NICOTINE REPLACEMENT THERAPY (NRT)SCTMS has contracted with Envolve, a major pharmacy network, to be able to provide specific smoking cessation medications and nicotine replacement therapies (NRTs), such as Nicorette, to you at no cost. In order for the cost of approved medications to be covered by the Program, you must utilize a pharmacy that is included in the Envolve Network. If you choose to utilize a pharmacy outside of the Envolve Network for smoking cessation medications, those medications will not be eligible for reimbursement.

TELEPHONE QUIT LINESAs a member of the Program, you have access to smoking cessation telephone Quit Line services at no cost. The Quit Line can put you in touch with a number of FREE resources to help you quit smoking. You will be paired with your personal Quit Coach who will help you assess your tobacco use and create a personalized program aimed at helping you quit!

SCT Management Services Overview — How Does This Work? This is a Smoking Cessation Program that provides specific, court-approved smoking cessation services FREE to you.

These services are provided and arranged by SCT Management Services (SCTMS) as the administrator for Smoking Cessation Trust. The Program is not an insurance plan, so there are no cost-sharing amounts, dues or fees required to access the services such as copayments, deductibles or coinsurance, as long as you see one of our contracted providers.

OFFICE VISITS & COUNSELINGAs a member of the Program, you have access to contracted providers to seek smoking cessation services at no cost to you. A complete list of available providers can be found online at :www.gilsbar360alliance.com/provider-search

If you received services from a provider that has not contracted with SCTMS or is not participating in the Gilsbar 360° Alliance PPO Network, your services will not be eligible for reimbursement by SCTMS. You can advise the provider to consider joining the Gilsbar 360° Alliance PPO Network by calling (866) 605-6797 and a Gilsbar representative will be happy to assist with this process.

What Resources Are Covered Under The Program?

Sometimes you need someone on your quit team you can talk to, help you make a plan and cheer you on along the way.

When you partner with a coach, you’ll work together to come up with a personalized plan to tackle your quit goals. You’ll work directly with and expert that knows how to help people quit. You’ll have 15-minute calls, about once per month, to get answers to your questions, support in overcoming obstacles and help in reaching your goals. You’ll also set small steps to work on between coaching sessions

When you with a coach, you’ll receive: • Free and personalized coaching sessions • A customized program designed for your specific goals • Access to all of the programs offered through the Smoking Cessation Trust

There are mandatory generic substitutions on certain medications and limits to the availability of the approved smoking cessation medications listed below. All medications require a valid prescription from your physician in order to have it filled. Please see the chart below for a complete list of the eligible medications covered under the Program.

Mandatory Generic Substitution Program – Mandatory Generic Substitution means that a brand medication will not be covered if a generic equivalent is available.
