Positioning Greater Manchester

as an internationally renowned

location for life science, med-tech

and digital healthcare research and

innovation.

WearedelightedtobringyouthefirstannualreportfromHealthInnovationManchester.

2017/2018hasbeenayearofmajortransitionaswebroughtManchesterAcademicHealthScienceCentre(MAHSC)andGreaterManchesterAcademicHealthScienceNetwork(GMAHSN)underthesingleumbrellaofHealthInnovationManchesterinOctober2017.

Despitethechallengesofamajortransition,wehavecontinuedtosuccessfullymaintaintractiononallexistingprogrammes.ThishasbeeninparalleltosecuringanewAcademicHealthScienceNetworklicenceagreementfromNHSEngland,andpreparingforthere-accreditationasanAcademicHealthScienceCentre

Inadditionto‘businessasusual’wehavealsomadesignificantinroadsincreatingkeyinfrastructure,governanceanddecision-makingmechanismscriticaltoinjectinginnovationintothehealthandsocialcaresystem.

ThishasincludedtheformationofthenewHealthInnovationManchesterBoardwhichdrawsonexpertisefromacrosstheNHS,socialcare,industryandacademia;worktoestablishtheInnovation,MonitoringandPrioritisationCommittee(IMPC),whichwillprioritiseandoverseetheinnovationandimprovementworkprogramme,thedevelopmentoftheinnovationpathwaytosimplifythelandscapeforindustryinnovatoions,andpavedthewayfordigitalinteroperabilityandinnovationhubsthatwillimprovethecareweprovidetopatientsnowandinthefuture.

2017/2018hasalsobeenoneofnewcollaborations.In2017wesignedamemorandumofunderstandingwiththeAssociationofBritish

PharmaceuticalIndustries(ABPI)-thefirstofitskindintheUK.Thisisanew,yetalreadyblossomingrelationshipwhichhasledtosometrulyinnovativeprojectsaroundtheeliminationofHepatitisC,pricingmodelsformentalhealthmedication,andimprovingthehealthofCOPDpatients.Someoftheseareoutlinedinourdeliveryprogrammesnapshotonpage16.

WealsohadthepleasureofaddressingseniorrepresentativesfromthepharmaceuticalindustryduringtheABPI’sAnnualConferencewhichhasalreadyyieldedsomeveryinterestingpotentialprojects.

InadditiontotheABPI,wehavealsostartedtheprocessofagreeingasimilarmemorandumofUnderstandingwiththeAssociationofBritishHealthcareIndustries(ABHI).Weexpectthesigningformalitiestobecompletedinthe2018/2019financialyear.

Asillustratedthroughoutthisreport,HealthInnovationManchesterisdeliveringahugeportfolioofprogrammesandprojectsthatwillhave,andarealreadyhavingaprofoundeffect,notjustonthehealthandwellbeingofour2.8mcitizens,butalsonationallythroughourworkwiththeAHSNNetwork(seepage7).

Finally,aswemoveinto2018/2019welookforwardtoforgingnewpartnershipsandcollaborationswithsystemstakeholdersandindustryinnovatorswhiledeliveringadiverserangeofexcitingprogrammesandembeddingthemechanisms,infrastructure,andcultureacrossthesystemtocreateanenvironmentwhereinnovationflourishes-enablingGreaterManchestertobecomeaninternationalleaderinacceleratinginnovationthattransformscitizens’healthandwellbeing.

ABOUTHEALTHINNOVATIONMANCHESTERAsanacademichealthscienceandinnovationsystem,HealthInnovationManchesterisattheforefrontoftransformingthehealthandwellbeingofGreaterManchester’s2.8millioncitizens.

ItwasformedinOctober2017bybringingtogethertheformerAcademicHealthScienceNetworkandAcademicHealthScienceCentreunderonesingleumbrella,whichalsorepresentsGreaterManchester’swiderresearchandinnovationsystem.

HereinGreaterManchester,wehavetheuniqueabilitytodeliverinnovationintofrontlinecareatpaceandscalethankstoour£6bndevolvedhealthandsocialcaresystem,unrivalleddigitalassetsandambitions,exceptionalacademicandresearchcapabilityandthrivingindustrypartnerships.

OurcollectiveambitionistomakeGreaterManchesteroneofthebestplacesintheworldtogrowup,getonandgrowold.

