Accident Prevention & Emergency Response
Procedures QEPO 14 “Emergency Preparedness Procedure” and QEPO 14- 1 “Emergency Preparedness Procedure Outside Normal Working Hours” (as part of risk management) identifies potential accident and emergency situations, and the appropriate responses to minimise potential environmental impacts. The procedures are reviewed for adequacy at least annually as part of the management review, and after the occurrence of any incident. The procedure QEPO 14-2 “Evacuation Procedure” details the procedure on hearing the fire alarm.

1. PURPOSE
To define how P. CARNEY LTD. identities the potential for accident and emergency situations, how it responds to such situations and how it prevents and mitigates any environmental impacts that may occur.
2. SCOPE
This procedure applies to P. CARNEY LTD’s site at Crossakiel, Kells, Co. Meath.
3. PROCEDURE
Potential Accident and Emergency Situations
The following potential accident or emergency situations could result in damage to human health, the environment or property and must be prevented or contained. If because of one of these situations, there is a likelihood of damage to human health or the environment then the Quality & Environmental Manager or the Works Manager should be contacted immediately.

Emergency Preparedness & Response
3.2 Emergency Response Co-ordinator
The Works Manager is the designated primary Emergency Response Co- ordinator (and Fire Officer). In the absence of the primary co-ordinator, a deputy co-ordinator (the Maintenance Manager) will take responsibility to stand in for the co-ordinator. At least one other member of the Emergency Response Team will be on-call when work is being done on site.
3.3 Discovering an Emergency
When an employee discovers an emergency (re, section 3.0 of the procedure) and cannot immediately correct or safely contain it then he must contact the emergency co-ordinator or a member of the emergency response team.
The emergency response co-ordinator should try contacting the Quality & Environmental Manager or the Managing Director to notify them of the emergency. In the non-availability of the Quality & Environmental Manager and the Managing Director, the emergency response co-ordinator has the authority to immediately commit significant resources to protect human health and the environment in the case of an emergency.
3.4 Emergency Services and Agencies
The Emergency Response Co-ordinator is authorised to contact or request external emergency services as required or appropriate. The Emergency Response Co-ordinator, the Quality & Environmental Manager or the Managing Director will notify all emergency services and agencies that are stipulated by law in the event of a serious emergency. Follow up notification, communication and reporting of serious environmental incidents will be the responsibility of the Quality & Environmental Manager with the assistance of the Emergency Response Co-ordinator.
3.5 Accident and Emergency Response
If the person who discovers it cannot deal with a fire or accident immediately and easily then the Emergency Response Co-ordinator will be notified immediately. He will assess the risks and hazards of the incidents and will take

some of the following steps where appropriate and in proportion to the seriousness of the incident.
Step 1: Internal notification and raising the alarm Notify senior management Call the emergency response team Sound the fire alarm Arrange for someone to phone the emergency services if very serious
Step 2: Evacuate the area in danger Move immediate personnel back a safe distance Put on safety equipment if necessary Close off the affected area; evacuate the area or building Call the fire brigade if there is a fire, which cannot be dealt with internally
Step 3: Stop or isolate at source Press emergency stop button/shut off power If safe, directly stop at the source. Stop spill, leak, machine, shut off oil or gas, etc.
Step 4: Contain the damage If the fire brigade is on site and there is the possibility of contaminated fire- water leaving the site, it is the responsibility of the Emergency Response Co- ordinator to close the shut-off valve to the surface water drainage system. Identify contaminated materials, parts and area Try to hold damage within area, seal in Apply first aid Put high risk material into a sealed container Neutralise or treat spill, absorb, bund Prevent from rapidly dispersing, block drains, doors etc. Take all unsafe equipment out of service Divert or move to a safer area Notify neighbours, local authorities environmental agencies etc

