“THAT’S NOT MY PATIENT!” Things to Know“Empanelment” is the process of comparing the patients that your managed care organization has listed and the patients you have listed. HEDIS measures account for everyone who is listed as your patient, not just the ones who you see on a regular basis. You can provide better care to your patients when you know exactly who is on your panel.

Frequently Asked Questions:Won’t empanelment take too long? What if I don’t have enough bandwidth?

Initially, empanelment is a long process, but that your practice facilitator is here to help. After you clear up your panel and know who your patients are, the process can easily be completed monthly.

What happens to the patients who shouldn’t be on my panel?

You will report those patients to your managed care organization, and they will be assigned to the correct provider.

What if I don’t know how to run a report of my patients?

You can contact your EMR representative who can run a report for you or show you how to run a report.

Frequently Asked Questions:What if the clinicians in my practice don’t want to change?

Change is not easy, but if everyone can be involved in the change process and work on a PDSA cycle together, the clinicians often will start to see that change can be good.

How long does a PDSA cycle take?

A PDSA cycle should be short (no more than 4 weeks). The goal is to see if the change process is worth implementing.

Things to know“Quality Improvement” can refer to anything in your practice that you want to change.

QI: QUALITY IMPROVEMENT

• In P-TCPI, we are focused on improving well-child visit rates and immunizations.• All quality improvement involves several cycles of Plan, Do, Study, Act (PDSA).• Check out this 3-minute YouTube video, PDSA Cycle 101, and plan one thing you can test for improvement

TEAM-BASED CARE

Things to knowTeam-based care means:

• Clinic staff work together to the maximum of their certifications, but not beyond their certifications.• Staff understand their job descriptions and how they relate to the overall success of the practice.• You can work together instead of in silos.

ResourcesThe first step of helping your team work together is taking the time to check in before the day gets away from you. Check out this AMA’s module, Implementing a Daily Team Huddle: https://www.stepsforward.org/modules/team-huddles .

Frequently Asked Questions:I already feel like we work as a team, why do we need a huddle?

A huddle is a great way to check in with each other to proactively deal with issues. What if our practice doesn’t have time for a huddle?

The idea is to keep huddles short, no more than 10 minutes, and to deal with larger issues at a later time. How do we know if we are doing team-based care?

Does everyone in your practice understand their specific job description and how it relates to the success of the practice? If so, you are doing team-based care. If not, there is more to learn.
