Apply to join AYSO Region 891 - 1- AYSO REGION 891, Highland Park, IL

Apply to join AYSO Region 891. You will make application for a player, adult or youth volunteer, in our region, by using AYSO's web based program. The web address is http://www.eayso.org FireFox and Internet Explorer are the recommended browser applications. You can also use Safari but some of the features may conflict. The web site is secure. Region 891 uses the most advanced version of eAYSO which will let you give a Digital Signature. This means as a volunteer or player you no longer need to bring or send us paper forms. On the page below you see the opening screen when you launch eAYSO.


http://www.eayso.org Welcome Screen >If you have not used the software before click on First Time. You will be led through steps to create your own PROFILE with a LOG IN and PASSWORD (RED Arrow in illustration, below). GO TO NEXT PAGE OF THIS GUIDE >If you are a returning parent, you will click the second button marked LOG - IN (BLUE arrow in illustration, below) and the software will take you to the main menu where you will choose your activity. PLEASE SKIP TO PAGE EIGHT below.


FIRST TIME USER will choose First Time at the opening screen and is led through the following screens

As a first time user you initially choose to make application for a child player, or for an adult or youth volunteer. To make application for a child player >Choose the first choice to move to the player application process, then click “NEXT”

When choosing to make application for a child (red circle illustrated on the previous page) you are asked to provide information about YOURSELF (not the player). Fill in your name and home telephone number in the fields provided. eAYSO checks if you may already have an older profile or have previously made application for a child. We do not want duplicate records. Click NEXT to move to the Profile Creation screen. You will be asked to create a profile, a unique password protected access. Click NEXT to move to the Profile Creation screen


Create your profile: YOU MUST CREATE a PROFILE to apply to Region 891- DO NOT SKIP this step!!

Create your profile- The field entitled "EMAIL" is where you create your LOG -IN. This is in the form of an email address. This email address is NOT used to send you messages. It is strictly a Log in or ID. This LOG- IN does not change. It is static. If you change the email address you use for messages, you will still need to use the email address you typed in here to log-in, to access your player's record or your own volunteer record. Please make a note of the email address you use and make note of your password. YOU MUST CREATE A PROFILE TO MAKE APPLICATION TO REGION 891 - DO NOT SKIP THIS STEP Remember your Log In and Password . You will need it again to use all eAYSO features and to apply next year. Click NEXT at the bottom of the Profile Creation screen to begin the actual player application.


After creating your profile, you are ready to complete an application for a player.

Click NEXT at the bottom of the profile creation screen The player application has six steps or tabs. The first step asks you to choose a region. We are Region 891. Enter that number in the field (box) as shown below. Proceed through the remaining tabs or screens in the application by clicking NEXT at the bottom of each information-gathering field. Some fields are required, marked with an asterisk, others are not required but we appreciate as full information as you can provide.


THIS CONCLUDES INSTRUCTIONS FOR FIRST TIME USERS

The next two screens illustrate how a parent or volunteer who previously made a profile returns to eAYSO to make player or volunteer applications, pay on-line, update information, view a child's team assignment or use the other features offered by eAYSO. **************************************************************************************** New users may skip to Page 10 of this guide for instructions on completing the player application. But please note how easy it will be to return in the future, now that you have a profile. Store your Log In and password where you can easily find it to return to your player or volunteer records.


RETURNING USERS who previously created a profile:

Returning parent with profile: Choose Login on the main screen (shown on first page of this guide) and input your log in and password in the next screen, illustrated below. This brings you to the MAIN MENU page.


Main Menu: Returning parent can now select from the player-related or volunteer related menus.

Choose Application for New Player or Application for returning player who played AYSO last year or in previous years. OR choose to apply as a new or returning Volunteer (Re-application required of all volunteers each membership year.)


COMPLETING THE PLAYER APPLICATION Instructions and tips for all eAYSO users

The player application has six tabs or screens .The first step asks you to indicate the region you wish to join. We are Region 891. Enter that number in the box. Click "Next" to advance through the remaining screens. BE SURE TO PROVIDE AT LEAST ONE VALID EMAIL ADDRESS; There are three email address fields, one for the child applicant, one for each parent. Do provide minimally one email address. REGION 891 relies exclusively on email communication; if you do not provide a working email address you will miss vital program information and it will be very difficult for your child's coach to contact you. You will move through six screens asking for name, address, birth date, telephone information, emergency contact information, information about yourselves as parents, etc. If you skip a required field the software will not let you proceed to the next screen. Keep in mind that this information is stored so in future years you will only have to update information which has changed. A bit of effort now will make submitting future applications effortless. Parent Telephone Numbers: Please provide your HOME telephone number in the home telephone field. There is a dedicated field for cell and workplace and business numbers. Reapplication for a returning player follows these same steps as for a new player. You will be led through six tabs or screens. All you need do, to re-apply, is to update fields with old or wrong information. A quick process! THE NEXT SCREENS DESCRIBE TWO IMPORTANT STEPS IN THE PLAYER APPLICATION: - Capturing YOUR volunteer interests to help the region - The digital signature process


EVERY PARENT: Please HEED this next screen: Parent Jobs, Tab 4 in the player application process.

