1. What is a Copay (Copayment)?

• A copay is a flat rate you’ll pay for a specific service.

• Once you pay the copay, your insurance will pay the rest of the cost.

A doctor’s visit costs $175.00

and your copayment is

$50.00.

You will pay the doctor $50.00 at the time of your

office visit.

Your insurance company will then pay the

remaining $125.00

2. What is a Pre-existing Condition?

• Pre-existing conditions are excluded from coverage under this policy

• They are known medical or psychological conditions that you have had and received treatment for during the 6 month period prior to your insurance coverage.

What is a Claim Form?

• A claim form is a printed document that is used to request reimbursement (money back) from your insurance company.

• This form can be found on Secutive’s website

How to Submit a Claim for Reimbursement

Download a claim form from the Secutive

website and fill it out completely

Take a photo of your receipts to keep and

send the receipts stapled to the claim

form to:

Not Feeling Well?

Go to Urgent Care or Walk-in Clinic (not the Hospital)

• Physicians and nurses are there to help you

• You will pay a $50.00 co-pay

• Open 7 days a week and typically in the evenings.

Illnesses such as…

Sore throats, minor cuts, cold/flu, sprains and strains, urinary tract infections, earaches, simple fractures or minor burns can be treated there.

If you have a Medical Emergency… Call 911 or go to the nearest Emergency Room (ER)

Chest Pain

Loss of Consciousness

Life threatening situations

Difficulty breathing

Seizures

You will be charged $350.00 if you use an ER for a condition

that is not considered life threatening

If the Doctor Gives You a Prescription…

Ask for the generic version

of the medication.

prices in your area.

Have your prescription filled at the

pharmacy and pay for the medication.

Submit a claim form along with 1. your receipt

for the Doctor’s visit AND 2. the receipt for the

cost of the medication to your insurance

company…

Wait to receive your 100%

reimbursement (money back)

from the insurance company.

Save the receipts for your medications! You will need to send:1. a copy of the prescription receipt 2. the bill from the Doctor’s visit AND3. the claim form to the insurance company to be reimbursed.

Do Not Use the Phrase “Office Visit” in Your Claim Form!

Office Visits to a private Doctor’s office WILL NOT be

paid for (covered) by your insurance.

Seeing a Doctor at an Urgent Care Center WILL be paid for (covered) by your insurance

Seeing a Doctor at the Emergency Room in a

Hospital WILL be paid for (covered) by your insurance

if it is a life-threatening emergency.

When you submit a claim form to your insurance

company, do not use the phrase Office Visit

Dental Care in the U.S.

Dentists in the U.S.

• Typically have private practices and are expensive

• Don’t accept your insurance

Dental Clinics in the U.S.

• Are usually part of a large university with a medical school

• Offer much less expensive dental care

• Are staffed by Dental Interns

Call us and we can assist you with finding a clinic close to where you are.

Or, enter in your internet search box…College of Dentistry and the name of your city (or closest large city) and search for an on-campus dental clinic.

2. What is ?

• Pre-certification is the process of obtaining permission (authorization) from the insurance company for you to have a medical procedure done.

If you are hospitalized, please call the 24 hour United Healthcare within 48 hours at 1-800-251-1712 or send an email to:

If you become ill and are told by the Physician that you need to have a surgical procedure, the first step for you is to call United Health Care to get permission (authorization) to have the surgical procedure.
