Carers Trust 4all is a local charity providing high quality support services for carers and people with care needs. We are a network partner of Carers Trust, Britain’s largest car-ers charity which champions carers issues at a national lev-

el.

Our aim is to relieve the stresses experienced by unpaid car-ers whose caring responsibilities may be due to a loved

one's disability, illness or age.

The definition of a carer: If you care for a child or adult with a physical disability, a mental health problem, a learning disability or someone who is elderly, frail or confused, you

We are delighted to report on an expansive year of change and due to popular demand we have once again produced our

Annual Review in the format of a desk calendar.

On 1 April 2013, the merger with Liverpool Crossroads was completed. Following a period of Corporate Trusteeship, a

merger with Crossroads Care Cheshire West, Wirral and Shropshire followed on 1 October 2013 extending our area of

benefit to Cheshire East, Cheshire West & Chester, Wirral, Shropshire, Liverpool, Sefton, Warrington, Knowsley, Manchester,

Tameside and surrounding areas. With effect from 1 April 2014 a further merger with Bury Crossroads was completed

extending our area still further into Bury.

Whilst our legal name remains Crossroads Care Cheshire, Manchester & Merseyside Limited, we have changed our trading

name to Carers Trust 4all with effect from 1 April 2014. We feel that this more accurately reflects the extent and diversity of

the organisation and our identity as a Crossroads Care Network Partner of Carers Trust. The change of trading name is part

of the process to unify the charity and bring staff under one banner, which will allow us to grow and thrive in the future.

Whilst operational services have continued as normal, to ensure that Carers Trust 4all continues to be sustainable and

successful we have undertaken a range of measures to enable the charity to work smarter and become more cost effective.

Part of the process has been the integration of crucial support functions which have been centralised at our Head Office and

include HR, Finance, Fundraising & Development and Communications & Marketing.

Major changes to local government, health & social care and the trend towards strategic commissioning and tendering

continues to increase and strenuous efforts have been made to identify new revenue streams to maintain growth and

sustainability and importantly fund essential services for carers and those they care for.

We are fortunate to be led through strong Governance provided by our experienced and dedicated Board of Trustees.

Following recent mergers we have further enhanced our organisation through the development of a Strategic Leadership

Team and Senior Management Team supported by trained dedicated staff at all levels. Registered Care Managers in each

location ensure the organisation is safe, caring and responsive to the needs of carers and the people they care for. Our

organisation has been further enhanced by the appointment of a Training Manager.

In 2015 we will celebrate our 25th year supporting carers and the people they care for.

Look out for our 25 - 4 - 25 fundraising events where we’ll challenge all our supporters to raise money and awareness for

carers by taking part in events focusing around the number 25.

Quality, dignity and excellent value continue to be high on our list of priorities when delivering support services to carers and people with care needs and we were delighted that we continue to receive positive outcomes from our inspection by the Care Quality Commission.

Investing in our people

We invest in an on going programme of recruitment to ensure effective service delivery. Our workforce plan includes the recruitment of Carer Support Workers to compliment and grow our services. Maintaining a competent and trained workforce is key in delivering a range of services to a diverse range of people’s individual varied and complex needs. We have a rolling recruitment process, plus a comprehensive induction and ongoing training programme.

Our core training plan for the year means we have delivered a wide range of training and have maximised use of resources such as e learning, gov-ernment funding and specialist organisations willing to share their exper-tise with us.

This year we have recruited a dedicated training manager who will ensure that all our staff have the right training.

Following several mergers we have undertaken a skills audit of the trustees to ensure that we have the best people, with the right skills, supporting the charity. The team have welcomed new changes to practices and value ongoing training updates which ensure that best safe practice is adopted.

Putting Carers at the Heart of Everything

Best value

Making sure we work to remain competitive is essential to ensure that services are affordable whilst maintaining a quality service. Our menu of services will support carers to use personal budgets or direct payments which are person centred, flexible and affordable. We are seeing an increasing demand for this service and will continue to respond by delivering high quality care and support to carers and people with care needs in the local community. We work with relevant statutory, third sector and private organisations to ensure that all those who may benefit from a Carers Trust 4all service are aware of what is available and how to access appropriate support.