1. PURPOSE
To define the procedure to be followed should an emergency situation occur outside normal working hours (8 am to 5 p.m., Mon.-Thu, 8 am to 4 p.m. Fri.)
2. SCOPE
This procedure covers emergency situations, which might occur, outside normal working hours. Such situations are identified as follows: -
Accident occurring causing injury Absenteeism leaving only one person on the site (this does not apply to security personnel, a person heating the furnaces, a person cleaning the furnaces, or office cleaning staff) Fire (which cannot be controlled with extinguishers) Breakdown
3. PROCEDURE
3.1 Accident First aid boxes are located in the following areas: -
Furnace Area System 2 & 3 Laboratory
3.1.1 Minor If the injury is considered to be very minor e.g. small cut or scrape, particle of dust in the eye etc., then it may be possible for the injured person or his colleague to treat the injury using the necessary materials from one of the First Aid boxes.
Such accidents must be reported to the Health & Safety Co-ordinator or the Works Manager when normal working hours resume, i.e. the following morning.

3.1.2 More Serious If the injury is deemed to be more serious e.g. bad cut, etc., a First Aid Officer may be required to treat the injury. If a First Aid Officer cannot be contacted then Dr. Lapin @ells), the Company Doctor, can be contacted.
In this case, the injury must be reported to the Health & Safety Co-ordinator or the Works Manager
3.1.3 Serious If the accident is of a serious nature, e.g. bad burn, broken limb etc., the Company Doctor should be contacted immediately. If the injured person or his colleague thinks an ambulance is required, then the Ambulance service at our Lady’s Hospital, Navan should be contacted. The Managing Director, Health & Safety Co-ordinator or the Works Manager must be contacted.
3.2 Absenteeism If one or more of the employees fail to turn up for work and an employee finds themselves in a situation where s/he is the only person on the site, then s/he must report this immediately by phone to either the Health Health & Safety Co-ordinator or the Works Manager. Every effort will be made to ensure that there are at least two people on site at any particular time.
3.3 Fire In the event of a fire breaking out on site, which cannot be controlled with fire extinguishers, Kells Fire Brigade should be contacted immediately. All employees must evacuate the area immediately, as described in QEPO 14-2 “Evacuation Procedure”. The Emergency Response Co-ordinator must be contacted immediately should this situation occur.
3.4 Breakdowns In the event of a breakdown, which may give rise to a safety hazard, the Health & Safety Co-ordinator must be contacted. All other breakdowns, which will interrupt production, must be reported immediately to the Maintenance Manager.

PURPOSE
To define the procedure to be followed on hearing the fire alarm.
SCOPE
This procedure applies to all personnel except for the members of the emergency response team as outlined in QEP 0 14, unless otherwise stated.
RESPONSIBILITY
The Quality and Environmental Manager or delegate is responsible for the formulating, reviewing and updating this procedure in conjunction with relevant personnel,
EMERGENCY RESPONSE TEAM
The emergency team are exempt from complying with this procedure as they have the responsibility for minimising the risk to the environment during an emergency situation. However, they must comply with this procedure, if it possible for them to endanger themselves or others, when performing their duties to minimise the risks to the environment.
PROCEDURE
On hearing the fire alarm:-
(4 Each person will cease work immediately, turn off their machine(s) where relevant, if it is safe to do so (except for the CIMP’s, Granulator’s and Rasper’s in the granulation area).
@I Furnace operators will ensure that the furnaces are not in a tilted position and that oil and gas are turned off at the main shut off valves if it is safe to do so.

(4 All persons will leave their work area immediately and go as quickly as possible to the assembly point, which is located inside the main gate.
(4 It is the responsibility of the Emergency Response Co-ordinator or a member of the emergency response team to check that all employees have evacuated their work areas and are present at the assembly point.
(4 No person is permitted to return to their work area until they receive authorisation from the Emergency Response Co-ordinator or another member of the emergency response team.
6. EVACUATION DRILL
This procedure shall be practised twice per year. It is the responsibility of the Emergency Response Co-ordinator to perform these drills. The Emergency Response Co-ordinator shall maintain appropriate records for all cases of evacuation.