Despitehavingoneofthefastestgrowingeconomiesinthecountry,peopleheredieyoungerthanthoseinotherpartsofEngland.Cardiovascularandrespiratoryillnessesmeanpeoplebecomeillatayoungerageandlivewiththeirillnesslongerthaninotherpartsofthecountry.Ourgrowingnumberofolderpeopleoftenhavemanylong-termhealthissuestomanage.

Therefore,HealthInnovationManchesterhasapivotalroleinbringingforwardaconstantflowoftargetedinnovationsandputtingthemthroughaneffectivebutstreamlinedevaluationprocesssotheyareadoptedatpaceandscaleacrossour10localities.

Wedothisbyharnessingtheassetsofourworld-leadingresearchers,fosteringpartnershipswithindustryandsupportingourNHSandsocialcareproviderstoadoptinnovation,suchasthroughdigitaltechnology,changestopracticeoroptimisingmedication.

In2017/18ourthreebusinessobjectiveswere:

ThesuccessofHealthInnovationManchesterisintertwinedwiththesuccessofourpartners,soweworkasonesystem.Thisincludescommissioners,providers,Universities,theBiomedicalResearchCentre,CollaborationforLeadershipinAppliedHealthResearchandCare(CLAHRC),ClinicalResearchNetwork,industry,nationalbodiesandinvestmentagencies.

Asareflectionofthisdynamicsystem,theHealthInnovationManchesterBoardismadeupofleadersfromhealthandsocialcare,academia,researchandindustry.Thisensureswehavesystem-widebuyinofourstrategyandplans.YoucanreadmoreaboutwhotheBoardareonpage29.

TheBoardisunderpinnedbytwomaincommittees–theInnovationPrioritisationandMonitoringCommittee,aswellastheResearchandEducationCommittee.ThesecommitteesalsoincludenominatedrepresentativesfromacrossGreaterManchestertoensurewehavetheagreementandcommitmentfromall.Thissupportsquickerdecisionmakingandspeedsuptheinnovationpathway.

WereceivenationalfundingfromNHSEnglandaspartofourAcademicHealthScienceNetworklicenseandattractconsiderableinvestmentduetoourAcademicHealthScienceCentrestatus.WealsoreceivefundingfromourGreaterManchestersystempartners.Amoredetailedoverviewofourfinancescanbefoundonpage29.

InApril2018HealthInnovationManchesterbecamehostedbyManchesterUniversityNHSFoundationTrust,ensuringourworkisunderpinnedbytherobustgovernance,transparencyandstandardsexpectedofapublicbody.

DevelopedanenhancedClinicalTrialsUnit(CTU)tosupportthenextgenerationofclinicaltrials(includingrealworldtrials)thataremoreresponsivetotheneedsoftheprivatesector.

CreatedaHealthInnovationManchester-NationalInstituteforHealthandCareExcellence(NICE)DataLab.Linkingdifferentdata-setstogetherandusingstate-of-the-artanalytics,theDataLabistestinghowbigdatacanprovideevidencerelatingtotheeffectivenessofnewandexistingtreatments,andproducenewhealthinsights.

SecuredInnovateManchesterAdvancedTherapyCentreHub(MATCH)£9mawardfromInnovateUKtoestablishoneofthreeAdvancedTherapyTreatmentCentres.

Clinical research excellence

TheGreaterManchesterResearchHub,HealthInnovationManchestersupportsresearchgenerationanddeliverythroughconnectiontostateoftheartresearchinfrastructure,academicexpertise,aresearch-savvyworkforceandstreamlinedprocessesincludingcostingandcontracting.

OurclosecollaborationwithNorthWestEHealthalsoprovidesopportunitiestoempowerthelifescienceindustrytoruncosteffectiveclinicaltrialsfromfeasibilitystudiestofullclinicaltrialmanagementanddelivery.

Inaddition,HealthInnovationManchesterenjoysaproductiverelationshipwiththeGMLocalClinicalResearchNetwork(LCRN)whichisnumberonenationally(allowingforpopulationsize)forrecruitmenttolife-scienceindustryclinicaltrials.Consequently,theGMLCRNisapreferredproviderforanumberofpharmaceuticalcompaniesandContractResearchOrganisations(CROs).

Wearedevelopingapipelineofnewtreatmentsandpathwaysthatwilladdressthehealthneedsofourpopulationinyearstocome.

