Hemodialysis in the Kingdom of Saudi Arabia
Introduction
In 2016, a total of 243 hospitals and dialysis centers were
recorded by SCOT in the Kingdom of Saudi Arabia. They were
classified into 5 sectors namely, the Ministry of health
(MOH), Gov’t Non-MOH, Private and Charitable Hospitals,
King Abdullah Hemodialysis Projects and Outsourcing
Dialysis Program (Davita and Diaverum). These centers were
facilitated by 250 Consultant nephrologists, 379
nephrologist specialist, 456 GP’s and 4506 Nurses are
distributed in these hospitals and dialysis centers
throughout the Kingdom, equipped with a total of 7,235
hemodialysis machines and 5,858 outlet connections.
By the end of the year, a total of 16,315 patients were
recorded on hemodialysis therapy, of these 4,150 were new
patients. Blood groups among dialysis patients were also
noted with blood type O averaging 50% and AB being the
least averaging less than 10%. Age Distribution of patients
reveals that most were in the productive age group of 26-65
years (69 %). Sex was also recorded showing that male
patients composed 56% of patients and female (44%). Saudis
represents 85% of HD patients on Dialysis compared to Non-
Saudi with 15%. Vascular access among dialysis patients
showed that AVF or arteriovenous fistulas was the most
preferred site with 61% of the patients having them.
Hepatitis B and C infection has been a continuous challenge
for health care providers and a lot of efforts to control them
were undertaken amongst patients undergoing
Hemodialysis. Hepatitis B infections were in good control
with 4% of patients infected this year; it is worth mentioning
that the number of patients with Hepatitis C has been
continuously declining for the past five years with the
current average of 12% this year. Primary cause of ESRD was
hypertension (38%) and Diabetes Mellitus 40% is the leading
cause of ESRD.
The proportion of hemodialysis patients treated with
Erythropoietin ranges between zero and 100% with average
of 83%. Oral Vitamin D was given to 53% of patients, while
13% of them receive it via injectable. Other medications such
as insulin and oral hypoglycemic were also utilized with 24%
of patients are receiving insulin, 10% are on oral
hypoglycemic and 11% were put on diet control.
Active HD patient’s blood levels such as Hematocrit, albumin
and calcium were also monitored. In which, 19% of HD
patients have Hct level of < 30%, 12% have albumin of <30
g/dl, 26% have phosphorous levels greater than 1.9 µmol/L
and 24% have calcium levels of less than 2.1 µmol/L.
Of the 16,315 patients on HD, 2,708 (17%) are in the active
waiting list or those patients evaluated and are ready for
renal transplantation. While the mortality rate among HD
patients by the end of 2016 is10%.
The number of Dialysis patients is increasing continuously
over the last years; it is expected to exceed the 20,000
patients by the end of 2020.

Dialysis Centers in the Kingdom of Saudi Arabia
In 2016, there were a total of 243 dialysis centers registered in the Saudi Center for Organ Transplantation (SCOT). A 4.7% increase in new
hemodialysis patients were recorded this year. The Dialysis Population Net Annual Increase of 4.6% has also been recorded in 2016.

Hemodialysis centers and affiliation
A total of 243 dialysis centers were recorded and their affiliations
were categorized into five(5) sectors composing of (134) HD Units
from the Ministry of health (MOH), (22), Gov’t Non-MOH, (50),
Private sectors & Private Charitable Hospitals, (6), King Abdullah
Hemodialysis Projects and (31), Outsourcing Dialysis Program
(Davita 13 &Diaverum 18)(see figure 4.2.1.).

Hemodialysis machines, patient nationality and sex
distribution
A total number of 7,235 hemodialysis machines were recorded in
the Kingdom of which, MOH hospitals has 3535(49%) hemodialysis
machines, government Non-MOH hospitals with 890 (12%), private
Hospital & charitable hospitals 1041 (14%), King Abdullah
Hemodialysis Project 562(8%) and outsourcing dialysis program
(Davita, 493 and Diaverum, 714)with a total of 1207 machines or
(17%) (See figure 4.3.1.).

By patient nationality, a total of 13,702 were Saudis while 2,408
were non-Saudi corresponding to 16,315 of patients on
hemodialysis (see figure 4.3.3.). Comparing to the previous year, a
1% increased among Saudi patients are shown while non-Saudi
patients has increased by 4% this year.

