Some important issues related to the clinical relevance of the highly sophisticated and costly neurotological investigations:The most important aspect in all neurotological investigations is neither the sophistication of instruments that are being used nor the excellence of the infrastructure. It is the dedication, commitment, honesty, expertise, clinical insight and knowledge of the man behind the machine i.e., the person operating the instrument which should ideally be a medical person and not a glorified switchpresser / technician which is most often the case not only in our country but even in the most developed countries. The clinician who has ordered the test should also have sufficient knowledge to gauge the authenticity of the test and interpret the findings in the light of the clinical history and the signs and symptoms of the patient. It is only when each of these is perfectly in order that these sophisticated neurotological investigations have any relevance and clinical significance. Otherwise these are just pieces of nicely printed paper with some meaningless graphs on fancy letterheads that can confuse and completely mislead the most astute clinician. Just ordering some neurotological investigations and then taking clinical management decisions on the basis of the printouts doled out from a clinic without verifying its authenticity and reliability and without matching it with the clinical findings is ridiculous if not dangerous to say the least. Most neurotological investigations can be extremely misleading if it has not been done by the correct person. Unfortunately, getting the 'correct person' is not easy and it remains the weakest link in the chain. Not uncommonly most of the neurotological investigations are done by paramedics and sometimes even by untrained staff none of whom have the requisite clinical knowledge and insight which is essential for carrying out the test ethically. These tests are not just pressing some computer switches and then collecting a printout from the printer. Most of the neurotological investigations require extremely sophisticated evoked potential recording systems, very advanced image processing technologies and advanced computing methods all of which are prone to a lot of artifacts that can mimic and may not be distinguishable from the actual response. The active cooperation of the patient is also very very important for a proper recording free from artifacts.

  • Someimportantissuesrelatedtotheclinicalrelevanceofthehighlysophisticatedandcostlyneurotologicalinvestigations:Themostimportantaspectinallneurotologicalinvestigationsisneitherthesophisticationofinstrumentsthatarebeingusednortheexcellenceoftheinfrastructure.Itisthededication,commitment,honesty,expertise,clinicalinsightandknowledgeofthemanbehindthemachinei.e.,thepersonoperatingtheinstrumentwhichshouldideallybeamedicalpersonandnotaglorifiedswitchpresser/technicianwhichismostoftenthecasenotonlyinourcountrybuteveninthemostdevelopedcountries.Theclinicianwhohasorderedthetestshouldalsohavesufficientknowledgetogaugetheauthenticityofthetestandinterpretthefindingsinthelightoftheclinicalhistoryandthesignsandsymptomsofthepatient.Itisonlywheneachoftheseisperfectlyinorderthatthesesophisticatedneurotologicalinvestigationshaveanyrelevanceandclinicalsignificance.Otherwisethesearejustpiecesofnicelyprintedpaperwithsomemeaninglessgraphsonfancyletterheadsthatcanconfuseandcompletelymisleadthemostastuteclinician.Justorderingsomeneurotologicalinvestigationsandthentakingclinicalmanagementdecisionsonthebasisoftheprintoutsdoledoutfromaclinicwithoutverifyingitsauthenticityandreliabilityandwithoutmatchingitwiththeclinicalfindingsisridiculousifnotdangeroustosaytheleast.Mostneurotologicalinvestigationscanbeextremelymisleadingifithasnotbeendonebythecorrectperson.Unfortunately,gettingthe'correctperson'isnoteasyanditremainstheweakestlinkinthechain.Notuncommonlymostoftheneurotologicalinvestigationsaredonebyparamedicsandsometimesevenbyuntrainedstaffnoneofwhomhavetherequisiteclinicalknowledgeandinsightwhichisessentialforcarryingoutthetestethically.Thesetestsarenotjustpressingsomecomputerswitchesandthencollectingaprintoutfromtheprinter.Mostoftheneurotologicalinvestigationsrequireextremelysophisticatedevokedpotentialrecordingsystems,veryadvancedimageprocessingtechnologiesandadvancedcomputingmethodsallofwhicharepronetoalotofartifactsthatcanmimicandmaynotbedistinguishablefromtheactualresponse.Theactivecooperationofthepatientisalsoveryveryimportantforaproperrecordingfreefromartifacts.