We continue to see growing demand for carer breaks indicating the needs of carers is huge; therefore we are committed to rising to the challenge of securing funding for future years.

There are two wonderful singing and musical projects run by Carers

Trust 4all. These groups are designed to support people over 50 or

those with a poor memory, dementia, and additionally the unpaid

carers who support them. The aim is to enjoy the therapeutic benefits

of singing the songs we remember, and the memories we associate

with music and lyrics.

Why Music?

The groups are based around music and singing as they have been

shown to have a wide range of benefits for people, including those

with memory problems or dementia:

• Singing Together is great fun! Singing in a group also makes it

easier for people to quickly make friends.

• Almost everyone can enjoy music, even those people who

have lost the power of speech can hum along, tap their feet,

clap their hands, play percussion instruments or even dance!

• Singing also brings health benefits, for example it improves

lung capacity and improves breath control; it also stimulates

memory and

conversation.

In honour of our 25th Anniversary in 2015, Carers Trust 4all is excited to launch our 2015 Fundraising Campaign:

We don’t mind what it is, as long as you raise a few pounds along the way. For 25 years we have been dedicated to providing high quality services for carers and the people they care for. Carers can be any age and care, unpaid, for a family member or friend who, due to illness, disability, mental health problems or addiction, cannot cope without their support.

Our Finances

The total income for Crossroads Care Cheshire, Manchester and Merseyside Ltd in the year ended 31 March 2014 was £3,667,053 of which £2,336,305 was secured from local authority, NHS and charitable grants and £140,034 was from donations and fundraising. Income from services to private clients was £931,274.

This year we had exceptional income of £223,468 from the transfer of funds from merging organisations of which £97,864 came from Crossroads Care Liverpool, Sefton and Warrington Ltd and £125,604 came from Crossroads Care Cheshire West, Wirral and Shropshire Ltd. The bal-ance of income came from interest on our bank ac-counts and income from the hire of rooms and facilities and from our fundraising activities.

The total expenditure for the year was £3,421,055 leaving a surplus of £245,998 (with a 'real' surplus of £22,530, excluding the exceptional income from the transfer of balances from merging organisations).

Overview of Shropshire

Shropshire is England’s largest inland county said to be almost as big as Majorca....but not as warm!

Shropshire Council covers an area of 1234 square miles; in fact South Shropshire alone is 395 square miles. Shropshire’s population is older with a 17% higher proportion of older people than the National average.

Service

We provide Carer Breaks in Shropshire, supporting carers and those they care for emotionally and practically; enabling them to continue with as normal a life as possible; retain the skills they have and maintain their physical, mental and social wellbeing for a longer period.

The basic premise is that when we are there taking over the carer’s role we do what the carer would be doing and a little bit more to help the carer as much as possible; this is identified at the initial assessment stage and then updated annually or when circumstances change.

During our visits we listen and support peoples’ social and emotional needs; providing stimulus and supervision while the carers takes a break.

We provide one to one support to enable people with care needs to go on outings where they are able to access community based activities or attend appointments.

Carers Trust 4all are the foremost organisation supporting carers and the people they care for and we have offices in six locations throughout the north west.

We provide the best quality service and our highly trained staff will provide you with a professional, safe and friendly service.

We will tailor a support plan to suit your needs using 25 years experience and this ensures that you get the very best service available.

Examples of our home-based services Morning support, including help to get up, wash or

Reginald is a gentleman in his early eighties; he and his wife Jean have two sons, however both live a considerable distance away. One of Re-ginald’s daughter-in-laws travels from Cumbria once a month at the weekend to offer Reginald some support. Recently Jean had a fall and broke her hip. Following a long spell in hospital and a reablement programme, she is now more reliant on Re-ginald than ever, despite having support to assist with Jean’s personal care, Reginald rarely has a break from his caring role, so was referred to Carers Trust 4all for a Carer Break. The Carer Break helps to ensure that Reginald has some time to himself and is able to enjoy a little social time, Carers Trust 4all provides a Carer Support Worker to spend 3 hours a week with Jean. This means that Re-ginald is able to go out to meet friends for a game of bowls feeling reassured that Jean will have stimulating company and be well looked after in his absence.