Here you will indicate your volunteer INTERESTS The fourth screen is important. Here you are reminded that we need YOUR help as a volunteer. You are asked to indicate your volunteer INTEREST. >There are some roles which will be specific to this child and his team (coaching, team parent for example). So some volunteer roles are listed under your child's name. Please choose the child-specific role in which you are interested for each of the children for whom you are making application. >The final right hand column "Jobs NOT child specific" lets you indicate ways you wish to help us not connected to a specific child or team. (i.e., help distribute photos, trophies, etc.) PLEASE do not bypass this screen. NOTE: this is NOT the official volunteer application. In order to help as you indicate HERE you will have to complete the separate volunteer application. You will do that after you have finished making application for your child(ren) by returning to the Main Menu screen and clicking on the Volunteer Application link on the right hand column on that screen. Your volunteer interest(s) selected in the player application will be reflected in the Volunteer Application and can be edited.


The second to last screen; all important Digital Signature. Follow instructions carefully to complete the

three important steps Three important steps: 1) click in the checkbox (highlighted by green arrow) 2) type your name.(circled in red in the image above); the date will be added automatically 3)CLICK REVIEW at the bottom of the screen(highlighted by red arrrow) to get to the final SUBMIT screen. Your signature of agreement to the waiver, assumption of risk and consent will be recorded digitally.

The application will not be complete and will not be filed if you skip any of

these three steps.


Ready to give digital signature- after completing all the other steps - follow these directions carefully to ensure

the application is captured by the software.

After all information is entered in the earlier steps you will come to this second -to- last screen. NOTE the instructions, in blue at the top of the screen, highlighted here by the red arrows. Scroll down this page to get to the check box and yellow field in which you type your name- as illustrated in the next page of this document


Submit Screen, the final screen in the player application- follow the BLUE instructions on top of the screen

carefully to complete the SUBMIT screen.

When you click REVIEW on signarure page, you are taken to the final SUBMIT screen with a message to scroll down. The date has been added electronically. Your signature has been captured electronically as stated by the text now appearing on the lower left corner of this screen. CLICK SUBMIT and you have completed the application!!!!!


Success! Application filed for a player

Directly after this green message appears the software will open back at the MAIN MENU page. NOW you may: a) Make application for another new or returning player. b) Pay online, or go to the LEFT side column and c) Choose to APPLY as a volunteer or apply as a returning volunteer ******************************************************************************************** IF you do NOT see this green message your application has not been filed. (You will be unable to pay or print the form and unable to pay on - line). Most likely you skipped a step. What to do? Return to the Main Menu, click on the Update Player information option, choose to EDIT your child's application, page through all the steps being careful to complete all fields, continue through the final two pages, typing your name, reviewing and submitting as described, above, in this document. Now the correctly completed application will be accepted and you can proceed to pay online. We suggest you print a copy of the player application- as further confirmation that your child's application has been done correctly.


Back to the -Main Menu-to make more applications for players or as a volunteer AND to pay online AND

complete your volunteer application You now can : make application for another new player OR make application for a returning player OR Pay online OR apply as a new or returning volunteer All volunteers new or returning, adult or youth, MUST File a fresh application each Membership (or Playing) year.


eAYSO is much more than registration software! Take a look at the menu options: After your child's application is processed by our registrar, YOU can log in, see the team number and coach contact information. You can also update your family demographic information if address, phone, email address etc changes. Note the dark blue area, upper left. Here you can change the email address to use as your Log IN, change your password AND (if coach or referee volunteer) sign up to take a requisite training course. Our non -subtle last word. DON'T FORGET TO REGISTER AS A VOLUNTEER! Even as little as two hours a year will help Region 891 continue to provide a high quality program for your child. If you have time, consider coaching or officiating. You need know nothing about soccer. AYSO's proven training will prepare you to be a good coach or game official. Help us help your child! The volunteer application process is similar to that of the player application process. You will be asked for certain information which is kept encrypted and sequestered at the AYSO National Office. This information is needed for the background checks conducted on each volunteer. AYSO wants our youth kept safe!