Sex distribution among hemodialysis patients had shown that male
patients 9082 (56%) had increased by 1% compared to last year and
females consisted 7233 or (44%). Though comparing from previous
year’s data female patient had increased by 3.7% this year (See
figure 4.3.5. & 4.3.6.).

Hemodialysis patients age distribution
Age group distribution among hemodialysis patients has shown
that majority of the numbers of patients were in the age groups
between 26to 65, composing of 75% of the total HD patient
population. While the trend of age distribution among HD patients
are increasing on ages between 26 to 75 years old, There is a slightly
increased in number seen on ages between less than 10 to 25 years
old comparing to last year (see figures 4.4.1., 4.4.2. and 4.4.3.).

Blood group, types of vascular access and erythropoietin
treatment
Blood group among HD shows no significant changes for the past
years wherein, blood type O (50%) consistently dominates in
numbers among blood groups, followed by type A at (26%), type B
at (19%) and type AB at (5%)(See figure 4.5.1.).

The types of vascular access for HD patients has shown that AVF
(arteriovenousfistula) has been a consistent choice of vascular
access among dialyzing patients with (61%) of total HD population,
followed by Jugular cath. at (26%) which has shown an increasing
trend for the past 5 years, followed by other vascular access at (8%)
and vascular graft at (5%) wherein no significant increase has been
shown (see figure 4.5.3. & 4.5.4. for trends of vascular access).

Erythropoietin treatment among HD patients had a total of 13,014
which is 80% of the entire HD population, in comparison to the
previous year, there was a2%decreaseon the number treated with
EPO-r. There was a constant increase with the use of EPO-r over the
past 5 years (See figures 4.5.5. and 4.5.6.).

HBsAg, HCV and HIV status among HD patients
This year, 734 HD patients were positive for HBsAg which is 4% of
the total HD population in addition to 2,029 (12%) patients positive
for HCV. There was a 1% increase in the total positive HBsAg
infection. On the other hand; there was a decreasing numbers of
HCV infection from the last 5 years, this year a least 1% decrease
was noted compared to the previous year (see figures 4.6.1.
&4.6.2.). Following the trend of HBsAg and HCV infection, there was
a constant decrease in the number of patients infected with HCV
while HBsAg positive patients have a flactuating trend of the
numbers of patients being infected. In addition, SCOT has added
the number of HIV infected patients. (See figure 4.6.3.).

Region wise, HD patients in western region had the highest
percentage of HCV infection composing of 45% of the total number
of HCV+ve patients, followed by central region with 25%, southern
14%, Eastern 9% and Northern 7% (see figure 4.6.4).

Causes of end-stage renal disease
For the past 5 years, the 2 major causes of end-stage renal disease
among HD patients had been due to diabetic nephropathy and
Hypertensive Nephropathy with the latter being the consistent
cause of ESRD (see figure 4.7.1) It is worth mentioning that in the
previous year Hypertensive Nephropathy had been noted as the
leading cause of ESRD. Though this year with (40%) of ESRD
patients, Diabetic Nephropathy has now been again the leading
cause of renal failure followed by Hypertensive Nephropathy with
(38%) and Unknown Etiology (7%). (See table 4.8.).

Prevalence of diabetes mellitus and hypertension in

In 2016, the prevalence rate of patients with Diabetes Mellitus
(DM)Hypertension (HTN)and patient with both (DM & HTN) are
gradually increasing through the years(see figure 4.8.2.). Patients
with both (DM & HTN) still has the highest prevalence rate of 35%,
followed by HTN only with 33% which has increased by 1%
compared to previous year and the DM only with 17% of the total
number of patients, while patients with Non-DM & HTN 17% has
decreased by 1% compared to previous year (See figure 4.8.1.)

Waiting list for renal transplantation
Status of patients for work-up and on active waiting list were also
monitored for HD patients and by the end of this year, a total of
2,708 patients (17%) were in the active list and ready for renal
transplantation with another 20% (3,263) on the waiting list for
work-up. Comparing with the previous year numbers, the
percentage of patients who on the list for work-up remain the same
at (20%) while the percentage on active waiting list was slightly
declined(See figure 4.9.1. and 4.9.2).