Kath is a very independent lady, having had a stroke she endeavours to maintain her independence and likes her husband to be able to have some time to himself

Kath’s stroke has left her with a left sided weakness and despite her determination to be independent she relies very much on her husband to assist her with day to day living skills.

To enable Michael to do the weekly shopping, knowing that Kath will have someone with her in his absence, Carers Trust 4all provide a Carer Support Worker to support Kath while he is out.

Kath enjoys the company and any assistance with tasks she is unable to do without support and Michael is able to do the shopping free from the worries that Kath would otherwise be lonely or left unsupported.

Case Studies

Carers Trust 4all support people in a variety of ways, below are two case studies where Carers Trust 4all have provided invaluable assistance.

At this time of year there are dozens of runs, swims, cycles and

an assortment of other challenging events.

One of the best ways that people raise money for Carers Trust

4all is by signing up for one of these events and then collecting

sponsorship which they later donate to our charity.

Over the last 12 month the children’s services have provided a whole range of activities for children and young people with disabilities. Disabled children have enjoyed regular Fun Club activity days where they have created some wonderful crafts including a seaside frieze. The children have enjoyed the facilities at Overton House and had teddy bear picnics in our lovely garden, made a host of biscuits and cakes – all based on the themed activity for the day. The children have made gingerbread skeletons, snowmen and Christmas tree cakes, they have made their own Father’s Day and Mother’s Day cards, the children have enjoyed making pancakes and Valentine cards, they have had Easter egg hunts and always enjoy a DVD, story time and a sing-along session. The Asperger’s Social Skills Groups, run by a Speech and Language Therapist, have had regular therapy sessions. Taking advantage of our sensory room, training kitchen and activity space, the group work on developing a range of social skills that help equip the young people to cope better with every day social interactions that they find difficult. In addition to the structured Social Skills Group the young people also have a regular social outing; this helps them to put into practice their social skills and provides opportunities for the young people to participate in everyday social activities that other young people of a similar age enjoy. The outings that they have enjoyed have included trips to The Sea Life Centre, Lego Land and entertainments like bowling and golfing and Laser Quest. There have also been social skills groups for adults and young adults where they have had opportunity to enjoy appropriate social activities – the adults groups are actively involved in planning and arranging what they do and where they go, they have had barbecues and games evenings at Overton House, and have enjoyed meeting up at a local pub for an evening meal with friends.

CHILDREN’S SERVICES

“Our son really loves attending the groups. We really feel that he has come such a long way since he first started attending - keep up the good work!’

Barnstondale Residential Weekends Carers Trust 4all has extensive experience in running clubs for children and young people with disabilities. Working in partnership with the Children and Young People’s Department we have provided outreach work in the community for children with complex needs and disabilities for over 22 years. We cater for a wide variety of

disabilities and additional needs.

Our clubs are a real success. The sessions provide opportunities for young people to interact and develop and also provide a much needed break for carers to enjoy time for themselves or with

family and friends.

The service

We offer a ‘Holiday Break – Away’ eight times each year for children and young people with additional needs. We provide the service at the Barnstondale Activity Centre in the Wirral. The breaks offer full board and an exciting activity programme planned

around the needs of the child or young person.

The breaks provide positive experiences for children by promoting friendship and encouraging social activities, new experiences and supportive relationships. We ensure that the provision we provide is available on a planned and regular ba-sis at times when families and young people are

ready for a break.

Manchester has seen a steady growth of home care support and carer breaks delivered over the last 12 months. The increase is predominantly due to self funding through Individual or Personal Budgets and Independent Service Funding. This is a reflection of the high quality service that is delivered in Manchester. From the feedback we have received we believe that people are choosing Carers Trust 4all because they recognise quality in the service we provide.

Over the last 12 months, there has been an increase in referrals for larger packages of care through Continuing Health Care, whereby the role has been to provide a “personal assistant” role and also through the Adult Mental Health Team. This is an area we will be working with health partners to looking to provide more support in the future and is an opportunity to develop additional services to support carers and the people they care for.

In order to enable the Manchester location to continue delivering high quality services we have seen over 5 carer support workers complete their QCF Level 2 & 3 in Health and Social Care and over 10 staff members completed their NCFE Level 2 in Safer handling of Medications and Dementia Awareness as the number of referrals for people with dementia has increased.