TRANSFORMING THE HEALTH AND

WELLBEING OF OUR 2.8M CITIZENS

In 2017 / 2018 we have:

WorkedcollaborativelywiththeAtrialFibrillation(AF)AHSNnetworktodelivernationalandlocalimprovementsaroundAFmanagementanddetection.Thishasalreadypreventedanestimated13strokesinGreaterManchesterandresultedin468patientsbeingofferedtreatmentwithspecialistpharmacists,andwherenecessaryassessmentinprimarycare.

DeliveryofthefirstAtrialFibrillationCCGpharmacy-ledanticoagulationclinicwhichidentified6,059patientsacrossthecityofManchesterwitha‘readcode’ofatrialfibrillation.Ofthese1,603patientswere‘knownnottreated’whichequatesto52expectedstrokes.Wearenowfundingawork-streamin2018/19totacklethis.

DeploymentofERAS+,apre-andpostoperativesurgeryprogrammetooptimisepatientrecovery.Implementationatonetrusthasreducedpost-surgerypulmonarycomplicationsby50%resultingina3dayreductioninlengthofstayandannualsavingsofover£0.5m.A£500kgrantawardhasbeensecuredfromtheHealthFoundationtotakethisworkforwardincludingtheroll-outacrosssixtrustsinGreaterManchester.

DeliveredacontroltrialofaTroponin-onlyManchesterAcuteCoronarySyndromes(T-MACS)decisionaidtoolthatwasdevelopedinGreaterManchester.ThishasalreadyresultedinabenefittopatientssuspectedofAcuteCoronarySyndromeandfinancialsavings.ThisisnowbeingdeployedacrossGreaterManchesterandthewiderUK.

DeliveredthefirstRainbowClinic-aspecialistserviceforwomenandtheirfamiliesinasubsequentpregnancyfollowingastillbirthorperinataldeath.Thishasalreadybenefited140families,prevented9like-for-likestillbirths(Contributed2.19%oftheavoidedstillbirthsnationwideduringoperation)andhasgeneratedan8foldreturnoninvestment.ThisisnowbeingdeployedacrossGreaterManchester.

IncorporatedPINCER:(pharmacist-ledinformationtechnologyinterventionformedicationerrors),anauditsoftwarepackagewhichhelpsGPpracticesreviewpatientcaseloadsandhighlightpatientswhomaybeatriskofprescribingerrorsintoGMPrimaryCareMedicalStandards.Thisisresultinginareductioninmedicationsafetyincidentsandreducingunwarrantedvariation-improvinghealthoutcomesforourpatientpopulation.

IMPROVING CARE FOR PEOPLE WITH COPD

HealthInnovationManchesterisworkingwithhealthandindustrypartnerstodevelopandimplementanambitiousplantoradicallyimprovethecareandtreatmentofpeoplewithCOPDacrossthe10localitiesofGreaterManchester.

COPDisthefifthbiggestkillerintheUK.PatientswithCOPDsufferwithseverebreathingdifficulties,oftenresultinginhospitalisation.Therearearound67,000patientsdiagnosedwithCOPDlivinginGreaterManchesterandtherearelikelytobehundredsmoreunknowinglystrugglingwiththedisease.

ThecostofmanagingCOPD-relatedhospitaladmissionsandmedicationsforGreaterManchesterisaround£73millioneachyear,withfurtherindirectcostsestimatedtobeasmuchas£1.8

billion.Infact,thecity-regionhasthesecondhighestadmissionratesforCOPDinEngland,aswellaspooroutcomesandunwantedvariationbetweenGPPracticesandlocalities.

Theprogrammebuildsontheground-breakingSalfordLungStudy,whichexaminedthesafetyandefficacyofnewmedicationstotreatrespiratoryillness.

Thiswastheworld’sfirstdigitallyenhancedrandomisedreal-worldtrialtoincludeabroadandinclusivepopulationofpatientsinaneverydayclinicalpracticesetting-madepossiblebythesingleelectronicmedicalrecord.

TheaimoftheCOPDprogrammeistoimprovethehealthofCOPDpatientsthroughclinicallyappropriateandcost-

effectivetreatmentusinginhalers.Thiswillbeachievedthrough:

Reviewingpatients’existingtripletherapyprescribingtoidentifythosewhocansafely‘stepdown’fromthecorticosteroidelementoftheirmedication,

ReviewingpatientsreceivingmedicationwhichisnotinlinewiththelatestGreaterManchesterMedicinesManagementGroup(GMMMG)guidelinestoenablepatientstoswitchtorecommendedalternativeswhereclinicallysafetodoso.

