One-on-one tutoring for youth ages 6 to 18 years of age. Services offered include after-school

tutoring workshops, help for English language learners and assistance with student publications.

Other services include daily writing prompts, help completing homework, help reading silently

and aloud. Volunteers help with field trips, assist with the yearly Young Authors’ Book Project

and tutor students after school. Help is offered to students with writing projects in public schools

The National Breast and Cervical Cancer Early Detection Program provides free breast exams

and mammograms for women ages 40 years or older. The Family Planning, Access, Care, and

Treatment program provides family planning and some free gynecological services. Free

immunizations at birth up to 18 years of age. AIDS/STD testing available. Dental and optometry

services available. Health plan enrollment and transportation assistance available. Office hours

Assist Immigrants and Refugees Program

Through Assist Immigrants and Refugees Program, Catholic Charities of Los Angeles assists

newly-arrived immigrants in learning English, obtaining education, navigating legal matters, and

training for jobs that will provide at least a living wage. American social norms are taught as

well. These programs have a proven track record of helping immigrants transition and contribute

ChildNet Youth and Family Services

Consists of five divisions. (Academy/school has closed as of February 28, 2017, per operator.)

Foster Family Network (direct phone number, 1-562-492-9527) recruits, trains, certifies and

supports foster homes for children with 24-hour counseling support. Behavioral Health Services

provides mental health services to low-income youth; Wraparound Support Services is aimed at

keeping at-risk teens out of psychiatric and probation facilities. Office hours are Monday to

Children's Bureau

This location is one of the 21 FamilySource Centers in Los Angeles County providing the

one-e-app system for financial assistance, high-risk youth employment, education services and

case management. Offers a multi-service child welfare agency, child abuse prevention program,

parent education classes and in-home parent training services in both English and Spanish.

Children’s Bureau also offers a parent support group, the NuParent Organization. Foster care and

adoption services, family recruitment, ongoing training, supervision, family reunification

must have one of the following diseases or conditions: ALS, multiple sclerosis, Parkinson’s

Colors LGBTQ Youth Counseling Center

A safe space for lesbian, gay, bisexual, transgender, queer, intersex, fluid, pansexual,

Delancey Street Foundation

Rehabilitation program providing assistance to people with felony convictions, people abusing

substances, gang members, perpetrators and victims of abuse to assist them in rebuilding their

lives. Services include literacy help, homeless issues, anger management, education and

vocational mentoring. Minimum participation is two years. Foundation does not accept people

with mental disorders, disabilities, individuals on medication, arsonists or sex offenders.

Delhaven Community Center

Services offered include emergency welfare for families, homework help program, after-school

recreation and transportation, summer camps, food supplies, new shoes for children enrolled in

Employment Services Program by Disabled Resources Center, Inc.

Disabled Resources Center, Inc. helps to empower people with disabilities to live independently

in the community, make their own decisions about their lives and advocate on their own behalf.

This organization provides services to all people with disabilities, regardless of their age, gender,

Center for the Pacific Asian Family

Services include a 24-hour hotline for women and children who are survivors of domestic

Family Preservation by El Centro Del Pueblo

El Centro del Pueblo is a community service agency for the purpose of alleviating and mitigating

the negative affects of gang violence. We are dedicated to improve the quality of life for children,

of services in Central, Northeast and South Los Angeles. El Centro provides services for families

Haven Hills

Services offered include a 24-hour emergency hotline, an emergency shelter for battered women

and their children, a nonresidential group, and individual counseling for domestic violence

Hillsides Family Center

Center is a residential group home for children and youth 8 to18 years of age. Services offered

are long-term care, short-term emergency services, family reunification, mental health services, a

substance abuse program and a nonpublic school on-site. Serves abused children and wards of the

court. Accepts emotionally disturbed children. Provides therapeutic behavioral services,

Assembly Bill 3632 services and a transitional living/housing program for youth 18 to 24 years of

HIV Prevention

At the Minority AIDS Project, we are committed to reducing suffering and deaths due to

HIV/AIDS in African American and Latino communities by making HIV/AIDS education and

related health services available and accessible. HIV Prevention program provides accurate,

up-to-date health education about HIV/AIDS in communities of color. This is done through a

broad range of outreach activities that work effectively within the broad spectrum of cultures,

neighborhoods and ethnicities in Los Angeles.

