~tLSUDDENCARDIACARREST (SCA)AWARENESSFORMThe Basic Facts onSudden Cardiac Arrest

Website Resources:American Heart Association:

Lead Author: Arnold Fenrich, MDand Benjamin Levine, MD

Additional Reviewers: UIL MedicalAdvisory Committee

Revised 2016

What is Sudden Cardiac Arrest?~ Occurs suddenly and often without

warning.~ An electrical malfunction (short

circuit) causes the bottom chambersofthe heart (ventricles) to beatdangerously fast (ventriculartachycardia or fibrillation) anddisrupts the pumping ability of theheart.

~ The heart cannot pump blood to thebrain, lungs and other organs of thebody.

~ The person loses consciousness(passes out) and has no pulse.

~ Death occurs within minutes ifnottreated immediately.

What causes Sudden Cardiac ,Arres~ ,Inherited (passed on from family)conditions present at birth oftheheart muscle:Hypertrophic Cardiomyopathy -hypertrophy (thickening) of the leftventricle; the most common cause ofsudden cardiac arrest in athletes inthe U.S.

Arrhythmogenic Right VentricularCardiomyopathy - replacement ofpart of the right ventricle by fat andscar; the most common cause ofsudden cardiac arrest in Italy.

Marfan Syndrome - a disorder of thestructure of blood vessels that makesthem prone to rupture; oftenassociated with very long arms andunusually flexible joints.

Inherited conditions present at birthof the electrical system:Long QTSyndrome - abnormality inthe ion channels (electrical system) ofthe heart.

Catecholaminergic PolymorphicVentricular Tachycardia andBrugada Syndrome - other types ofelectrical abnormalities that are rarebut run in families.

Nonlnherited (not passed on from thefamily, but still present at birth)conditions:Coronary Artery Abnormalities -abnormality of the blood vessels that ~supply blood to the heart muscle. This ~is the second most common cause ofsudden cardiac arrest in athletes inthe U.S.

Aortic valve abnormalities - failureof the aortic valve (the valve betweenthe heart and the aorta) to developproperly; usually causes a loud heartmurmur.

Non-compaction Cardiomyopathy -a condition where the heart muscledoes not develop normally.

Wolff-Parkinson-White Syndrome -an extra conducting fiber is present inthe heart's electrical system and canincrease the risk of arrhythmias.

Conditions not present at birth butacquired later in life:Commotio Cordis - concussion of theheart that can occur from being hit inthe chest by a ball, puck, or fist.

Myocarditis - infection orinflammation of the heart, usuallycaused by a virus.

Recreational/PerformanceEnhancing drug use.

Idiopathic: Sometimes the underlyingcause of the Sudden Cardiac Arrest isunknown, even after autopsy.

What are thesymptoms/warning signs ofSudden Cardiac Arrest?

Fainting/blackouts (especiallyduring exercise)

~ Dizziness~ Unusual fatigue/weakness~ Chest pain~ Shortness of breath

Nausea/vomitingPalpitations (heart is beatingunusually fast or skipping beats)Family history of sudden cardiacarrest at age < 50

ANYof these symptoms and warningsigns that occur while exercising maynecessitate further evaluation fromyour physician before returning topractice or a game.

What is the treatment torSudden Cardiac Arrest?Time is critical and an immediateresponse is vital.~ CALL911~ Begin CPR~ Use an Automated External

Defibrillator (AED)

What are ways to screen forSudden Cardiac Arrest? The American Heart Associationrecommends a pre-participation historyand physical including 14 importantcardiac elements.

The UlLPre-Participation PhvsicalEvaluation - Medical History formincludes ALL14 of these importantcardiac elements and is mandatoryannually.

What are the currentrecommendations for screeningyoung athletes? The University Interscholastic Leaguerequires use of the specificPreparticipation MedicalHistory formon a yearly basis. This process beginswith the parents and student-athletesanswering questions about symptomsduring exercise (such as chest pain, dizziness, fainting, palpitations orshortness of breath); and questionsabout family health history.

It is important to know if any familymember died suddenly during physicalactivity or during a seizure. It is alsoimportant to know if anyone in thefamily under the age of 50 had anunexplained sudden death such asdrowning or car accidents. Thisinformation must be provided annuallybecause it is essential to identify those atrisk for sudden cardiac death.

The University Interscholastic Leaguerequires the Preparticipation PhysicalExamination form prior to junior highathletic participation and again prior tothe 1st and 3rd years of high schoolparticipation. The required physicalexam includes measurement of bloodpressure and a careful listeningexamination of the heart, especially formurmurs and rhythm abnormalities. Ifthere are no warning signs reported onthe health history and no abnormalitiesdiscovered on exam, no additionalevaluation or testing is recommendedfor cardiac issues/concerns.

Are there additional optionsavailable to screen for cardiacconditions?Additional screening using anelectrocardiogram (ECG)and/or anechocardiogram (Echo) is readilyavailable to all athletes from theirpersonal physicians, but is notmandatory, and is generally notrecommended by either the AmericanHeart Association (AHA)or theAmerican Collegeof Cardiology (ACC).Limitations of additional screeninginclude the possibility (-10%) of "falsepositives", which leads to unnecessarystress for the student and parent orguardian as well as unnecessaryrestriction from athletic participation. There is also a possibility of "falsenegatives", since not all cardiacconditions will be identified byadditional screening.

When should a student athletesee a heart specialist? If a qualified examiner has concerns, areferral to a child heart specialist, apediatric cardiologist, is recommended. This specialist may perform a morethorough evaluation, including anelectrocardiogram (ECG),which is agraph of the electrical activity of theheart. An echocardiogram, which is anultrasound test to allow for directvisualization of the heart structure, mayalso be done. The specialist may alsoorder a treadmill exercise test and/or amonitor to enable a longer recording ofthe heart rhythm. None of the testing isinvasive or uncomfortable.

Can Sudden Cardiac Arrest beprevented just through properscreening?Aproper evaluation (PreparticipationPhysical Evaluation - Medical History)should find many, but not all, conditionsthat could cause sudden death in theathlete. This is because some diseasesare difficult to uncover and may onlydevelop later in life. Others can developfollowing a normal screening evaluation, such as an infection of the heart musclefrom a virus. This is why a medicalhistory and a review of the family healthhistory need to be performed on a yearlybasis. With proper screening andevaluation, most cases can be identifiedand prevented.

Why have an AED on site duringsporting eventsThe only effective treatment forventricular fibrillation is immediate useof an automated external defibrillator(AEO).AnAEOcan restore the heartback into a normal rhythm. AnAEOisalso life-saving for ventricularfibrillation caused by a blow to the chestover the heart (commotio cordis).

Texas Senate Bill7 requires that at anyschool sponsored athletic event or teampractice in Texas public high schools thefollowing must be available:

};> AnAEOis in an unlocked locationon school property within areasonable proximity to the athleticfield or gymnasium

};> Allcoaches, athletic trainers, PEteacher, nurses, band directors andcheerleader sponsors are certified incardiopulmonary resuscitation(CPR)and the use of the AEO.

};> Each school has a developed safetyprocedure to respond to a medicalemergency involving a cardiacarrest.

The American Academy of Pediatricsrecommends the AEOshould be placedin a central location that is accessibleand ideally no more than a 1 to 1 1 /2minute walk from any location and thata call is made to activate 911 emergencysystem while the AEOis being retrieved.