ELIMINATION OF HEPATITIS C

GreaterManchesterhastheambitiousaimtobecomethefirstUKcityregiontoeliminateHepatitisCby2021.

Thevirus,whichaffectstheliver,cansometimescauseseriousandpotentiallylife-threateningdamageifleftuntreated.

Thereareestimatedtobearound17,450peopleinGreaterManchesterlivingwiththeinfection,includingaround7,000whoareundiagnosed.Ofthosediagnosed,only28%areengagedwithspecialistservices.

TheHealthInnovationManchester

projectaimstoeliminateHepatitisCbyusinganetworkedandphasedapproach.

Communitypharmacieswilldeliverpointofcaretestinganddrybloodspottestingtomaximisethenumberofpeopletestedandidentifyhighriskpatientsaswellasprovidingtreatmentinamoreconvenientlocationfortheindividual.

Pharmacieswillbetargetedintermsofmethadonedispensingandopiatereplacementtherapyaswellasthoseoperatingasafeneedleexchange.

Followinganinitialpilot,theprojectwilllooktotestandtreatthewiderpopulationgroupsathighriskofinfectionandarapidtestingandtreatmentregimewillalsobeimplementedforthoseinorenteringprison.

Theprojectaimstobringspecialistservicestothepatientatthepointofneedanddevelopanew,morecost-effectivetestingandtreatmentinfrastructure.Itwouldalsoresultinareductioninassociatedhealthcarecostsandabetterqualityoflifeforpatients.

ERAS+ ENHANCED RECOVERY AFTER SURGERY

PatientsundergoingsurgeryatsixGreaterManchesterhospitalswillbepreparedfortheexperienceinthebestpossiblewayusingtheEnhancedRecoveryAfterSurgery(ERAS+)programme.

Thesurgicalpathwaybuildsonthesuccessofthein-hospitalprogrammebutexpandsittoincludesixweeksofpre-surgerypatientpreparationandpost-hospitalrecoverysixweeksafter,withpatientsandtheirfamilysupportedthroughaSurgerySchool.

Therearearound250,000high-riskelectivemajorsurgeriesayearinEnglandandWalesandthereisapost-operative

pulmonarycomplicationrisk(PPC)ofupto30%.Complications,suchrespiratoryfailureorpneumonia,canincreasethelengthofstayinhospitalandreducelifeexpectancyaftersurgery.

TheERAS+programmeplacesthepatientatthecentreoftheirownrecoveryandsupportsthemtobedynamicintheirowncare.Itencouragesincreasedactivity,betternutrition,oralhealthcareandthepracticeofchestexercisestohelpreducechestproblems.

Itaimstoreducecomplicationspost-surgery,withevidencesuggestingareductioninpulmonarycomplications

by50%,reducethelengthofstayinhospitalbyaroundthreedaysandimprovequalityoflifeforpatientsforsixto12monthsaftermajorsurgery.

TrustsparticipatinginERAS+areManchesterUniversityNHSFoundationTrust,StockportNHSFoundationTrust,BoltonNHSFoundationTrust,ThePennineAcuteHospitalsNHSTrust,SalfordRoyalNHSFoundationTrustandTheChristieNHSFoundationTrust.

HEALTHY HEARTS: SYSTEMATIC IDENTIFICATION OF HIGH-RISK CVD PATIENTS

HealthyHeartsisaninnovativeprogrammeaimingtoreducedeathsfromCardiovascularDisease(CVD)throughbetteridentificationofthoseatriskofheartattackorstrokeinGreaterManchester.

TheprojectwillworkcloselywithClinicalCommissioningGroupstofindthoseathighestriskofCVD,includingthosewithhighbloodpressureandhighcholesterolinasystematicandtargetedapproach.Theprogrammewillalsolinkwithtargetedhealthchecksandlifestyleinterventions.DatashowsthatifeachCCGintheareareachedthelevelofthe

fivebestsimilarCCGs,therewouldbe356fewerdeathsforunder75-year-oldseachyear.

Itisestimatedthatidealcontrolofdiagnosedhypertensives–thosepatientswithhighbloodpressure–couldsave470heartattacksand700strokesoverthreeyearsinGreaterManchester,afinancialsavingofover£13.2million.

