Please note: PTCB requires CPhTs to earn only pharmacy technician-specific (“T-specific”) credit to qualify for recertification. ALL CPE credit must be T-specific.

You will have until Saturday, April 20, 2019 to complete your online evaluations and submit your request for CPE credit. Do not delay in completing your KCHP evaluations. When completing your program evaluation for credit, if a CPE program number ends in both “P” and “T”, pharmacists must choose the “P” program and technicians must choose “T” program.

POSTER PRESENTATIONSINSTRUCTIONS FOR RECEIVING CREDIT FOR POSTER SESSIONSFriday, April 5, 2019 (1.0 credit hour)
To take advantage of important research, KCHP is excited to provide the opportunity for continuing pharmacy education credit during the poster presentation session at the KCHP 2019 Annual Meeting. The session is worth 1 credit hour. You must review and discuss 4 posters in a single session to receive 1 credit for that session.
Please Note: 4 posters = 1.0 Contact hour/0.1 CEU, no partial credit is available

Instructions on how to obtain CPE credit for the poster session
1. Identify four (4) posters in a single session that interest you and that will expand your education, knowledge and continuing professional development (CPD) at the KCHP Annual Meeting.
2. Add the Poster ID# and title to your Poster Session CPE Verification Forms on the paper provided in your meeting folder.
3. Visit the selected posters and discuss them with the authors/presenters. When reviewing a poster, be sure to evaluate the model used for the initiative, how the data collected was assessed and what outcomes were observed. Learners are encouraged to ask questions of the poster presenters.
4. Please request any financial disclosures from poster presenters and indicate the response in the Financial Disclosure box on the Poster Session CPE Verification Form.
5. Ask the presenter (or co-presenter) to sign your form once you have reviewed your selected poster. When you have all four signatures for the session, please bring the form to the validation station at the meeting registration desk.
6. Meeting staff will review and keep your documentation for ACPE poster attendance records.7. Meeting staff will provde the access code for the session(s) you attended. Write the codes in your
syllabus or notes.8. You will then complete your CPE process by going to stlcop.edu/ce and evaluating all the CPE
program(s), including the poster sessions. See page 8 for detailed instructions on completing your CPE evaluation process for credit.

MISSIONThe mission of the Kansas Council of Health-System Pharmacists Foundation is to advance public health and pharmacy practice in hospitals and health systems throughout Kansas by supporting and promoting research, training and education.
VISIONThe vision of the KCHP Foundation is to support the provision of safe and effective medication use in Kansas hospitals and health systems through:
1. Facilitation and promotion of research that leads to measurable improvements in safe and effective medication use,
2. Development and support of education and training programs designed to target health system and societal issues impacting safe and effective medication use,
3. Reward and recognition of institutions and individuals (pharmacists, technicians, residents and students) who have achieved excellence in performance, practice and/or leadership related to medication use in Kansas.
FOUNDATION ACTIVITIESThe KCHP Foundation was founded in December 2010. In that short amount of time, the Foundation has hosted a golf tournament fundraiser, Royals tailgate and multiple gift basket fundraisers. Look for the gift basket silent auction at this year’s Annual Meeting and the Fundraiser at Lawrence Beer Company, both of which will support the KCHP Foundation. The KCHP Foundation is also an official Annual Meeting program sponsor.
POSTER AWARDSThe KCHP Foundation is involved in multiple aspects of the poster sessions at the Annual Meeting. Volunteers from the Foundation Board coordinate the review of abstracts submitted for presentation for originality, methodology, and applicability to health system pharmacy. Posters presented at the meeting are judged and awards in each of the following categories are awarded, as applicable: original presentation, encore presentation, and student presentation.
CONTRIBUTIONSThe KCHP Foundation has many ideas to further it’s mission and vision in the years to come. These goals cannot be reached without the support and generosity of KCHP members and the facilities at which they practice.