The staff attended the 100th birthday party for Peggy Salts, who has used the service for over 4 years. Peggy utilised the service to enable her to remain living independently and was definitely a firm favourite with all our carer support workers as her cheery and friendly nature was inspiring. Unfortunately Peggy has now passed away but she fought bravely to enjoy her life so that she could celebrate her 100th birthday with family and friends.

Carers Trust 4all have worked in partnership with Marie Curie to provide holistic care and support, to people in the last 3 months of life across Liverpool since October 2012.

We deliver 24 Hour 365 days support to carers and people with care needs.

The service is available on the basis of need against an agreed predefined criteria, irrespective of diagnosis and can respond to crisis and planned respite for carers.

We operate alongside and support the core services provided by GPs, district nurses, specialist nurses and specialist palliative care

STARS Project Liverpool

Palliative care is the active holistic care of patients with an advanced progressive illness. The main focus of palliative care is to achieve the best quality of life for patients and their families. The Carers Trust 4all Palliative Care Teams deliver an excellent, fast response, palliative care service to enable those with a life limiting illness to die at home if

that is their preferred place of care.

The needs of those accessing the palliative care service can change on an hour to hour basis and staff with specialist training are vital to meet families emotional and practical needs at all times. It is essential that this is a fast response service and our dedicated team aim to respond quickly and flexibly to meet the needs of families, often responding the same day to make sure those who need support can

access it.

Our staff may not be able to add days to life, but they work hard to add life to those remaining days. We go to great lengths to ensure all of our patients make the most of every day and, by doing so, help families build positive memories to help them through the most

difficult of times.

Crossroads Care Bury started the year celebrating 21 years providing support to local carers, plus a strategy to merge with a suitable Crossroads scheme in order to continue providing services to Carers in Bury. Whilst the Trustees were busy strategically with merger plans as well as governing the scheme, the daily operations continued to expand with the commencement of a new information and advice service in the three local ASDA stores. This service proved successful in raising the profile and ASDA Bury continue to be a staunch supporter of Carers Trust 4all. The Board of Trustees in Bury selected Cheshire, Manchester and Merseyside as their mer-ger partner and from Christmas 2013, the merger gained pace with a merger date of 1 April 2014. The Care team continued to work very hard, delivering over 48,000 hours of support made by some 70 support workers. The service they provided has diversified over the year as more carers and people with care needs received personal budgets and they exercised their right to choice and control how they spent their budget within Bury The Christmas Ball proved a huge success raising over £1,000. Several members of staff along with patrons, Will Ash and Jack Deam ran the Great Manchester 10k race in May which boosted funds to pay for additional services. An abseil down Peel tower was completed by Paula Nelson and her family which raised £2,000 for Musical Memories. To celebrate 21 years in Bury a special annual general meeting was held in the presence of the Mayoress and key dignitaries produced a film on ‘A day in the life of a support worker’. The Musical Memories group ended the meeting with a live performance which soon had everyone singing along!. There was also a special presentation to Barbara Thornton who has been employed as a support worker since day one!. Over the course of 2013, a huge amount of work has gone into merging Bury Crossroads with the newly formed Carers Trust 4all. The trustees and management teams have worked hard to ensure a smooth transition. And though mergers are not without their challenges we believe that the Carers Trust 4all will continue the Crossroads tradition of providing excellent service to carers and the people they care for.

The economic value of the contribution made by carers in the UK is £119bn per year.

Mr Barns is an 81 year old man who has been diagnosed with Alzheimer’s. Mr Barns is in denial and believes that he is mentally fit and well and cannot understand his wife’s concerns and her reluctance to leave him unsupervised. Mrs Barns is 79 years old and is the sole carer for her husband.

Mrs Barns is finding her caring role increasingly stressful; she cannot find time to attend to daily tasks, attend appointments, go shopping or simply find time for relaxation. Mrs Barns has three daughters who can only offer limited support as they live outside the area; additionally Mrs Barns keeps her worries about Mr Barns’s worsening condition to herself as she does not wish to cause her daughters anxiety.

On one occasion, Mrs Barns went to the shops as Mr Barns was keen for her to go and told her that he would be fine. On her return, Mr Barns had disappeared and eventually the police were called. Mr Barns was discovered in a local park, upset and confused as he did not remember where Mrs Barns had gone.