TheprojectaimstoreducedeathsfromCVDbyatleast600by2021andidentifythoseathighestriskofCVDtoenabletreatment,lifestyleinterventionsand

self-managementopportunities.HealthInnovationManchesterhasalsoprovidedpracticesinGreaterManchesterwithaccesstotheAliveCorKardiahand-heldAtrialFibrillation(AF)detectiondevicetoimprovecareforpeoplewithAFandreduceAF-relatedstrokes.

Safetyandensuringpatientsarenotharmedwithinahealthandsocialcaresettingiseveryone’sresponsibilityandGreaterManchesterisinauniquepositiontobeabletosupportandcreateasmoothpathwaytoimprovement.

ThroughthePatientSafetyCollaborative,ajointinitiativefundedandcoordinatedbyNHSImprovementanddeliveredbyHealthinnovationManchester,wecanworkdirectlywithlocalteams,supportingthemtomakesuretheyhavetherightskillsandresourcestoimplementimprovements.

Wecanalsosharegoodpracticeacrossthehealthsystem,focusonpeople-centredcareandbuildrelationshipswith

NHSstaff,businessandacademiatostimulateinnovationandimprovement.ThePSCwillfocusonthreenationalareasofwork:

DeterioratingPatients:Toreduceharmandenhancetheoutcomesandexperienceofpatientswhoaredeteriorating.

CultureandLeadership:Tohelpcreatetheconditionsthatwillenablehealthcareorganisationstonurtureanddevelopacultureofsafety.

MaternityandNeonatal:Toimprovematernityandneonatalcare,specificallyreducingtherateofstillbirth,neonataldeathandbraininjuriesoccurringduringorsoonafterbirthby20%by2020.

ThePSChasbeensetuptosupport,connectandprovideinitiativesandactivitiestodriveimprovementandensurepatientsafetyisembeddedthroughouttheGreaterManchesterhealthandsocialcarecommunity.

RAINBOW CLINIC: SPECIALIST ANTENATAL SERVICE

Becomingpregnantagainafterastillbirthisanincrediblydauntingprospectforwomenandfamilies,oftencharacterisedbythefearofrepeatingtheexperience.

Womenwhohavehadastillbirthareatincreasedriskofcomplicationsinsubsequentpregnancies,includingstillbirth,pre-eclampsia,placentalabruptionandlowbirthweight.

Itisalsoassociatedwithincreasedpsychological,emotionalandsocialchallenges.

TheRainbowClinic,atSaintMary’sHospital,isaspecialistservicefor

womenandtheirfamiliesduringasubsequentpregnancyfollowingastillbirthorperinataldeath.

Itcaresforfamiliesfromthetimeofthepostnatalappointmentonwardsandintoasubsequentpregnancy.

Itengageswithwomenearly,ensuringtheyareontherighttreatment,makinganynecessaryreferralsandprovidingmoredetailedultrasoundscanning.

Theadditionaltestsandcontinuityofcare,providedbyasmallteamofspecialistdoctorsandmidwives,resultinimprovedoutcomesforthebabyaswell

asimprovedpsychologicalwell-beingforparentandbetteruseofNHSresources.

HealthInnovationManchesterwillsupportthescale-upacrossGreaterManchestermaternityunits,aimingtoreducetheratesofmaternaldeaths,stillbirths,neonataldeathsandbraininjuriesthatoccurduringorsoonafterbirthby20%by2020andthenby50%by2030.

ThecharityTommy’sisalsogivingadditionalfinancialsupportasvalidationoftheclinicalprogresstoensurescaleandspreadoftheproject.

REDUCTION OF FRAILTY-RELATED FALLS AND FRACTURES

Forolderpeoplelivingwithfrailty,afallorfracturecanresultinarapiddeteriorationinhealthandsignificantlossofindependence.

Thereareapproximately65,000hipfracturestakingplaceintheUKeachyear,costingthehealthandsocialcaresystemaround£2billion.Itisimperativethatsystemsareinplacetohelpidentifypeoplewhoareatriskoffallsandfracturestoensurethereareappropriatetreatmentandservicesthatmeettheirneeds.

TheHealthInnovationManchesterprojectaimstoreducefallsandfracturesrelatedtofrailtyacrossGreaterManchesterthroughearlyintervention,treatmentandmanagementofat-riskpatients.

Theprojectwillsupportprimarycarebyfindingat-riskpatientssothatthey

canbetreatedwithappropriatebone-sparingtherapies,includingmedicationtostrengthenbones,andsupportedwithservicesinthecommunitytohelpmanagetheircondition.

