Correct selection of subcategory ensures relevant questions will be asked

For this example, we will use Single PFM Implant crown- pre-cementation

Agree to Statement of Understanding

Event Report-1st Page

Event report 1st page divided into 4 sections:

• When? Event Identification-date & time of event

• Who? Subject- identifies the subject

• Where? What? Event Context- location and description of event

• How severe? Event classification

– Verifies category and subcategory

– Requires user to enter severity of event

When? Event Identification

The date of discovery defaults to today’s date. Please update if needed.

The date of event is the date the event occurred. Ideally the event report is entered as soon as possible so that more details are remembered.

“Patient” is the default selection. The subject can be changed based upon the category selected.

Who? Identify Subject

Enter identifying information such as MRN-Medical Record Number. It’s equivalent to our EHR or “E” number. Then click “Search Patient” button. Or you may search for the patient using their name, and/or date of birth.

Where? What? Event Context

Select the correct location of the event.

Please do not use “DN-Student Clinical Teams” for DMD Teams events. Instead select DN-Team. Expand list to select correct team.

ALL events that occur in the DMD Teams clinics should have the team number as the location! Please do not select “DN-Prosthodontics” for pros lab cases as that is not the location where the event occurred.

What? Event Context

Enter the facts of the event in the “Event Description” field:

The field allows 500 characters. Event descriptions should be brief and factual.

How severe? Event Classification

Select the severity of the event.

The Category and Subcategory have auto-populated based upon our earlier entries.

To add a document, such as a letter, select the “Attach Document” button.

Additional Questions:For this type of Implant event, there are four additional questions to be answered:

then select all teeth that apply:

• Selection of the correct lab is crucial!

• To quickly check the lab name in axiUm:

• In patient’s EHR, select “Labs” tab:

If there is more than one lab case, select the lab name of the correct case:

This patient has three lab cases. For this example, we are working on an implant supported crown on #29. We can see #29 under the “Site(s)” column.

The lab name is under the “Labs” column.

When questions are answered, click the “Done” button in the lower right corner.

Enter NameAlthough the option exists to enter events anonymously, if more information is needed, it is very helpful to have the name if the reporter of the event.

Remember, our purpose is to improve patient care!