Mrs Barns contacted the Alzheimer’s Society to request urgent help and was referred to Carers Trust 4all who after an initial assessment visit put in place a regular Carer Break.

Mr Barns was initially reluctant to agree to a Carer Break but with gentle persuasion agreed to a male carer support worker. Mr Barns enjoys the time he spends with his carer support worker; they go on walks to the local lake and visit the garden centre and Mr Barns enjoys male company. Mrs Barns plans her day in advance and visits friends or attends appointments in the knowledge that Mr Barns is safe and well cared for.

At times, Mrs Barns can simply enjoy being at home on her own when Mr Barns is out enjoying the time he spends with his carer support worker.

Mrs Barns’s relationship with her daughters is more relaxed as she is less anxious about her husband’s condition and fears less for the future.

Carers across the UK

There are almost seven million carers in the UK – that is one in ten people. This is rising.

Every year in the UK, over 2.3 million adults become carers and over 2.3 million adults stop being carers. Three in five people will be carers at some point in their lives in the UK.

Out of the UK’s carers, 42% of carers are men and 58% are women. The economic value of the contribution made by carers in the UK is £119bn per

year. Over the next 30 years, the number of carers will increase by 3.4 million (around

60%). The number of people over 85 in the UK, the age group most likely to need care,

is expected to increase by over 50% to 1.9 million over the next decade.

Many of our projects and groups depend heavily on fundraising and donations and we are hugely grateful for all the hard work that people put in to support carers.

Over the last year we have seen thousands of dedicated people run for us. Some as part of the Yule Yomp, 10k and 3k runs organised by Cheshire Hash House Harriers and others who have run independently. We have had people running the 10k race in Manchester, plus other brave souls running the Liverpool and London marathons.

We have seen keen volunteers hold street collections and bag packs at local supermarkets, plus a keen baker selling cakes at Congleton Carnival. We’ve even seen volunteers hosting Play Your Cards Right games at a tea party in the park event in Shropshire.

Staff have got in on the act too as one of our RCM’s, Tina Blake, accompanied by her son and daughter, leapt out of an airplane, thankful-ly they were all wearing parachutes and the jump raised lots of money for Carers Trust 4all.

Staff in our different locations also threw themselves into Carers Trust’s new national campaign - Britain's Best Breakfast. Where in October people around the country were inviting friends around to have breakfast and raise money for carers.

Young Carers and Young Adult Carers

Our Young Carers and Young Adult Carers projects aim to provide the following support for young people:

1:2:1 support Activities and outings Group sessions Trips and breaks away Training and development Support accessing further education Volunteering opportunities Young carers may choose to remain hidden due to fears around stigma and bullying which in turn may leave them feeling isolated and unaware that there are many young carers, across the country who are in a similar position.

Through group sessions young carers across Cheshire East are able to meet with other young carers, of a similar age who also have caring responsibilities.

These sessions not only offer a stress free and safe environment for the young carers, but also a chance to learn and develop new skills through the delivery of informal education and by raising social awareness.

A group of young carers from Crewe and Nantwich area got the chance to experience a trip to Aerial Extreme, the Monkey Forest and Frankie and Bennies, this was through the support of Crewe and Nantwich Rotary Club.

The Young Carers thoroughly enjoyed the day and the high ropes task was a real challenge for some; it was great to see the young carers encouraging and supporting each other through the tasks. Everyone who attended the day really enjoyed it.

The internet continues to grow at a near inexplicable rate and social media is one of it’s key driving forces.

It’s an excellent way to raise awareness about carers and a way to reach hidden

carers, those who don’t know that they are a carer, or those who have no idea where to reach out for help.

This last year has seen a complete revamp of our website and we have increased our use of social media, primarily Facebook and Twitter.

How your legacy gift could help to support unpaid Carers

Carers Trust 4all, a Crossroads Care service, is a network partner of Carers Trust which is a national charity supporting carers across the UK.

What your gift could achieve

By leaving a gift to Carers Trust 4all in your Will, you can support unpaid carers, many of whom are still children. We work with carers as young as 8, who are looking after sick or disabled parents or siblings. Many of whom spend as much as 7 hours every day, outside school, providing care.