Throughearlyandincreasedidentificationofosteoporosisandotherhigh-riskfactors,theestablisheduseoffractureriskassessmenttoolsinprimarycareaswellaspatientbehaviouralandlifestylechangesthrougheducationprogrammes,itishopetherewillbeareductioninfracturesandassociatedcosts.

PhaseoneisbeingundertakeninTamesideandGlossopLocalitywiththeDukinfield,StalybridgeandMossleyFrailtyMultidisciplinaryTeam(MDT)pilotlaunchedon31January2018.

Ithasbeensetupasavehicletosupporttheidentification,reviewand

managementofseverelyfrailpatientsusingtheelectronicFrailtyIndexandfractureandfallsriskassessmentITtools.

OnceapatientisidentifiedasbeingseverelyfrailtheMDTmembersdiscussandagreeonasetofappropriateoutcomesandactionstosuittheneedsofthepatientwhichcanincludeseveralclinicalandnon-clinicalinterventions.

Thismayincludereferralstocommunitygroups,physiotherapy,CommunityResponseService,Extensivistservice,socialprescribing,prescriptionofbonestrengtheningmedication,etc.

EightweeksaftertheMDTmeetingafollowupiscarriedouttoensurethatallagreedoutcomesandactionshavebeenachieved.

MENTAL HEALTH PRICING REVIEW MODEL

HealthInnovationManchesteraimstodemonstrateinnovationinthefieldofmentalhealththroughthetestingofanoutcomesbasedpricingmodelforschizophreniatreatments.

Second-generationantipsychotics(SGAs)representanadvanceinthelong-termmanagementofschizophrenia,particularlyregardingextrapyramidalmotorsafetyandsubjectivetolerability.

However,theevidencethatthemedicationsareeffectiveformaintenanceandstabilityislimited.

Asarelapseinschizophreniacancostbetween£12,000and£25,000,industry

partnerJanssenhavedevelopedanoutcomespaymentschemeandrebatetodemonstratetheirbeliefthattheirmedicinescanplayaroleinpreventingrelapseandthekeycostthatgoeswithit.

WorkinginpartnershipwithHealthInnovationManchester,JanssenofferedMentalHealthTrustsinGreaterManchesteraccesstotherebateschemewhichreimbursestheproviderifthetreatmentdoesn’tworkasplanned.

Theschemeproposesarefundforthecostoftheinjectionshouldtheperson,prescribedtheinjection,failtoachievetherequiredoutcome.

The scheme has provided real-world data as to whether the second-generation antipsychotic injections prevent relapse and admission as well as potential financial savings of £10,000 against medication costs for 2017, with the scheme in place for two trusts until June 2021.

The anticipation is that the tracking of the patient journey and understanding discontinuation and relapse will lead to better care with improved outcomes.

Itisestimatedthattherearecurrentlyover30,000peoplelivingwithdementiainGreaterManchesterwiththecityregionspending£270millionayeartreatingandcaringforpeoplewithdementia.

Withanageingpopulationandrisingnumbersofpeoplebeingdiagnosedwithdementia,itisoneofthegreatesthealthchallengesfacingthecountryandGreaterManchester.

TheDementiaUnitedOperationalGroupareworkingwithpartnersacrossGreaterManchestertoimprovetheexperiencesofpeoplewithdementiaandtheircarers,andreducepressureonthehealthandsocialcaresystem.

HealthInnovationManchesterwillsupportDementiaUnitedthroughtheidentificationandimplementationofsuitablenewinnovationstosupporttheoverallobjectives.

Thevisionistoimproveoutcomesbyensuringoptimalaccessanduptakeofinnovativetechnologiesandtreatmentsforeligiblepeople,aswellasensuringthehealthandcaresystemisgearedtoprovidethebestsupportforpeoplelivingwithdementia.

T-MACS: TROPONIN ONLY MANCHESTER ACUTE CORONARY SYNDROMES DECISION AID

Patientspresentingwithchestpainattheemergencydepartmentarethegroupmostcommonlyrequiringhospitaladmission.Troponinsareafamilyofproteinsfoundinheartmusclesthatproduceamusclecontractionwithserialtroponintestingremainingthestandardofcaretoruleoutheartproblems.

TroponinonlyManchesterAcuteCoronarySyndromes(T-MACS)isadecision-aid,acomputerisedclinicalpredictionmodelwhichcalculateseachindividualpatient’sprobabilityofacutecoronarysyndromesfollowingasinglebloodtestatthetimeofarrival.Thisprobabilityisusedtoassigneachpatient

totherelevantriskgroupandsuggestacourseofactionforthecliniciantofollow.

