has a phone and has had it for longer than 60 days, it may take up to 90 days to switch them.

7. If you are emailing the forms and proof, send them together in one email at a time. Do not send multiple customers in one email. You can fax or mail the information to the fax and address on the new Lifeline Certification Forms.
