Dear Colleagues,

The year 2015 was filled with both achievements and challenges as we focused on fulfilling our mission of ‘Providing Milwaukee County residents with the resources to affordable healthcare.’ As part of our commitment to our community, we strive to manage our agency in a way that demonstrates the value we place on sound fiscal management, professional development and continuous improvement.

Our mission is to help those in need to find their way to a better quality of life and greater self-reliance. We do this through direct service, through collaborations that expand and strengthen our outreach, and through perseverance in addressing the needs of the community we serve. As implementation of our strategic plan moves forward, we pledge to uphold the agency’s standard of excellence, in our quest to provide the very best service available to those in need.

Poverty held a persistent grip on Milwaukee since 2014, according to estimates released by the U.S. Census Bureau. The city’s poverty rate of 29% — unchanged from a year earlier — was nearly double the national rate of 14.8%, leaving Milwaukee as the nation’s 2nd most impoverished big city. We responded to the shifting needs of Milwaukee County residents by implementing new programs and services often in partnership with other organizations. Moreover, we are committed to providing high quality services that make accessing health care easier and affordable.

As Leaders of Outreach Community Health Centers (OCHC), we are proud of the many benefits and services we offer to the Milwaukee community. OCHC’s capable and compassionate management and staff are committed to providing preventive and educational programs intended to improve the health of our community.

Our goals for 2016 will be just as focused and aggressive. We will continue to improve our healthcare services, enhance our homeless and supportive care programs, and continue our business development initiatives through additional collaborations and new partnerships.

Meera MacDonald, MALinkages to Care Coordinator

Community Services

I met Marcus* after he was informed by a blood bank that he could no longer donate because he was HIV positive. They didn’t give him any resources for treatment so he sought out the OCHC Outreach Team on his own so that he could get some answers.

Once Marcus was referred to me, I was able to connect him to care where he has reached undetectable levels of HIV (his treatment goal), and his immune system has doubled in strength. Marcus only works sporadically and has recently become homeless. I was able to get him into a rental assistance program and he now has moved into his own place and continues to follow his treatment guidelines.

Clients like Marcus make my job gratifying and rewarding!

Behavioral Health

Tom*, who had been homeless for many years, became involved with Outreach Community Health Centers (OCHC) through a Milwaukee County Access Clinic referral for psychiatric services. He attended OCHC therapy our other support services including MyWish, Path, SOAR and Primary Care.

Tom was eventually able to get into transitional housing, while being treated for mental and physical health issues. As he improved, though he was able to attend MATC, it was clear that Tom’s mental health challenges prevented him from maintaining steady employment. With the help of the SOAR program, he was granted SSI disability.

In May 2014, Tom graduated with a degree in Carpentry from MATC and eventually transitioned into a room and boarding house unit where his carpentry skills were quickly recognized.

Tom continues to attend weekly therapy and continues to make positiveimprovements.

In February 2015, I met Jose* at the Guest House. He had no place to live and no hope for the future. He had been a construction worker for over 30 years. The day I met him, he was diagnosed with throat cancer,hernia and depression. He hadn’t been to the doctor in over 50 years.

While he was receiving chemotherapy, I met with him weekly at the Guest House. I watched him go from a man full of life to someone without any energy at all. In June 2015, he was placed in a Home Linc in Cudahy where I continued to visit with him.

After moving, he developed a passion for cooking and decided to take culinary classes for fun.

Finally I got him approved for SSDI in August. He now has an apartment, stable income and a renewed sense of purpose.His cancer has been in remission since the summer!* not real names

Our mission and programs grow broader and stronger each year thanks to support from a broad array of Community Partners. Their commitment to efforts like ours reflects their generosity and determination to build a Milwaukee

community that provides a high quality of life for ALL people. For 2015 and 2016, these partners are lending their support to a number of different Outreach programs, and we want to thank them one and all for helping

Outreach Community Health Centers serve almost 11,000 people in 2015!

AURORA HEALTH CARE, through its Better Together Fund, provided $467,838 so we could add a Patient Navigator to our Behavioral Health team and make some renovations to our Adult Primary Care Clinic that will help us better serve our patients.

A MILWAUKEE HEALTH CARE PARTNERSHIP grant of $247,000 enabled us to hire a new psychiatrist who specializes in the treatment of opioid addiction to our service team, as well as a Psychiatric Nurse to increase capacity in our Behavioral Health Clinic.

DIRECT RELIEF, an international humanitarian aid organization, provided Outreach with over $170,000 in medical supplies and equipment for use in our Adult and Family Primary Health Clinics.

Those clinic services also benefited from the generosity of the EVAN & MARION HELFAER FOUNDATION, who contributed $1,500 toward our delivery of healthcare services to the homeless population.

2015 also marked the beginning of a new partnership with OUTPOST NATURAL FOODS. In addition to contributing over $3,000 toward our programs, Outpost has invited us to be on-site in stores throughout the month of January to promote the benefits of Outreach services. The FOREST COUNTY POTAWATOMI FOUNDATION extended its generosity to help homeless people who suffer from chronic mental illness gain access to safe housing through a $10,000 grant to our PATH program. A $2,500 gift from the BREWERS COMMUNITY FOUNDATION in support of our Street Outreach program complimented that effort.