We also provide educational services to the public including free HIV counseling and testing,

AIDS 101 classes, staff training in HIV/AIDS issues for social services agencies, speakers for

public events, information at health fairs and community festivals, conference workshops and

trainings of volunteers. Serves Los Angeles, Orange, Riverside, San Bernardino, and Ventura

Home Avenue Adult Day Health Care Center

The Home Avenue Adult Day Care Center seeks to expand health care and human services to

Homeless Prevention by the City of Pomona

The City of Pomona plans, coordinates and organizes strategies to assist the City's homeless

population in collaboration with other public and private organizations. Our homeless assistance

programs provide financial assistance and services to prevent individuals and families from

becoming homeless. Homeless prevention provides assistance to at-risk families through

Housing-Integrated Mobile Health Team Program by St. Joseph Center

Integrated Mobile Health Team provides mental health, physical health and substance abuse

services to chronically homeless individuals through a multi-disciplinary staff working as one

team to help clients obtain and maintain permanent supportive housing. The service model is

designed to improve and better coordinate the quality of care for individuals with a mental illness

and their families, if appropriate, who are homeless or have recently moved into Permanent

Supportive Housing (PSH) and have other vulnerabilities.

The program’s goal is to use a client-centered, housing-first approach that uses harm reduction

strategies across all modalities of mental health, physical health, and substance abuse treatment to

improve client well-being. Serves Los Angeles and Orange County Residents. Hours of

Los Angeles LGBT Center

The Los Angeles LGBT Center offers an HIV clinic, anonymous HIV/STD testing, mental health

Latino Resource Organization

LGBTQ services program responds to the needs of the LGBTQ communities by providing

culturally competent services in a safe, non-judgmental environment. The LGBTQ program

understands that lesbian, gay, bisexual and transgender individuals experience oppression,

Long Beach Multi-Service Center For The Homeless

Fourteen co-located agencies serving the homeless. Centers provide emergency food, seasonal

housing (November through March), shelter services and referrals. Also offer financial, rental,

utility and transportation assistance. Centers offer emergency motel and restaurant vouchers, case

management services, and affordable housing assistance. Office hours are Monday, Tuesday

Mental Health Services

Mental Health Services has complete outpatient mental healthcare services on site. Psychiatrists

Center oversees five different programs for developmentally disabled adults. Offers daycare

programs including health education in weight loss, peer youth education, disease management,

Salida Del Sol Adult Day Health Care

The center serves the region's neediest adults with basic services ranging from exercise classes to

medical services. The center is staffed with full-time or visiting nurses, therapists, dietitians and

other medical experts. Most of the seniors at the center (about 65 patients ranging from ages 24 to

102) require heavy intensive care. Workers at the day care regularly administer medication, feed

seniors, help them to the restroom, monitor blood sugar/pressure and report symptoms to the

Adult Day Health Care of Fresno and Clovis counties

Adult Day Health Care offers occupational and speech therapy as well as dietary and nursing

services. A registered nurse is present daily to evaluate and provide medical and nursing care.

Social work services are available to help participants locate needed services, and counseling is

provided to participants and their families. Serves individuals 18 years of age and older with a

physical or mental impairment. Hours of operation are Monday to Friday, 8 a.m. to 2 p.m.

Agency on Aging — Family Caregiver Support Program

The Agency on Aging offers services through their Family Caregiver Support Program. This

program provides caregivers with practical tools to alleviate stress and avoid burnout,

networking opportunities to decrease isolation and share concerns, and stopgap measures to

stabilize the provision of care during times of extreme difficulty. Services include support

groups, classes and case management. Serves Fresno and Madera counties; accepts Medi-Cal

members. Hours of operation are Monday to Friday, 8 a.m. to 4:30 p.m.

Alcoholics Anonymous (AA)

AA offers a self-help alcoholism recovery program utilizing the 12-step approach. Classes are

offered in English and Spanish at no cost individuals/families. Note, the center is only open for

meetings. Hours of operation are Monday through Friday from 10:30 a.m. to 4:15 p.m. and

Saturday from 9 a.m. to 1 p.m.

Adult Day Centers

OASIS and People Always Learn Something (PALS) Adult Day Centers specialize in Alzheimer

and dementia care — offering therapeutic activities for seniors, and allowing caregivers a

much-needed break. PALS is a program of OASIS for people 40 and older with mild cognitive

impairment in the early stages of dementia. OASIS and PALS provide a full day of stimulating,

recreational and therapeutic activities for people in the earliest to the most advanced stages of

dementia tailored to each participant's personal interests, needs, strengths, cognitive and physical

abilities to nurture self-esteem and dignity. Caregiver support is also provided. Medi-Cal will

not cover the cost of the programs, but financial assistance is available through private donations

and a grant through the Fresno-Madera Area on Aging. Offering attendance options to fit in

Asian Pacific Islander Clinic

At the Asian Pacific Islander Clinic, case management teams provide comprehensive, culturally

and linguistically appropriate outpatient and community-based specialty mental health services

to transition individuals who meet medical necessity. Services are provided using a wellness and

recovery model and include assessments, clinical and rehabilitative services, case management,

peer support, and medication assistance. Hours of operation are Monday to Friday, 8 a.m. to 4

p.m.; walk-ins are accepted.

