Executive Summary

Leprosy situation in Bihar as on 31st March 2009 was as follows: Projected Population -100979764, Total number of new cases detected- 20086.

PR – 1.07 ANCDR /100000 --19.89, Child proportion among new cases- 16.64%,

Gr-2 proportion among new cases - 2.1%. Reaction/Neuritis----1010

ANCDR /100000, PR / 10000, Child % and Disability grade II% of Patna district-

19.0,1.1,16.9, & 2.6; Bhoj Pur-12.1,0.6,20.2, & 2.7; Buxar 15.7,0.86,17.2 & 0.8; Aurangabad- 18.5,

0.9, 14.8 & 2.2; and Nawada are-25.8, 1.2,16.2 & 2.6 respectively.

ANCDR of all the districts have declined progressively but Nawada district is having higher

rate than state average. PR of Patna and Nawada are more than the state average. Child ratio in all the

districts are in the average range except Bhoj Pur, which is having 20.2 % of child ratio, though PR is

only 0.6. In this district healthy contact is being given priority.

Bihar was the first state in India, where NLR started it’s work, but the support teams were

placed in the year 2000. With a gap of 9 months after withdrawal of DTSTs on 31st March 2007, NLR

project started functioning with the concept of capacity building of DNTs since January 2008 having

project area of five districts in the southern part of the state. Districts covered by NLR are Patna, Bhoj

Pur, Buxar, Aurangabad and Nawada.

Status of NLEP in the NLR districts is going on well but due to non- availability of

reporting and recording formats are affecting the activities. Monitoring of NLEP activities from the level

of CMO and above needs further attention.

A financial procedure needs further simplification. Orientation training of peripheral staff with other

senior health staff to be continued to maintain tempo of the programme.

Introduction Bihar is a state in eastern India having it’s capital at Patna. Bihar is bounded in north by Nepal,

south by Jharkhand, east by West Bengal and west by UP. Though in the year 2000 it’s southern mostly

tribal belt was separated and a new state Jharkhand was formed, still it is one of the populous

state of India having projected population of 1009797764 ( 8,28,78,296 as per 2001 Sensus ) with

SC population of 1,45,26,077 and ST population of 16,68,333. Bihar’s population is mixed of all the

religions namely Hindus, Muslims, Sikhs, Christians, Buddhists etc. The state is historically very

important. The state has remnants of “Ashoka The Great”, World’s first democracry of Vaishali,

Nalanda university, Birth place of sikh guru Guru Govind Singh Jee , has added stars on it’s glory.

Bodh Gaya as a Buddhist pilgrim as well as Gaya as a Hindu pilgrim has kept the state on

international map.

The state of Bihar having more than 70% of it’s population below poverty line, several

constraints and obstacles including natural calamities, environmental, socio-cultural and administrative

has been performing very well in NLEP with the support of ILEP ( four members NLR,DFIT,LEPRA

and

TLMI). Bihar was the first state in India, where NLR started it’s work, but the support teams were

placed in the year 2000. NLR is supporting five districts in the southern part of the state including the

capital.

NLEP goes on under the umbrella of NRHM. During 2008-09, no fund was released for the activities

and as such no NLEP activities could be undertaken.

Again in 2009-10 NLEP came on track and normal functioning of the programme started since June

2009.

NLR India is providing support in five districts of Bihar namely Patna, Bhojpur,

Buxar, Aurangabad and Nawada. In these districts routine activities like capacity building of DNTs in

respect of case identification, diagnosis, management, supervision skills, SCG activities in PAL homes

was done with desired results.

PATNA DISTRICT PATNA is the capital of the state of Bihar, situated almost in the centre of the state. The

district is bounded in the north by the river Ganges, south by Jahanabad district, east by the districts of

Begusarai & Nalanda and in the west by Bhojpur district. It covers an area of 3202 sqkms with a projected

population of 5738496 including the SC population of 955378 and ST population of 13257. Patna has

58.16% of rural population. Overall M:F ratio is 1000:873 with literacy rate M:F 73.81% & 52.17%

respectively.

Patna has very good network of Health Care System having two medical colleges, one Dental

college and a very advance health care system IGIMS which is also has been allowed to start medical

education. These are in addition to the district’s health infrastructure ( Annex-1) which is having 23 PHCs,

60 APHCs, 16 urban health posts. Patna has a DNT comprising of DLO, MO, MSW, NMS and a

physiotherapist. ( Annex-2)

BHOJPUR DISTRICT

Bhojpur is the neighbouring district of state capital at about 60 KMs. It’s boundaries are in the north

river Ganges, south Rohtas & Jahanabad district, east Patna district and west Buxar district. It is the birth

place of Babu Kunwar Singh and Babu Jag Jivan Ram. The district is spread in an area of 2334 sqkms

with a projected population of 2722261 having SC population of 393781 and ST only 11234. Major 86%

of Bhojpur district lives in 1007 villages. M:F ratio of the district is 1000:900 having literacy rate of

74.78% and 42.81% respectively.

