Amy McGuire Porterexecutive director and Ceo, national osteoporosis Foundation

“It’s important to try to get enough calcium from the foods you eat. When your diet does not have enough calcium for your body’s needs, calcium is taken from your bones.”

CHALLenGesMay is National Arthritis Awareness Month, but it is always important to consider the health of your bones and joints.

improve your bone health

Did you know that a woman’s risk of a hip fracture is equal to her combined risk of breast, uterine and ovarian can-

cer? And among those people 50 and older, one in two women and an estimated one in four men will have an osteoporosis-related fracture in their lifetime.

The National Osteoporosis Foundation (NOF) believes that bone health should be on the top of your wellness list. The single most proactive thing you can do to prevent osteoporosis is to understand your risk factors for the disease. Speaking with your healthcare professional about osteoporosis will help you bet-ter understand your own risk for the disease as well as your options to improve or maintain your bone health.

Certain people are more like-ly to develop osteoporosis than others. You are more at risk based on things like your family history, diet, activity level, race and ethnicity or other medical conditions you may have. Many

of the choices you make each day can affect your bones—like get-ting your daily recommended amounts of calcium and vitamin D, engaging in regular weight-bearing exercise and avoiding smoking and excessive alcohol.

By making healthier choic-es you can help to reduce your risk of osteoporosis as well as the painful fractures it can cause. So what can you do today? Start with diet and exercise.

the importance of calciumIt’s important to try to get enough calcium from the foods you eat. When your diet does not have enough calcium for your body’s needs, calcium is taken from your bones. Vitamin D is also important because it helps your body use cal-cium. You may have seen news re-cently about vitamin D daily in-take recommendations released by the Institute of Medicine (IOM). Since 2008, NOF has recommend-ed that healthy people ages 19 to 49 get 400 to 800 International Units (IU) of vitamin D every day and that adults age 50 and older get 800 to 1,000 IU every day. NOF’s recommendations for daily intake

remain higher than IOM’s but fall well within the margin of safety. NOF strongly recommends that you consult with your healthcare professional for your particular re-quirements.

it’s not too late to exerciseIn addition to its many benefits to bone health, exercising reg-ularly is important for overall health and well-being. Like your muscles, your bones get stron-ger when you make them work. Weight-bearing and muscle-strengthening exercises are im-portant for building bones when you are younger and keeping them strong as you age. These ex-ercises can also help improve bal-ance and prevent falls.

It’s never too early or too late to take steps to improve your bone health. It’s important to start making bone health a part of your conversation with your healthcare provider.

The National Osteoporo-sis Foundation is commit-ted to the prevention, diag-nosis and treatment of this

disease. For more information, visit www.nof.org or call 1 (800) 231-4222.

While joint replacements are grabbing the headlines, many Americans are limp-ing along with milder cases of arthritis. they don’t need new joints, but they defi-nitely need some relief.

Exercise can help your joints as well as the rest of your body. However, when you have arthritis, exercise can often be a problem. One of the odder characteristics of mild to moderate arthritis is the way it comes and goes. You may not be able to get up the stairs after breakfast, but by afternoon you are playing tennis. “Let comfort be your guide,” advises Andrew Glassman, MD,

orthopedic surgeon and chief of reconstructive surgery at Ohio State University Medical Center. Exercise when you can, and when you are having an acute phase, pamper your joints with rest, ice, and elevation.

High-impact activities, such as running and jumping, are never a good idea for people with arthritis, particularly of the hips and knees. However walking, swimming, us-ing ski machines, elliptical trainers,

and rowing machines can provide a good workout without doing more harm than good.

over the counter solutionsWhen the pain and inflammation make you too uncomfortable, over-the-counter options can provide relief. Non-steroidal anti-inflamma-tories, such as aspirin or ibuprofen, are the old standbys. They can cause sometimes serious gastrointestinal

problems, so be sure to follow dosage instructions carefully.

Many people swear by alternative medicines. Capsaicin (the chemi-cal that makes chili peppers hot) is a mild irritant that helps relieve pain (but not inflammation) by “tying up the lines that transmit pain signals,” explains Glassman. Other popular alternative treatments are chon-droitin and glucosamine (natural components of cartilage). Research on the effectiveness of these alter-natives is ongoing, but they do seem safe. Glassman advises his patients to try them and see if they help.

ankle injuries. A brace or splint keeps bones and ligaments from tearing which provides protection and reduces swelling.

*1st April 2003 through 31st December 2009.IMPORTANT INFORMATIONIndications: Total Knee replacement is indicated for joint disease resulting from degenerative, rheumatoid and post-traumatic arthritis and for moderate deformity. Contra-Indications: It is not indicated for those with infection, compromised bone stock, skeletal immaturity, and severe instability of the knee, mental or neuromuscular disease. Common Side Effects from Knee Replacement Surgery: Like any surgery, knee replacement surgery has risks which include, but are not limited to, infection, nerve damage, blood clots, reaction to anesthesia, heart attack, blood loss, stroke, and pneumonia. Implant related risk which may lead to a revision of the knee implant include wear of the implant, reaction to particle debris in the joint, dislocation, fracture, loosening and metal sensitivity. The information presented is for educational purposes only. Speak to your doctor to decide if joint replacement surgery is right for you. Individual results vary and not all patients will receive the same post-operative activity level. The lifetime of a joint replacement is not infinite and varies with each individual. Your doctor will help counsel you about how to best maintain your activities in order to potentially prolong the lifetime of the device. Such strategies include not engaging in high-impact activities, such as running, as well as maintaining a healthy weight. When it’s time for a new beginningFred Funk, PGA Golf Professional, shares his experience having a total knee replacement surgery.