Behavioral Health — Adult Services

This program provides mental health services to low-income adults in order to help them with

their day-to-day lives. With locations in Bakersfield, Lamont, Delano, Frazier Park and Fresno,

psychiatrists, psychologists, clinicians and therapeutic support staff provide a wide range of

behavioral health care. Adult Behavioral Health Services provide psychiatric evaluations,

medication management, co-occurring capable and specialty mental health services and

Central California Legal Services (CCLS)

The CCLS provides a range of services from advice to court representation for victims of abuse.

They assist with naturalization, legal permanent resident card renewals and U-Visa/VAWA

applications. CCLS employs attorneys, paralegals and support staff to provide civil legal

services at no cost. Other programs offered by CCLS include health services (advice and

representation for those having difficulty accessing health care services) and housing assistance

(representation for administrative hearings on Section 8 voucher terminations). Services are

Central Valley Regional Center (CVRC)

The CVRC offers advocacy, evaluation, assessment, information and referral, and coordination

Clubhouse Youth Program by Fresno American Indian Health Project

Clubhouse Youth Program mission is to enhance the growth and well-being of American Indian

Clubhouse is a no cost after school program for American Indian youth in the Fresno city limits.

Tutoring and educational activities. Summer youth camps ages 5 to 12 years. Offering peer

workshops that develop self-esteem, confidence, and job skills, prevention and awareness

Encourage Tomorrow

Encourage Tomorrow provides after-school and lunch-time youth development programs

including cultural awareness lessons, service-based learning, career development, college

mentoring cultivation, and job skills and shadowing opportunities. In addition, tutorial services

Eoc Health Services Clinic Community Health Center

The Eoc Health Services Clinic Community Health Center provides education, counseling and

treatment services to men, women and teens who are at-risk for unintended pregnancy and

sexually transmitted infection (STI). They offer confidential and comprehensive medical

Fresno County Department of Public Health Community Health Division

The Fresno County Department of Public Health Community Health Division offers standard

hepatitis C and HIV testing, but they also offer rapid oral and blood HIV testing. Additionally,

hepatitis prevention/education, HIV test counseling, HIV/AIDS prevention education, partner

notification, and treatment information is provided. Hours for HIV testing are

Monday to Thursday from 8:30 a.m. to 11:30 a.m.

Fresno LGBT Community Center

The Fresno LGBT Community Center is open to the public. They offer access to information

and local and national LGBTQ resources including local publications such as News Link and the

CCA Newsletter. They offer reference materials relating to the LGBTQ civil rights movement as

well as printouts of recent court decisions affecting the LGBTQ community. Hours of operation

are Tuesday through Saturday from noon to 5 p.m.

Fresno Rescue Mission

The Fresno Rescue Mission offers an emergency family shelter that provides 14 to 30 days of

temporary shelter to families as well as food, clothing and personal care; referral to other

agencies that can assist them; and love/hope through spiritual support. To assist them in starting

over in an apartment, families are given food packages and other needed supplies when they

leave the shelter. Monday through Friday from 8 a.m. to 5 p.m.

Gay and Lesbian Center — Bakersfield

The Gay and Lesbian Center seeks to improve the well-being of the LGBTQ community with

cultural and educational programs and an extensive referral network. They offer community

building socials and other activities like picnics, dinner outings and camping trips. They also

offer a program that brings together lesbian, gay, bisexual, transgender and ally professionals

Guardian Angel Home Health Care-Rehabilitation Service

Guardian Angel Home Care provides quality home health services which meet the needs and

expectations of those injured, ill, disabled, or challenged. Guardian Angel Home Health Care is

comprised of multi-disciplinary professionals who have expertise in skilled nursing, physical

therapy, occupational therapy, speech therapy, medical social services, and home health aide

assistance. Serves California residents. Hours of operation are Monday to Friday,

8:00 a.m. to 5:00 p.m.

Heritage Adult Day Health Care

Heritage Adult Day Health Care offers therapeutic services to seniors. Transportation to and

from the facility is offered at no cost, and breakfast and lunch are provided. Patients typically

stay at the center for five to six hours. Nurses are on staff to monitor each patient’s health, social

workers provide 1:1 sessions, and nutritionists provide consultation. Every week, patients are

taken on field trips to various stores to purchase hygiene products. Serves Fresno County

residents; accepts Medi-Cal members. Hours of operation are 7:30 a.m. to 4:30 p.m.