SinceimplementationatManchesterRoyalInfirmary,over3,500patientshavebeentreatedusingT-MACS,withthealgorithmprovingsuperiortoNICEguidelines.

Morethantwo-thirdofpatientscanbetreatedinanambulatorycaresetting,suchasoutpatientclinicsoremergencydepartments,withthevastmajoritydischargedonthesameday,comparedtoatwo-dayaveragestaywithroutinecare.

T-MACSwonManchesterFoundationTrust’sTransformationPrizein2016andHealthInnovationManchesteraimtoimplementitacrossGreaterManchestertoincreasethequalityandefficiencyofhealthcareprovidedtopatient.

Theprojectcouldresultinimprovedqualityoflifeforpatients,duetoquickerandmoreeffectivediagnosisandtreatment,moreappropriatetriagingofpatientsandisprojectedtosave£100millionperyearifrolledoutacrosstheNHS.

QUALITY IMPROVEMENT - LIFE QI AND THE Q COMMUNITY

HealthInnovationManchestersupportsasystem-wideapproachtodrivingqualityandimprovementandoffersafreetooltohealthandsocialcareorganisationsacrossGreaterManchester.

LifeQIisaweb-basedplatformdesignedtoassistfrontlinestaffrunningQualityandSafetyimprovementprojectsandconnectwithacommunityacrossthecountrytosharebestpractice.

Thetool,developedaspartofthePatientSafetyCollaborativeinpartnershipwithSeeData,supportsfrontlineNHSandsocialcareteamstoplan,monitorandreportprogressoftheirimprovementprojects.

TheflexibleLifeQIapplicationcontainstoolstohelpimprovementworkandmakeiteasytoseeprogress.Itallows

teamstocreatedriverdiagrams,conduct“Plan,Do,Study,Act”(PSDA)cyclesandvisualiseresultsthroughcharts,aswellascreatingabankofQIprojectswhichcanbeshared.

UserscanalsoconnecttoaQIcommunityofpracticeacrossthecountry,encouragingcollaborationwithteamsworkingonsimilarchallenges,avoidingduplicationandsharinglearningandsuccesswhilebuildinganetworkofpeoplecommittedtoimprovingcare.

Morethan140peopleandorganisationsworkingonserviceimprovementprojectshavealreadysigneduptotheLifeQItoolthroughHealthInnovationManchester.

HealthInnovationManchesterhasalsopartneredwithAdvancingQuality

TheQcommunityisaninitiativeconnectingpeoplewhohavehealthandcareimprovementexpertiseacrosstheUKwithopportunitiestoshareideas,enhanceskillsandcollaborate.

TheQCommunityBookClubwillfeaturefivesessionswhichwillexaminekeyqualityimprovementliterature,hearfromlocalandnationalimprovementleadersandexploreonlinetoolstohelpQcommunitymemberssharediscussionandapplylearning.

GREATER MANCHESTER BIOLOGICS OPTIMISATION - VIRTUAL BIOLOGICS CLINIC

HealthInnovationManchesterissupportingasystem-wideapproachtoreducingunnecessarywastageofhigh-costbiologicdrugsacrossGreaterManchester.

Currentlythereisasubstantialspendingonapprovedbiologics,aclassofmedicationthatisnotsynthesizedchemicallybutinsteadharvesteddirectlyfrombiologysuchasenzymesorantibodies,withanestimated£45-50millionspentacrossGreaterManchester.

Alongsidethishighspend,thereareloosegovernancearrangementsfortheiruse,withpoormechanismsinplaceto

adequatelytrackandrecordusage.AVirtualBiologicsClinic(VBC)wascreatedatManchesterRoyalInfirmaryinordertoensurethattheprescriptionofbiologictherapieswasbasedprimarilyontheneedsofpatientsandnotoncostandtoincreasethespeedatwhichpatientscouldgainaccesstothetreatmentandprovidemoreopportunitiesforpatientstoparticipateinclinicalresearch.

TheVBCeffectivelyimplementedapan-ManchesterbiologicspathwayinRheumatologyatMRIwiththeinnovationschangingthedepartment’sprescribingpatternsandincreasing

researchrecruitment.Italsoincreaseddepartmentalefficienciesandgeneratedcostsavingsthroughhigheruptakeofcheaperbiologicstherapies.

