LETTER to theCommunityThe Community Health Center Foundation and the Ohio Multi-County Development Corporation (OMCDC) represent a valuable partnership of staff and resources that work together for the betterment of the community at-large. Each have separate goals, but strive to serve those in need — the Foundation by elevating worthwhile causes in the community and OMCDC by developing and operating programs to meet crucial needs.

In this annual report you will see the ways these two organizations serve the community and work to enhance services provided at OMCDC and Com-munity Health Center.

Ohio Multi-County Development Corporation (OMCDC) is a community development corporation and a sister organization to the Community Health Center Foundation. The organization provides affordable housing opportu-nities to low and moderate income persons in Carroll, Cuyahoga, Holmes, Lorain, Medina, Portage, Stark, Summit and Wayne Counties. HOUSING PROGRAMSIn 2011, OMCDC provided 265 housing units for low-income individuals in Northeast Ohio. Receiving much of our funding from the U.S. Department of Housing and Urban Development and the Summit County Continuum of Care, OMCDC received the most funding awards in the county to provide housing and supportive services in our area.

TRANSITIONAL HOUSINGOMCDC’s transitional housing programs allow residents rent subsidy for up to two years while tenants reintegrate into an independent lifestyle. Rent is determined on an individual basis and in some cases, a portion is saved on the tenant’s behalf and returned back to them at the end of the program.

Horizon HousePopulation: Scattered group homes for homeless “older adolescents” ages 18–22 years old. Residents are required to be pursuing an education and part time employment. Number of units: 16Location: Summit County

Peachtree Estates IPopulation: Two year housing for single parents who were recently reunited with their children.Number of units: 6Location: City of Cuyahoga Falls

Project HopePopulation: Housing for single men and women in recovery.Number of units: 20Location: scattered apartments throughout Summit CountyIn 2011, more than $11,000 in savings was returning to six successful gradu-ates of the Hope program. This money helps give our participants a founda-tion for their new futures.

Touchstone Supportive Housing ProgramPopulation: Transitional housing for single women in treatment. Women with Medicaid (in or outside of Summit County) those with children, who are pregnant or disabled, are eligible. Number of Units: 16; Semi-private suites with shared common areas, on-site child care, dining and laundry.Location: City of Akron

PERMANENT HOUSINGOur permanent housing projects are designed for specialized populations. Once our residents meet the application criteria and continue to abide by the lease agreement, residents may stay permanently. Our tenants are moni-tored by our housing coordinator and provided with supportive services, if needed, with the ultimate goal of insuring that our tenants can remain in independent housing that is suitable for their specifi c needs.

As a community development corporation, OMCDC owns and operates CommStaff Employment Services Agency. CommStaff works to fulfi ll tem-porary and permanent employment opportunities in the medical, clerical and food service fi elds. The organization also can provide payrolling services, background checks and many more human resource functions to help save clients money. The Community Health Center Foundation is a not-for-profi t organiza-tion founded in 1985 to support the programs of Community Health Cen-ter (CHC). The Foundation develops resources that enable CHC to fulfi ll its mission through public fundraising events, grant activities and housing projects. Since 1992, the Community Health Center Foundation has raised over $1.25 million for the support of Community Health Center’s Mission.

Along with an annual campaign, our fundraising events raise awareness about Community Health Center’s mission and build strong relationships with individuals in the community. Our Golf Classic, held annually in August at Fairlawn Country Club, has been sold out for the last fi ve years, raising over $50,000 every year. Break the Cycle of Addiction during National Recov-ery Month in September, is a community awareness event and raises about $25,000 annually since its inception in 2010.

FinancialsCommunity Health Center Foundation’s budget is a mix of funds raised throughout the year that are distributed to Community Health Center to fulfi ll their mission; long term investments in tax credit and other housing projects with OMCDC; and as a landowner of commercial properties.

The Foundation also has established an endowment as part of long-term strategic planning. With a goal of $1 million, this investment vehicle will provide dividends that can support the non-profi t mission of the Foundation and its partners for years to come.
