Types of Inspection Forms• Two types: Self Inspection and Form I• Two types: Self-Inspection and Form I

• Self-Inspection

• Sheep, Goats, and Swine (must have Premise/Scrapie ID)• Can be used for in-state movement, change of ownership, shipment out of state, commercial inspection , g p, p , p

slaughter, and custom slaughter (see slides 5 and 6 for instructions on custom slaughter)

• Range Cattle (must be branded)• Can be used for in-state movement or commercial inspected slaughter; must have a Form 1 for change of

ownership, custom slaughter, or shipment out of state

• Dairy, Feedlot, and Non-Range Cattle• Can be used for in-state movement, change of ownership, shipment out of state, and commercial

inspection slaughter

Self-Inspection Fees• Cattle (Range, Non-Range, Dairy, and Feedlot), Sheep, Goats, and Swine

• $25 for a 25-page feedlot self-inspection book

• $20 for a 20 page dairy self inspection book• $20 for a 20-page dairy self-inspection book

• $10 for a 10-page self-inspection book for non-range, range, sheep, goat, and swine owners

• You will not receive your certificates until you have paid and we have received the fee

• Beef Council Fee of $1.00 per head has to be paid with the completed certificates (No charge if transporting to an in-state auction or if the purchase of the animal was less than 10 days prior)

• Equine are not included in the Self-Inspection ProgramEquine are not included in the Self Inspection Program

Form I Inspection

• In order for a Form 1 to be issued:• Range cattle should be branded with the owner’s current Arizona recorded brand.

• The owner will have an inspection form (Form I or Self-Inspection certificate) or other documentation (e.g. auction invoice) from the prior owner.

• If the prior inspection form is from another state, a valid Certificate of Veterinary p p , yInspection (CVI) must accompany it. An entry permit number will be recorded on the CVI.

• A Bill of Sale will accompany the Form I.

Seasonal Pass

• An Inspection Form must be provided to obtain a Seasonal Pass for showing livestock:

• CattleCattle

• Form I for in-state cattle or a valid Certificate of Veterinary Inspection (CVI), including an entry permit number and official ID, for out-of-state cattle

• Sheep, Goats, or Swine

• Self-Inspection (or Form I) for in-state sheep/goats/swine or a valid CVI, including an entry permit number and official ID, for out-of-state sheep/goats/swine

• Equine

• Seasonal Pass is not required• Seasonal Pass is not required

• $0.50 per animal exceeding 10

• Example: An exhibitor is showing 1 - 5 animals

• Cost - $5.00

• Example: An exhibitor is showing 12 animals

• Cost - $5.00 + $0.50 per 2 additional head = $6.00

• Arizona no longer requires Hauling Cards or inspections of equine.

• Be sure to obtain a Bill of Sale when purchasing an equine.p g q

Importing Livestock into AZ

• All out-of-state livestock must have proof of ownership and a valid Certificate of Veterinary Inspection (CVI).

• Th CVI h i b l di i h i d• The CVI must have an entry permit number, excluding equine, that was issued to the veterinarian by the AZ State Veterinarian’s Office.

• Depending on the state of origin, the State Veterinarian might have additional irequirements.

When to Walk Away from a Purchase

• Cattle• If the owner will not provide you with Bill of Sale and a Form I which shows a transfer into

your namey• If the animal came from out of state without a valid Certificate of Veterinary Inspection

• Sheep/Goats/Swine• If the owner will not provide you with a Bill of Sale and a Self-Inspection certificate (or

What if I Purchased Livestock without Proper Documentation?

• Cattle• Contact the previous owner for a Bill of Sale and request them to obtain a Form I. If they will not, contact