Homeless Veterans Taskforce by The American Legion

To help our struggling homeless veterans, the Legion's Homeless Veterans Task Force, works to

ensure local services and resources are available to homeless veterans and their families. The

Task Force, which has chairpersons in each department, collaborates with government agencies,

homeless service providers and veterans service organizations to develop and implement

initiatives that will help homeless veterans. Additionally, Homeless Veterans Task Force

chairpersons can provide direct aid to veterans and their families who reach out to them. A

veteran who is homeless or facing possible homelessness can contact a chairperson, who will

provide guidance and assistance to the veteran and his or her family. Serves California

Residents. Hours of Operation are Monday to Friday, 8 a.m. to 5 p.m.

Kings View’s Blue Sky Wellness Center

The Kings View’s Blue Sky Wellness Center offers mental health services to adults by focusing

on key mental health recovery principles including dispensing hope, freedom of choice,

modeling self-determination and personal responsibility, and emphasizing that people can and

do heal from a mental health diagnosis. They offer self-help and support groups as well as a

variety of activities such as computer access, library time, a weekly clothes closet and a gym for

Legal Services for Immigrants by Mil Mujeres

Mil Mujeres providse legal services, community education and empowerment to low-income

Hispanic immigrants and their families in the United States. They offer immigration legal

services to the low-income individuals in the Latino community, specializing in immigration

benefits for survivors of violent crimes in the United States. Our main focus are survivors of

gender related crimes. However, we also provide services to other survivors. Offering help in the

following areas: U-visa (for victims of specific crimes), Violence Against Women Act, Deferred

Action for Childhood Arrivals, T-visa (for victims of trafficking), and Special Immigrant

Juvenile Status.

Madera Rescue Mission

The Madera Rescue Mission is committed to the needy, destitute and homeless. They not only

meet temporary needs but also offer long-term help and solutions to those who truly desire a

change in their life. Serves Madera County residents. Hours of operation are Monday to Friday

Marjaree Mason Center

The Marjaree Mason Center offers individual and group therapy for adults and children affected

by domestic violence. They provide emergency and long-term safe housing, legal and financial

Noami’s House

Noami’s House provides overnight shelter to homeless women 365 days a year. They also offer

services at no cost through the combined resources of the Collaborative including hot meals,

medical and dental care, a day shelter, laundry and clothing assistance, case management,

storage, transportation, health and personal living education, and information and referrals.

Assessments are offered Monday to Friday at 3:30 p.m. and Saturday and Sunday at 12:45 p.m.

Safe Houses — Fresno Emergency and Transitional Living Safe House

The Safe Houses program assists survivors of domestic violence by providing them with

transitional housing, case management and educational opportunities. This program helps people

develop independent living skills and a means of income to help in attaining the necessary

resources to support their move into permanent housing free of domestic violence. Serves Fresno

Substance Abuse by Promesa Behavioral Health

This Substance Abuse program offers individual and group drug and alcohol counseling in an

outpatient setting to individuals who struggle with addiction. A wide range of addiction

The Bay Area Community Services Center offers cost-effective community-based services to

Behavioral and Community Services provide a broad range of counseling services, BCS staff offer group counseling in anger management, parenting and co-parenting skills. BCS houses most

of the outpatient mental health services of the agency that are outside of the Children's Services. For clients seeking therapy or counseling, therapists meet with new clients to complete an intake assessment to determine a treatment plan. The length of services depends upon the specific

Child Abuse Treatment Program (CHAT) by Child and Family Institute (CFI)

With funding awarded by the Federal Victims of Crime Act, Grant AT13101111, through the California Governor's Office of Emergency Services (Cal OES), CFI is able to offer

psychotherapy services to children and youth victims of crimes. CFI is able to serve children and youth victims who do not have access to such services due to a lack of resources, lack of insurance or Medi-Cal ineligibility. Treatment is offered to victims of physical, sexual, and

emotional abuse, parental substance abuse, child abduction, bullying and domestic violence. Serves El Dorado, Nevada, Placer, Sacramento, Sutter, Yolo, and Yuba County Residents.