In 2016, majority of HD units were from MOH with 134 (55%) hospitals around the kingdom, catering to the largest numbers of ESRD patients
with a total number of 6,765 (41%), followed by Private and Charitable hospital with 50 (21%) hospitals, the outsourcing hemodialysis Diaverum
& Davita with a combined 21 HD units or (14%) which has increase by 3% since the Gov’t is shifting their patients to these facilities, Military with
9 hospitals, King Abdullah Project 6 hospital, other hospitals 4, National Guard 4 hospitals and University hospital with 3 hospitals. The ratio
between patient and dialysis outlets are 2.7 pt. /outlet. (See table 4.9.).Regional distribution of MOH hospital has shown that southern region
has the most patient with 1,788 patients, followed by central region with 1,601, western 1,521, Northern 1050 and Eastern with 805. With the
Non-MOH hospitals, the outsourcing hemodialysis (Davita & Diaverum) has the most patients with 3,318 HD patients, followed private and
charitable hospitals institutions with 2,758 patients, Military hospital 1359, King Abdullah Project 1118, National guard 439, university hospitals
223, specialist hospitals 159 and others with176. The ratio between patients/outlets in both MOH and Non-MOH hospitals are 2.7 and 2.4%
respectively (See fig. 4.10. and 4.11.).

Peritoneal Dialysis in the Kingdom of Saudi Arabia
Peritoneal dialysis
In 2016, a total of 35 active peritoneal dialysis centers were recorded from MOH, Gov’t. Non-MOH, Private outsourcing dialysis
project (Diaverum). Among these PD centers, 18 centers were from MOH, 15 from Gov’t Non-MOH, 1 from Private and 1 from
Diaverum. Types of PD modalities per patients were Automated/Continuous Cyclic Peritoneal Dialysis or (CCPD/APD) machines
with 73% of patients, 21% were on Continuous Ambulating Peritoneal Dialysis (CAPD) and 6% were on Intermittent Peritoneal
Dialysis (IPD).
By the end of 2016, a total of 1,372 patients on peritoneal dialysis were recorded by SCOT, of these 461 were new patients. Age
Distribution of patients with ESRD has shown that 86% were adults and 14% were children.
Hepatitis B and C infection has shown that 1% of the ESRD patients are found to be HBsAg positive and 2% were HCV antibody
positive a 1% decreased from each compared to previous year.

Active PD centers and treatment modality
In 2016, a total of 1,372 active PD patients in the kingdom had been
recorded. A 5% increase from the previous year (see figure 5.1.1.).
The modality of treatment had shown that CCPD/APD are still the
most chosen PD modality among the patients with 999 (73%)
followed by CAPD with 291 (21%) and IPD with 82 (6 %) (See figures
5.1.2. and 5.1.3.).

Adult and pediatric PD patients
Among the 1,372 PD patients, 1184 (86%) were composed of adult
patients and the remaining 188 (14%) were pediatric (see figure
5.2.1.). Comparing to last year’s data the number of PD patients had
slightly increased by 5% in 2016 but still lower than the past 5 years.
(See figures 5.2.2.).

Serology status of HBsAg and HCV among PDpatients
In 2016, HBsAg+veand HCV+vestatus among PD patients were
reported in SCOT which shows HBsAg+ve infected patients 45,
remains at (3%) (See figures 5.3.1.)Though, results have only shown
varying changes in the past few years (See figure 5.3.2.). While,
HCV+ve patients with 64 patients has increased in percentage to
(5%) from last year’s 3% infected patients.(See figures 5.3.3. &
5.3.4.).

PD patients distribution in the region of the kingdom were highest
in central region with a total of 596 or (44%), followed by Eastern
region with 292 (21.3%), western 283 (20.7%), southern 166 (12%)
and northern region with 30 (2%).

Renal Replacement Therapy
In 2016, Patients on renal replacement therapy were composed of
HD patients with 16,315 (66%), then patients who had kidney
transplants with 7,015 (28%) and patients on PD with 1,372 (6%) of
the total population (see figure 6.1.). The current and projected
dialysis population had an average of 658patients per year which is
having a net annual increase of 7.7% annually.
