Changes to
Informed Consent Documents
We have made changes to the stamp on newly approved informed
consent documents. Starting December 1, 2016, all newly approved
informed consent documents will only have the approval date
stamped at the top; the expiration date has been removed.
All study teams will still be required to use the most recently approved
informed consent version.
Example of how stamped/watermarked consents will now look:
Can the PI or study team sign
up as a participant in their
own study?
Enrolling spouses, dependents or
study team members creates a
perception of bias and the potential
for undue influence in the informed
consent process and is thus not
allowed by the USF IRB. Study team
members can participate in research
conducted at the University, but not
in a study in which they are on the
study team or the spouse or
dependent of a PI or study team
member. Contact the QA-QI team