DevelopingaVBCandbiologicoptimisationinitiativesinallGreaterManchesterTrustswouldcreateaneffectivewayofdeliveringbiologicsforpatientsinthemostcost-effectiveway.

articulateshowtheGreaterManchestersystemwillwork

collaborativelywithpharmaceuticalcompanies.Despite

itsinfancythishasalreadyledtosomeveryinnovative

CentraltothedeliveryoftheInnovationPathwayhasbeenthedeliveryandon-goingdevelopmentofHealthInnovationManchester’sInnovationNexuswebportalandadvisoryservice,thewinneroftheNorthofEnglandBusinessServiceawardsin2016.

TheInnovationNexusenablescompaniestolodgeenquirieswithanintelligentinterfaceandreceiveeithersignpostingand/orguidance,orwhenappropriate,follow-onsupportservicestoassistwithprogressionthroughthedevelopmentpathway,fromearlystageclinicalengagement,toassistancewithadoptionandprocurementintotheNHS.

BioinformaticsandGenomicsthatrangefromaccreditedshort

coursestoMSc/PhDlevels.Accreditedcomponentsofthe

aboveprogrammescoupledwithspecialtyspecificmodules

canbecombinedintoflexiblepostgraduateprogrammesfor

clinicaltrainees.

DeliveredanewcollaborationbetweenwithTheRoyalCollegeofPhysiciansofEdinburgh(theRCPE)throughtheopeningofanofficeinManchester(RCPEManchester)inJune2017.TheRCPEiscommittedtopromotingthehighestclinicalstandardsandtheimplementationofrobust,evidence-basedmedicalpractice.TheRCPEhashostedanewseriesof‘hottopic’eventsandinternationalconsensusconferences,developingblendedlearningopportunitiesandco-badgedpostgraduatequalificationsandtherebyincreasinglocaltrainingandCPDopportunities.TheRCPE’sexistingnetworkofnationalandinternationalcentreswillbeusedtolive-streameducationalandtrainingeventsaroundtheworld.

DeliveredaOneManchesterTranslationalResearchTrainingDaywhichwasattendedbyover100PrincipalInvestigators,ResearchNursesandResearchProgrammeManagers(RPMs)fromacrossGreaterManchester.

DesignedanInnovativeDevelopmentalFrameworkthathasbeenpilotedbyourResearchProgrammeManagerNetwork(over130members)whichspecifiescompetencystandardsforeachleveloftheirappointment.

DeliveredaClinicalBioinformaticsMassiveOnlineOpenCourse(MOOC)designedtoprovideskillstohealthcareprofessionalstoenabletheanalysisandinterpretationofgenomicdata.Thishasattractedover13,000participantstodatefromacrosstheworld.

GreaterManchesterisacityunitedthroughpartnershipsestablishedforthebenefitofourpopulationandourlocaleconomy.

Asaregionofhighdiseaseburdenandhealthinequality,itisessentialthatourcitizensreceivethehighestqualityhealthandsocialcareprovision,deliveredbyahighlytrainedworkforceofhealthandsocialcareprofessionals.

ThroughtheopportunitiesprovidedbyGM’shealthandsocialcaredevolutionandthecreationofHealthInnovationManchester,wenowhaveameansofconnectinghealth,socialcare,highereducationinsight,andindustryorganisationswiththehealthandsocialcareprioritiesofourpopulation.

ThroughpartnershipworkingwithourlocalHEIsweplantodevelopGreaterManchester’sfirst‘Ed-City’.

Thisinitiativewillprovideaneducationandtrainingecosystemthatbringstogetherexpertiseineducation,bestpracticeinitiativesfromtheGreaterManchesterHealthandSocialCarePartnership,andlearninginfrastructuretoenhancetheeducationandtrainingneedsofournursingandalliedhealthprofessionalworkforce.

AddingvaluetothehealthandsocialcareprioritiesoftheGreaterManchesterpopulation,Ed-Citywillbuildbespokeprogrammesspecificallydesignedanddeliveredtomeettheevolvingneedsofournursingandalliedhealthprofessionalworkforce.

Insodoing,wewillcreatealearningenvironmentthatnotonlyattractsindividualstoourlocalUniversitiesviaauniqueeconomyofscalebutalsoprovideslife-longprofessionallearningandresearchopportunitiestoensuretheseindividualsareattheforefrontofthediscovery-carecontinuumwellintothenextmillennium.