How long were you dealing with your knee pain, and what obstacles did you have to over-come before you decided to undergo a knee replacement?

■■ I had been suffering with knee pain for several years and had gone through numerous surgeries to repair torn car-tilage in my right knee. By the end of the season, I was pretty much shot because I was limping around. I had weakness in my knee and my game deteriorated. I couldn’t stay competi-tive anymore. That’s when I decided to have my knee replaced. In 2009, after

discussing my options with my ortho-paedic surgeon, I underwent a knee re-placement surgery.

What was your recovery process like?

■■ Physical therapy is something I was committed to.  The great thing was that with each session I saw the difference I was making and the improvements in my function. The more I committed, the bet-ter it was. After a three-month re-covery, I was back on the Champi-ons Tour playing a full tournament schedule. In 2010, I won the Jeld-Wen Tradition, becoming the first player ev-er to win a PGA Tour-sanctioned event after total knee replacement surgery.

One thing that was really neat was six weeks after surgery my doc told

me, “I want you to go out and hit some balls.” I got on the range and I walked by a bunker. When my knee was really bad for two years I had to just put my feet on top of the sand and try to hit a shot without digging in and I had no stability. My feet would move all over the place. I just had no way I could re-ally go after the shot.

So I saw the bunker. I just walked in it and I started digging in and it didn’t hurt. I went, “Yes, this is cool!t” I could get in and I could actually have my bun-ker game back.

What is the single greatest benefit of successfully recov-ering from your knee replace-ment surgery?

■■ I wanted to have a chance to have my lifestyle back and my lifestyle is

playing competitive golf. I would be the first guy at the level I was play-ing at to have a replacement joint and come back. To this day I am just so glad with the decision I made to have my knee replaced. It was a fantastic de-cision—probably one of the smartest ones I’ve ever made.

What kind of advice do you have for someone struggling with arthritis or joint issues?

■■ I am not a doctor but if you are suf-fering from knee pain and are limited in your daily activities you may want to speak to an orthopedic surgeon about what treatment options may be right for you. I’m glad I did.

ankle Joint rePlaceMent

Consider your options■■ Although not as common as

knee and hip arthritis, ankle ar-thritis is being seen more frequent-ly, usually as a result of an injury to the ankle joint. New joint replace-ments have become available for use in the ankle joint. These newer replacements (or prostheses) last longer and provide better pain re-lief than their predecessors.

Ankle arthritis can be an ex-tremely debilitating problem be-cause walking and other tasks be-come painful. In the past, the most reliable treatment was an ankle fu-sion which ‘freezes’ the two bones of the ankle together. This method relieves pain but also eliminates normal ankle motion and causes an abnormal gait. An ankle joint replacement has the advantage of removing arthritis in the ankle but

does not sacrifice motion. Main-taining ankle motion means that you can walk more normally and prevent the wearing out of other joints in the leg and foot that can occur after an ankle joint fusion.

When considering an ankle joint replacement, you should seek the most intensely studied, wide-ly used and technologically ad-vanced prosthesis. Ankles with a “three-part” design fall into this category. Studies in the U.S. show that this design allows for good an-kle motion, lasts a relatively long time and provides excellent pain relief. Only one ankle replacement with a three-part design has been approved by the FDA for use in the U.S.

When treatments no longer helpto most of us, it still sounds like science fiction, but to the millions of people who have joints replaced each year, it is a chance to get back into life.

Having a joint replaced (knees and hips are the most common-ly replaced joints, but shoulders can be replaced, too) is not, as

you might expect, a small thing. The procedure is done only when pain and disability, usu-ally from arthritis, is so severe that it interferes with normal, daily activities, and other treat-ments no longer help.

While joint replacement might be considered a last resort, it is ac-tually a new beginning. Recent advances in technology have con-

tributed to the success of the sur-gery. Better materials, such as ti-tanium, oxidized zirconium, and strong yet flexible plastics make the joints operate more smoothly and last longer. Computer assisted sur-gery makes the installation of the joint more accurate and less risky. And thanks to the variety of mate-rials and styles available, joints can now be customized to suit the life-

style of the p a t i e n t . A golfer might choose a knee that pivots easily, while an avid gardener might choose a design that supports more bending and squatting.

What was once a crazy idea has now turned into an almost rou-tine way to regain lost mobility and greatly reduce pain. Accord-ing to the American Academy of Orthopaedic Surgeons, 90 percent of patients who have joint replace-ment find that their condition is improved after the surgery. Having a joint replaced won’t make you into the Bionic Man or Woman, but it can get you back on your feet.

■■ What should be on a patient’s checklist when choosing the best solution to their orthopedic surgery?Studies indicate their top concern for knee and hip replacements is how long their implant will last. Active Boomers like me feel they have the right to remain physically active, and they expect their knee or hip replacements to eliminate the pain that’s keeping them from it, and to do so for as long as possible. It is important for them to understand that not all implants are the same, and most surgeons only use one company’s implants—so if longevity is their top concern, they should discuss their options with an orthopedist.

The results of in-vitro wear simulation testing have not been proven to quantitatively predict clinical wear performance. Also, a reduction in total polyethylene wear volume or wear rate alone may not result in an improved clinical outcome as wear particle size and morphology are also critical factors in the evaluation of the potential for wear mediated osteolysis and associated aseptic implant loosening.