The district of Bhojpur has one district hospital, 12 PHCs, 27 APHCs and two referral hospitals.

DNT is having DLO, NMA, HE and PT .

BUXAR DISTRICT Buxar is the western district of Bihar.It is bounded in the north by river Ganges,south by Rohtas &

Bhahbua district, east by Bhojpur district and west by UP. Buxar is famous as taposthali of Maharshi

Vishwamitra of Ramayan erra. It has two battle grounds which has witnessed battles between Sher Shah

Suri & Humayun and Britishers & Mugals.

Buxar covers an area of 1757.6 sqkms having projected population of 16,68,275 mainly of rural

92.82% people. SC population is 2,24,861 and ST are 2923. M:F ratio of the district is 1000:901 with

literacy rate of 72.82% and 40.36% among male and females.

Buxar has one district hospital, 7 PHCs,& 18 APHCs. DNT consists of DLO, NMA, only two

member DNT.

AURANGABAD DISTRICT

AURANGABAD is a part of southern Bihar, bounded in the north by Jahanabad and Arwal

district, south by Palamu of Jharkhand state, east by Gaya district and west by Rohtas district. G.T. road

passes through Aurangabad. The district is famous for Surya temple in Deo block, 10 kmsfrom H/Q.

The district is spread in 3305 sqkms with projected population of 2442850 ( SC 558768, ST

nil) having rural population of 91.6% in 1978 villages. Ratio of M:F is 1000:936 and literacy rate 71.99%

and 42.4% among Male and Female respectively.

Aurangabad has one district hospital, 11 PHCs, 55 APHCs, and 03 Referral hospitals. District

Nucleus Team consists of one I/C DLO, 01MSW and 01 NMA.

NAWADA DISTRICT Nawada is 125kms from state capital. It is one of the hottest area during summar. National highway

connecting Bihar and Jharkhand divides the district into two unequal halves. It is surrounded by Nalanda

& Shekhpura districts in the north, Jharkhand state in the south, Jamui district in the east and Gaya district

in the west. The area of the district is 2494 sqkms and projected population 2150839 with SC 754309 &

ST only 1865. Rural population living in 978 villages, constitutes it’s 92.3%.M:F ratio is 1000:948 and

literacy rate 61.2% & 32.64% respectively.

DNT consists of an I/C DLO, NMS and PT. Nawada has 14 PHCs,21 APHCs,and two referral hospitals.

Other Developments

Most important stake holders in addition to DNT are BSHS, DHS and ACMO. The

ACMOs have been delegated the power to draw the cash in consultation with DM ( chair person of

DHS ). Funds for planned activities including IEC are released from BSHS to DHS of concerned

districts. Fund for the year 2009-10 was released in the second half of June’09.

Due to poor coordination among the members of the societies approvals and release of funds get

delayed. A guideline for vehicle hiring needs further simplifications.

NLR fund allocations for activities have been always in time and as per demand. For

transport there was no earmarked allocation but approval for required expenditure was there.

NLR quarterly review meeting was arranged and conducted at Hotel Windsor Exhibition

Road Patna on 12th

and 13th

August 2009.

Finance

The planned activities for NLR Bihar were NLEP Staff orientation training ,ANMs training

on DPMR of whole of Nawada district, and stake holders meeting of all the five districts and SCG

activities in PAL homes of these districts. Unplanned activities were training of trainee and trained

Nurses of PMCH and NMCH Patna.

Activities preformed were NLEP Staff training of all the five district with expenditure

incurred Rs 19453.00,ANMs training of Nawada district Rs17515.00, Stake holders meeting of only

two districts Rs 1950.00, Training of Nurses of PMCH Rs 8439.00, Training of Nurses of NMCH Rs

5000.00, Expenditure on SCG activities Rs 47218.00 and on NLR review meeting was Rs 19817.00.

Expenditure on all the activities has been under budget allocations.

Conclusion & Recommendations

ANCDR of all the districts have declined progressively but Nawada district is having higher rate

than state average. PR of Patna and Nawada are showing more than the state average. Child ratio

in all the districts are in the average range except Bhoj Pur, which having 20.2 % of child ratio,

Though having PR only 0.6. In this district healthy contact is being given priority.

Status of NLEP in the NLR districts is going on well but due to non availability of reporting and

recording formats are affecting the activities. Monitoring of NLEP activities from the level of

CMO and above needs further attention.

Financial procedures needs further simplification. Orientation training of peripheral staff with

other senior health staff to be continued to maintain tempo of the programme