Community-Based Adult Services by California Department of Aging

Community-Based Adult Services is a day health program for older adults and adults with certain chronic medical, cognitive or mental health conditions, or disabilities who are at risk of needing institutional care. This program used to be called Adult Day Health Care (ADHC). The primary objectives of the program are to restore or maintain optimal capacity for self-care to frail elderly persons or adults with disabilities and delay or prevent inappropriate or personally undesirable institutionalization. Services provided at the center include the following: professional nursing services; physical, occupational and speech therapies; mental health services; therapeutic activities; social services; personal care; hot meals and nutritional counseling; and transportation to and from the participants’ residence. Community Resources for Independent Living (CRIL)

CRIL offers independent living services to those with disabilities. Services include housing

Del Oro Caregiver Resource Center

The Del Oro Caregiver Resource Center is a nonprofit organization that serves individuals and

families who are providing care to brain-impaired adults and the frail elderly. Services include

information and referral, family consultation, family counseling, respite care, legal/financial

consultation, education and training, and support groups. Serves Alpine, Amador, Calaveras,

Colusa, El Dorado, Nevada, Placer, Sacramento, San Joaquin, Sierra, Sutter, Yolo and Yuba

Emergency Housing-Family Shelter Program by Next Move

Family Shelter provides short-term emergency housing, life skills coaching, alcohol and drug recovery support and education, on-site SETA head start, laundry services, housing relocation assistance, case management for one year, assistance with clothing, shoes and housewares to homeless families up to thirty days. Emergency Youth Shelter by Wind Youth Services

Wind Youth Services provides supportive services and opportunities to youth experiencing homelessness as they pursue self-determined lives of stability and independence. Through our programs and services, Wind Youth Services provides homeless and at-risk youth between the ages of 12 and 24 with basic safety net and survival services and a safe place to stay. The Emergency Youth Shelter provides a temporary, low barrier, emergency overnight shelter for youth ages 12-17 (18 and in high school is okay). This LGBTQ-friendly, trans-affirming, pet-friendly emergency shelter is the only of its kind in the greater Sacramento area, and providing a safe, non-judgmental, and supportive community environment. Services include, hot meals, counseling, case management and more.

Fountain Recovery Drug and Alcohol Treatment

The Fountain Recovery Drug and Alcohol Treatment program offers help to individuals and

families affected by alcohol and drug addiction. The program is 12 step-oriented as well as

abstinence- and evidence-based and has a foundation of encouragement, love, practical guidance

and personal attention. Their small client-to-staff ratio allows for a client-centered approach that

offers individualized attention to meet each client’s specific needs. The program has four

different levels of care: residential treatment, day treatment, intensive outpatient treatment and a

sober living environment. Follow-up to treatment includes a free Aftercare Group for clients

who have successfully completed treatment. They accept most insurance. Hours are Monday

Golden Rule Services

Golden Rule Services offers a variety of no cost, confidential HIV, STD and hepatitis C

education and prevention services that are accessible, affordable, and culturally and

linguistically appropriate. Services include rapid HIV and HCV testing, STD screening, needle

exchange, harm reduction services, HIV/STD risk reduction support groups, case management

services, individual peer-based risk reduction counseling, latex and polyurethane condom and

lubricant distribution, limited transportation to medical appointments, and information and

include dietary consultation, nutritional education and daily hot meals. Recreational activities

Opening Doors provides tools and support for survivors of human trafficking, refugees,

situation by starting or expanding small businesses and to help them take control of their

personal finances by educating them about the U.S. financial system; a Survivors of Human

Long-term care provides professional nursing and therapy for chronically ill or permanently disabled individuals. The IHSS program helps pay for services to help residents remain safely in their home. Services

accompaniment to medical appointments and protective supervision for the mentally impaired.

Sacramento Covered is a community-based nonprofit organization with a focus on improving

the overall health of Sacramento county residents. Services connect individuals and families to

underserved people; help the community overcome the challenges preventing them from getting

facility providing acute inpatient services to indigent residents and Medi-Cal members. The

Second Chance

Second Chance offers an emergency shelter for individuals and families. The maximum stay is

well as an individual and family counseling 12-step program. Serves Sacramento County

residents. They offer a "no-fee" rehabilitation program. Hours of operation are Monday through

Friday from 7:30 a.m. to 2:30 p.m.; intake is Monday through Friday from 8 a.m. to 2:30 p.m.

The YMCA of Superior California

The YMCA of Superior California offers programs to help youth learn and thrive through every

stage of life. Programs include aquatics and swim, childcare, health and fitness, summer camps,

youth development, healthy living, and youth sports. Serves Greater Sacramento area residents.

Hours of operation are Monday through Thursday from 5:30 a.m. to 10 p.m., Friday from 5:30

a.m. to 8 p.m., Saturday from 7 a.m. to 6 p.m. and Sunday from noon to 6 p.m.
