Autologous or Adult stem cells (ASCs) are the "multipotent" cells which are found throughout the body.

These cells do not involve embryos and is less controversial as compared to pluripotent cells.

Their ability to differentiate is more limited than pluripotent cells but is vast nonetheless.

The NIH’s  site provide a list of around 4500 ASC trials as compared with 27 for embryonic stem cells and 21 for induced pluripotent stem cells.

Treatable diseases using autologous stem cells

The diseases which are being treated nowadays using autologous stem cells mostly related to damage or destruction of bone marrow which includes diseases such as non-Hodgkin's lymphoma and Hodgkin's lymphoma, leukemia, multiple myeloma, and aplastic anemia.

Restoration of bone marrow is being done that has been damaged by total body radiation and high doses of chemotherapy used for cancer treatment.

Apart from above diseases, these cells have also been used for the treatment of diabetes, Parkinson's disease, sickle cell disease, and thalassemia.

Source of stem cells and the process of transplantation

The source of stem cells used in transplants can be :

Autologous stem cells - Patient receives stem cells from his own blood. 

Allogeneic stem cells - Patient receives stem cells from a donor. 

*The donor used in an allogeneic stem cell transplant can be the patient's identical twin, sibling, family member or an unrelated donor.

For Autologous transplants physicians usually collect the stem cells from the peripheral blood rather than the bone marrow cells.

The growth factor G-CSF may be used to stimulate the growth of new stem cells so they spill over into the blood.

*G-CSF is a protein that is produced naturally in the body.

The blood is removed from a vein and passed through a machine that separates the stem cells. The machine then returns the remaining blood through a needle in the person's arm or through a central venous catheter. This way of collecting stem cells is called apheresis.

In adults, most autologous transplants use stem cells from blood. In a child, the decision whether to use cells from the bone marrow or the blood depends on the size of the child.

Advantages of adult stem cells over autologous transplantations

In an autologous stem cell transplant procedure, the body recognizes the self- cells and does not lead to a condition known as Graft-versus-Host Disease (GVHD).

Treatment process does not require immunosuppressant's and histocompatibility matching.

The process of finding a donor for stem cell treatment is not required as the stem cells are taken from the patient itself.

Autologous blood is readily available, and immune reconstitution is more rapid than after allogeneic. 

Autologous transplants are relatively safe procedures, with low rates of complications and infections compared with other type of transplants.Treatment related mortality rate is 2% to 3% in patients in most of the studies.

Disadvantages of adult stem cells over autologous transplantationsApart from having a lot of benefits as compared to other types of transplantations, Autologous stem cells have some disadvantages as well:

Stem cells after transplantation may get infected with tumor cells and contribute to relapse of the tumor condition.

Autologous transplants cannot be used for genetic or acquired diseases in which the hematopoietic cells are injured or defective. 

Prior treatment with certain chemotherapy drugs or with radiation therapy to the spine or pelvis may reduce the number of stem cells that can be collected for an autologous transplant in certain cases.

Possible marrow injury leading to myelodysplasia (Either from prior chemotherapy or transplant regimen).