  • Theinsightandclinicalknowledgeoftheoperatorandinterpreteraswellasthatoftheclinicianisofparamountimportanceindistinguishingartifactsfromactualgraphs.ThisisamajorlimitationofallneurotologicalinvestigationsbeitanENGoraVNGtest,anECochGoraBERAtestandthisistheprimaryreasonwhyneurotologicalinvestigationsarenotaspopularastheyshouldbeamongstpractitioners.Themajorityofdoctorsdenouncetheneurotologicalinvestigationsandhaveafeelingthattheyactuallydonotserveanymedicalpurpose.Thisisnotwithoutreasonbecausethesetestsaremostlydonebythewrongpeoplewhodonothavetherequisiteinsight,ethicsandhonestyanddoleoutsomenonsenseinthenameofneurotologicalreports.Unfortunatelyinmanycases,suchreportsareacceptedbyclinicianswithoutverifyingitsauthenticity.Thisissometimesbecausetheclinicianisnotsufficientlytrainedinjudgingtheauthenticityoftheinvestigationsorsometimesduetoreasonsthatarebestnotdiscussedinpublicforums.Properlydoneandproperlyinterpretedneurotologicalinvestigationsdonebythemodernmachinesbytheproperpersonsdopresentatremendousamountofclinicallyrelevantmaterialwithoutwhichascientificneurotologicaldiagnosisisnotpossibleandwithoutadefinitediagnosisadefinitivecurativetreatmentisnotpossible.Onlyamedicaldoctorwithsufficientclinicalknowledgecandojusticetotheneurotologicalinvestigationsanditisnotsomethingthatparamedicscanbereliedupon.Anotherissuethatneedstobestressedinregardtotheinvestigationsofthevestibularsystemisthenecessityofatestbatteryapproachi.e.,combiningtheresultsofmanyteststoarriveatadiagnosis.Itistotallydifferentfromjustcarryingoutabloodtestandfindingmalariaparasitesinthebloodandthendiagnosingitasmalaria.Maintenanceofbalanceinvolvesdifferentsystemslikethevestibularsystem,theneurologicalsystem,themusculoskeletalsystem,thevisualsystemandeventhecognitiveandpsychologicalsystems.Nosingletestcandiagnosethewidevarietyoflesionsthatcanbemanifestedasvertigoorimbalance.Justthevestibularlabyrinthswhicharetheorgansforsensinghead/bodymovementshavefivedifferentpartswhichneedtobetestedseparatelybydifferenttests.WhiletheENG/VNGtestscanpickupsomeofthelabyrinthinelesionsinvolvingthevestibuloocularreflex(VOR),yettheydonottestthewhole

  • vestibularlabyrinth.Theybasicallytestonlythelateralsemicircularcanalandthesuperiorvestibularnerve,nothingmuchmore.Totesttheothersemicircularcanalsinthevestibularlabyrinth,namelytheanteriorandposteriorcanals,othertestslikethevideoaheadimpulsetest(VHIT)isnecessary.Totesttheotolithorgansinvestigationslikethesubjectivevisualvertical(SVV)andthevestibularevokedmyogenicpotential(VEMP)testsarerequired.ThissacculeandtheinferiorvestibularnerveisspecificallytestedbythecervicalVEMPtest.Thevestibulospinalreflexsystemcanbetestedbycraniocorpographyandthedifferentformsofposturographylikestabilometryandcomputeriseddynamicposturography.Totesttheconductivityofthenervesthattransmitthebalancesensationfromtheproprioceptorsindifferentpartsofthebodytothebrainaswellasthatofthenervesthatconductthemotoroutputfromthebraintothedifferentmusclesofthebody,testslikenerveconductionvelocity(NCV)studyandthesomatosensoryevokedpotential(SSEP)studiesarenecessary.Notonlythesevestibularinvestigationsbutalsootherrelatedinvestigationslikepsychometricstudies,audiologicaltestsandophthalmologicalstudiesarealsoimportantinalotofpatientsthatpresentwithbalancedisorders.Thebalancesystemisoneofthemostcomplexsystemsofthehumanbodyanditsstructural/functionalintegritycanonlybeassessedbyacombinationofmanytests.Nosingletestisastandalonetestinevaluatingthefunctioningofthebalancesystem.Nodoubtthatthesearecostly,cumbersomeandtimeconsuming(butdefinitelynoninvasiveandpainless)tests,buttofathomthecomplexfunctioningofthebalancesystemaverythoroughevaluationofthedifferentconstituentsofthesystemismandatory.
