Loch Haven Ob/Gyn Group (Department 7780) Loch Haven OB/GYN, serving Central Florida since 1989,

offers complete women’s health care through a team of experienced physicians, all of whom are Board

Certified by the American Board of Obstetrics and Gynecology. As leaders in both routine and advanced

obstetric and gynecological services, the physicians at Loch Haven offer the most comprehensive

women’s care available.

The Loch Haven physicians specialize in normal and high-risk pregnancies and are skilled in handling

complications during pregnancy or delivery. In case an emergency arises, at least one Loch Haven

physician is on duty in Florida Hospital Orlando 24-hours a day, every day.

In addition, the physicians at Loch Haven are highly trained in advanced gynecological procedures and

minimally invasive surgeries. They specialize in treatment for a variety of gynecological conditions,

including abnormal uterine bleeding, pelvic and vaginal pain, ovarian cysts, uterine fibroids, urinary

incontinence and more.

The community medical clinic for the last 13 years has been serving the indigent community. In the last

for the clinic to provide more than 15,000 visits serving over 5,000 people annually. Families who use

the clinic for care are assisted with prescription medications, dental care, applying for food stamps, and

other federal aid programs that meet basic needs. The Community Medical Clinic has 12 full-time staff.

Over the last five years we have added three physicians ( 2- family practice and 1- OB/GYN) as well as a

second ARNP. Additionally, nearly 70 PMC medical staff and physicians volunteer their time and

Our FMC clinic is designed to help provide medical care to the under-served portion of our population.

The payers listed above, along with Medicare, account for 76% of the patients served. Our services

include prenatal and OB services to young women including those without any health insurance when

they come to see us. Actual services provided for FY09-10 total $6,734,421; of which $3,865,606 is for

Self Pay and Medicaid/Mcd HMO patients.

The Rural Health Care Network is active in four counties involving primary care medicine. Physicians and

nurse practitioners work at four offices providing primary care in these rural, under-served counties.

Support services of these offices are maintained through the Department of Practice Management and

mental health treatment and medical follow-up for the child and family.

Kids House is privileged to be staffed with the best and brightest individuals. Thier compassion and

dedication is apparent in everything they do. Through a partnership with Florida Hospital, Kids House

provides a Physician Assistant and an Advanced Registered Nurse Practitioner that conducts forensic

medical exams. Kids House is also the home of the Seminole County Child Protection Team, staffed with

Heart Failure Clinic (Department 7264) The Heart Failure Clinic (HFC) was established in March 2006 by

the Florida Hospital Cardiovascular Institute to provide underserved patients with access to specialty

care for chronic heart failure management. Serving metropolitan Orlando’s 1.8 million residents, the HFC

provides free office visits, medications, diagnostic testing and social services until patients are connected

with community resources. The HFC has provided 2,097 patient visits since opening.

According to Workforce Central Florida, 7.7 percent of the area’s population is unemployed. That

translates into many people without insurance, and that number is growing. The trend is being felt at

the Heart Failure Clinic where in just the last year, the number of patients they serve has grown by more

than 250 percent. Clinic nurse practitioners say it is vital to have regular medical checkups for those who

have cardiovascular disease.

“We treat patients at the Heart Failure Clinic so they don’t go back into the hospital and can actually

improve their health and quality of life,” said Cindi Hayes, Cardiology Nurse Practitioner.

The Heart Failure Clinic serves patients with cardiovascular disease who have no insurance or are a non-

To improve the health of our younger residents, we provide registered nurses (RN) to area primary,

middle and high schools in our district. The presence of a RN provides expanded health capability within

the schools increasing school attendance by keeping students in school and allowing low income

students an opportunity for some healthcare. The RN in the school is the only healthcare professional

available to many students as they are unable to afford traditional healthcare. The RN provides valuable

education and other services highly valued by the students and school district. While this program has

been in existence for several years, it has been cut in recent years due to decreased funding. For

instance, in 2007 when our LIP was double the current allocation, we served 33 schools versus the 17

Research Support for Primary Care Initiatives

Amounts reflected in this section represent contributions to the University of Florida for research in

programs with a primary care focus. As a private, not-for-profit organization adhering to state statutes,

Shands is responsible for providing support to the UF Health Science Center and other universities and

community colleges for educational and research activities that benefit residents throughout the state.

Many physicians and researchers at the UF Health Science Center are regarded as national pioneers in

researching new treatments and therapies in a variety of medical disciplines. Support for disciplines and

clincial trials with a primary care focus included in this section include: Epidemiology, Aging, Diabetes and

Program C: Free Screenings

Early detection is the key to preventing illnesses from becoming fatal.  Wuesthoff offers many free

screening opportunities to area residents through health care events, such as the Men’s Health Summit,

In response to the lack of available care, The Children’s Trust, Miami-Dade Public Schools, Miami-Dade

County Health Department and ten partners (PHT + other providers) established HealthConnect in Our

Schools. The program was launched in 2006 and is now in 132 schools; 16 school based health clinics are

managed by JHS. The health teams comprised of a .5 nurse, .5 social worker and a PCAs at each school,

which provides services to meet children’s immediate school health needs and help connect them to a

medical home for ongoing medical care and community services.

Homeless Shelter

Both the South Dade and Miami Hope Homeless Assistance Centers have Jackson operated clinics within

the homelss center, which is operated by the Community partnership for the Homeless. We provide

primary care only to the clients that are staying in the shelter complex. Each clinic is staffed by a family

practice ARNP that treats both adults and children. Miami Hope is grant funded; South Dade Homeless

Center is currently funded by Jackson Memorial Hospital. We see approximately 550 clients monthly
